2005 Exemplary Homeless Program Nomination

The Homeless Programs Branch of the Center for Mental Health Services (CMHS) is seeking nominations for the 2005 Exemplary Program Awards. The purpose of this initiative is to identify and recognize programs that are using evidence-based and promising practices to serve individuals and families who are homeless and have mental illnesses and/or substance use disorders.

Nominations will be compiled and rated by an independent panel. Each selected program will be featured in a brief program description that will be broadly disseminated nationwide. Awards will be made at SAMHSA's National Training Conference Addressing Homelessness for People with Mental Illnesses and/or Substance Use Disorders to be held October 26-29, 2005, in Washington, DC. Travel expenses will be paid for one representative from each selected program to attend the conference.

Of particular interest are programs that demonstrate: use of one or more identified evidence-based or promising practices, linkage with or use of mainstream resources, evidence of addressing homelessness among individuals and families with mental illnesses and substance use disorders, and positive client outcomes.
