Administration on Aging (AoA) Grant Announcement: Empowering Older People to Take More Control of Their Health Through Evidence-Based Prevention Programs

The U.S. Department of Health and Human Services (HHS) through AoA, will award competitive grants to States to gain experience in how States can play a leadership role in mobilizing public/private partnerships at the State and community level to implement and sustain the delivery of evidence-based prevention programs at the community level though non-profit aging services provider organizations. Participating States will be expected to begin implementing programs in at least 3 geographic areas of their States. Participating States will also be expected to begin developing the infrastructure and partnerships that will be necessary over the long-run to effectively embed evidence-based prevention programs for the elderly within their statewide systems of health and long-term care.
A special feature of this current program initiative is that HHS is being joined by The Atlantic Philanthropies to greatly expand the scope and reach of the current grant opportunity, with particular emphasis on the deployment of the Stanford University Chronic Disease Self-Management Program (CDSMP). Through a grant to the Center for Healthy Aging at the National Council on the Aging, The Atlantic Philanthropies has generously agreed to provide funds and technical assistance to up to five grantees that show significant potential in developing State systems that will be able to reach large numbers of older adults with the CDSMP. The Center for Healthy Aging at NCOA, which is also serving as AoA's national resource center on evidence-based prevention, will select, on behalf of The Atlantic Philanthropies, the State programs to receive the awards during the latter half of the first year of the AoA grants. wards. The total investment over the life of the project for this entire effort by HHS, the participating grantees, and The Atlantic Philanthropies will exceed twenty million dollars.

Award Size:
AoA plans to fund up to 12 grants totaling up to $300,000 per project for each year for a three year period. Grantees are required to make a non-financial or cash recipient contribution (match) of at least twenty five percent (25%) of the total grant award.
