"Achieving Transformation Through Shared Leadership and Knowledge"

Theme: Transformation currently is a driving force throughout the entire mental health field. The major goal is to create a recovery and resiliency oriented care system that is consumer and family focused. This Conference will represent a departure from the past. The Conference will explore mental health transformation at the local, State, national, and international levels. The local frame will examine systems of care for children and adults, in both the public and private sectors. The State frame will focus on integrated service delivery systems that incorporate community and hospital care, and that involve the full range of State agencies and private sector services. The national frame will address the Center for Mental Health Services (CMHS) action plan and key linkages with other Federal agencies. The international frame will explore examples of transformation from other countries, such as Italy, Australia, Canada, and others. Key features to be explored at each of these levels will be shared leadership through new partnerships, and the sharing of knowledge about successful transformation strategies.

State Work Groups: A primary activity during this Conference will be work by State teams to discuss current and future transformation activities. The goals outlined by the President's New Freedom Commission on Mental Health and the strategies defined in the crossing the quality chasm framework will be used to orient this work. Transformational leadership and personal transformation will be explored as part of this process

Conference Schedule: The Conference schedule is as follows: Tuesday, May 30, participants will arrive, and optional pre-conference workshops will be conducted. Wednesday, June 1, the Conference will convene, and exploration will begin at each of the levels described above. Thursday, June 2, State work groups will share their transformation strategies. Friday, June 3, work from the Conference will be shared and integrated. The Conference will adjourn by noon.

Solicited Papers: Because of the working nature of this Conference, only a minimum number of volunteer papers will be accepted for presentation. However, volunteer papers will be solicited broadly. Papers not accepted for presentation will be made available to Conference participants electronically.

For the data and technology track, presentations are sought that exemplify partnerships between transformation and data activities; incorporate key performance measures that foster change; implement decision support models, especially DS2K+; employ information technology for service delivery, record-keeping, and consumer/family empowerment.

Proposals for Presentations

Proposals for presentations must include the following:

_A complete proposal cover form

_An abstract - limited to one page and not to exceed 200 words

_A half page biographical sketch for the primary presenter and each secondary presenter

_A labeled diskette for the abstract in MS Work (preferred), WordPerfect, or Rich Text format.

Each presenter and secondary presenter will be given a combined total of 30 minutes for his or her presentation, which includes time for audience participation. Two such presentations will be included in each breakout session. Presenters are encouraged to include a mental health consumer/survivor family member, and a broad diversity of presenters from State Planning Councils. All proposals will be reviewed to determine their appropriateness for inclusion in the programs.

Proposals must be postmarked no later than January 14, 2005. Please submit only one proposal per person/group. Faxed proposals will not be accepted. Notification of acceptance will be sent on or about February 28, 2005.

Last year's joint conference was attended by more than 650 persons, including State planners, data experts, consumers and family members who are mental health planning council members, and State and Federal program officials.

PLEASE NOTE: Proposals not accepted for verbal presentation will also be considered for electronic presentation. Those accepted for electronic presentation will be notified by February 28, 2005.

A final electronic presentation will need to be submitted no later than April 29, 2005, as a Power Point Presentation.
