Project TAPESTRY enhances Cuyahoga County's current systems of care efforts by increasing mental health service access and capacity. This project merges a premiere mental health case wraparound model (the PEP - Positive Education Program Connections) with a pioneering child welfare reform initiative (the Annie E. Casey Foundation's Family-to-Family initiative). The welfare initiative mobilizes families at a neighborhood level to work with professionals in order to improve child outcomes. Four high-poverty neighborhoods will be demonstration sites with increased access to an expanded array of high quality mental health services. Connections will provide the vehicle to link children and families to these services, including a menu of evidence-based treatments that will be developed as a direct result of this project. Both the schools and neighborhood partners will serve as primary referral sources for youth with SED, linking the education and child welfare systems for mental health services. The Cuyahoga County Family and Children's First Council (FCFC) is the organizing entity behind systems of care development. FCFC is spearheading the Strengthening Communities Youth (SCY) project through SAMHSA' s Center for Substance Abuse Treatment, which concentrates systems of care development through the substance abuse and juvenile justice systems. TAPESTRY expands and complements SCY by adding resources to the child welfare and education systems. Each system's reform efforts are linked together via their connection to the FCFC's system coordination plan. TAPESTRY has been designed to extend the influence of two other federal initiatives, Project SYNERGY!; a Safe Schools/Healthy Students initiative whose funding concludes in October 2003, and TDM Connect, a child welfare initiative currently under consideration for funding. Finally, the 6,400 youth with SED and their families currently being served will receive more effective services.

In the past two decades, Ohio's public mental health system has led the way to achieve reform and promote recovery. Now Ohio will build on that track record to achieve Mental Health Transformation. With strong leadership, the active involvement of all shareholders and support from the Mental Health System Transformation Grant, Ohio will knit together a wide range of resources and collaborations to achieve deeper change. Ohio approaches transformation as individuals and families approach recovery- as a meaningful process of internal change that requires strong external support. Ohio's resources include strong mental health leadership; effective advocacy groups; a demonstrated commitment to recovery, resiliency and cultural competence; nationally recognized approaches to promoting best/evidence-based practices, assessing outcomes, and evaluation research. Via this grant, Ohio will fully orient the mental health system and other systems to recovery, resiliency and culturally competent practices. Our goal and outcome is clear: children, families and adults will live their lives fully in their communities. Youth and adult consumers and families will be engaged in activities that affect their recoveries and service experiences, including person-centered planning, peer support and recovery/resiliency-oriented policy making. With the TSIG grant, Ohio will develop several new program approaches (i.e., trauma care for children and adults, older adult services and mental health supports for offenders reentering communities); integrate current policies and programs to emphasize person-centered planning, peer support and cultural competence; and develop the infrastructure to ensure effective collaboration and communication among all shareholders.

Strategoies may include: changing local attitudes and norms, and re-evaluating existing laws and policies. (1) Grantee must participate in national evaluation activities of the STOP grant program. (2) STOP Grantees must use the Strategic Prevention Framework (SPF), a five step evidence based process for community planning and decision-making. The five step rocess includes: needs assessment, capacity building, planning, implementation and evaluation. (3) STOP grantees must plan and implement a comprehensive approach inclusive of multiple strategies as emphasized in the 2007 Surgeon General's Call to Action to prevent and Reduce Underage Drinking located online at: http://www.surgeongeneral.gov/topics/underage drinking/calltoaction.pdf Emphasis should be given to environmental strategies that incorporate prevention efforts aimed at changing or influencing community conditions, standards, institutions, structures, systems and policies. In addition, grantees must select strategies that lead to long term outcomes. (4) STOP grantees must enhance, not supplant, effective local community initiatives for preventing and reducing alcohol use among youth. For current Drug Free Community grantees, STOP ACT foods can not be used to supplant or replace activities that are presently being supported by Drug Free Comunity funds, and , separate DFC and STOP ACT accouting systems must be maintained for the purposes of reporting.

The Hamilton County Drug Court is going to increase capacity and enhance services offered to their program participants. The program serves 4th and 5th degree adult felony offenders with drug driven charges. The number of offenders coming through the Court has increased and the treatment programs are not able to meet the demands without increasing their resources. There are currently 100 drug court referrals waiting to receive a chemical dependency assessment and another 100 clients who have been assessed and are waiting for an opening in the intensive outpatient level of care. The project will create an expansion to and enhancement of services to add to its already successful range of treatment services to the drug court offender. Treatment capacity will be expanded annually by 130 intensive outpatient clients and enhancements through the implementation of an evidence based assessment/pre-treatment model and clinical oversight and supervision to the program. The enhancements will include services by a Family Therapist and a Clinical Coordinator. Over the life of the grant, at least 390 additional clients will be served.

Wright State University through the Consumer Advocacy Model (CAM) program, an outpatient alcohol, drug, and mental health provider for persons with disabilities, proposes to address these needs through the creation of an e-therapy/community networks approach for Deaf persons referred to SUD treatment in Ohio. Over a period of three years, a minimum of 50 persons per year will be served through the state collaborative project that will both enhance the treatment experience and expand the number of Deaf individuals served. The proposed model will be based on knowledge currently available on best practices for this population, and it will include several components: 1. Individuals will be served in the "least restrictive environment" throughout Ohio in a combination of locally-available treatment that is augmented by e-therapy delivered via the CAM Program located in Dayton, Ohio. 2. Standard practices and levels of care will be enhanced through e-therapy that will supplement and strengthen the treatment episode, with an emphasis on learning about sustaining recovery. Group and individual counseling and support, and case management will be offered via electronic means, primarily though video conferencing and video phone technology. 3. All Deaf-specific services offered by CAM will be provided by individuals who come from a background in "Deaf culture", such as persons who are Deaf, children of Deaf adults (CODA), or Deaf services career professionals. Consumer feedback and guidance from a project Advisory Board will guide program content and service delivery.
Enhanced services will be coordinated by a treatment plan shared with the primary community provider and CAM. The treatment episode will be expanded to include "recovery education" that will involve engagement with self-help support meetings (AA/NA) that will be held regionally via video conferencing. Sorenson video phones, Polycom video conferencing equipment, and Cprint options.

This program will fill a gap in the substance abuse treatment continuum for Greater Cleveland Drug Court participants who are between the ages of 18-25, who are users of cannabis. Culturally competent case management services will be combined with outpatient Cannabis Youth Treatment therapy to enhance services and hopefully break the cycle of criminal behavior for these young offenders. Young, cannabis using offenders exhibit consistently low success rates within the context of Drug Court. Programming will be enhanced to provide closer monitoring and coaching along with substance abuse treatment targeted at this age group. The primary goal of this project is to improve graduation rates among this population, along with improvements in abstinence from drug use; increased employment and education, social connectedness, and housing along with reductions in the negative consequences of substance abuse, risky sexual behaviors, and criminal involvement. A total of 150 people will be served during the project period: 30 in Year 1, 60 in Year 2 and Year 3.

Ohio's ATR program will serve adults involved with the criminal justice system in four Ohio counties. The project will significantly increase the involvement of faith-based and community organizations as substance abuse service providers. A web-based voucher payment system and case management process will significantly increase client choice among culturally responsive treatment and recovery support services options. The target population will be adults in the criminal justice system in four urban Ohio counties: Cuyahoga, Franklin, Stark, and Summit. Clients will be diagnosed as substance abusers, substance dependent or substance abuse in remission. The majority of clients will be single, adult males, who are likely African American, with few resources and supports in the community as they attempt to enter recovery and reintegrate into society.

The purpose of Ohio's STAR-SI initiative is to promote state-level implementation of research-based Process Improvement (PI) activities to ensure accessible, efficacious treatment services for Ohio citizens. Ohio's plan for strengthening treatment access and retention incorporates and builds upon existing structures, processes and partnerships. Ohio Department of Alcohol and Drug Addiction Services (ODADAS) is Ohio's Single State Authority for substance abuse prevention and treatment services and will be responsible for the coordination and implementation of the Ohio STAR-SI. ODADAS has selected a partnership between ODADAS, STEPS at Liberty Center, a previous NIATx grant recipient, and local boards and providers to implement this project. This unique arrangement will facilitate the coordination and sustainability of both process and outcome continuous quality improvement efforts while making the most efficient use of resources. This arrangement will also provide a statewide network to disseminate lessons learned and encourage other providers to adopt this process.
