CMS CONFERENCE:
BUILDING SYSTEMS, SO THAT MONEY FOLLOWS THE PERSON

March 2-3, 2004

This conference, sponsored by the Centers for Medicare & Medicaid Services (CMS), is targeted to Consumers, Real Choice Systems Change, Aging and Disability Resource Center, Ticket to Work Medicaid Infrastructure, and Direct Service Community Workforce grantees; staff from State Medicaid, Health, Mental Health, Developmental Disabilities, and Aging Agencies; staff from advocacy organizations; researchers; providers; state legislators; Independent Living Centers.

This event is intended to showcase grantee efforts to improve services and supports for individuals with disabilities and long-term illness, and to facilitate the sharing of lessons learned across states. The conference will also highlight important CMS initiatives and New Freedom Initiative activities across Federal departments.

There is no conference registration fee.

Registration Application Deadline: Friday, January 30, 2004

All registration applicants will receive a confirmation of registration as soon as possible.

This conference is fragrance-free. Please refrain from using perfumes, colognes, or scented personal products at the conference.

Scholarship Information

Limited funds are available to provide financial assistance for hotel, travel, personal care attendants and other accommodations to consumers/self-advocates who want to attend this event.

Scholarship requests must be received by December 31, 2003.
