Ranked 4th in the U.S. in expulsion of children from pre-school, Delaware will use infrastructure development and service provision to create and sustain a competent, family-driven system of care, filling a critical gap in our state's mental health system.

DSAMH and DCMHS will work to improve the quality of the data reported, fill any remaining gaps, and respond quickly to new requirements. The following goals have been developed 1) Improve Delaware's state and local mental health data infrastructure; 2)Enhance data collection efforts to meet URS reporting needs; 3) Enhance use of web-based technology to collect and report data; 4) Develop and adopt data standards that promote improved management, service quality improvement, and compliance with HIPAA requirements; 5) Continue to develop and report integrated information using the Probabilistic Population Estimation (PPE)technique.

Delaware's integrated Children's Department will use this cooperative agreement to expand statewide our capacity to identify and assess child traumatic stress and increase access to effective, community-based trauma-specific treatment. The target population is children in public child welfare, juvenile justice, and child mental health systems with acute trauma related to sexual abuse, physical abuse, or witnessing violence.The Division of Child Mental Health Services as lead agency will apply its Care Assurance Model, a unique care management system to ensure that children get the right treatment at the appropriate level of intensiveness at the right time. Goal: To increase access to and improve the quality of community-based, evidence-based trauma-specific treatment and services for children in Delaware target population. Objectives: 1) Identify, with stakeholder consensus, evidence-based practice(s) to use. 2) Pilot the evidence-based treatment statewide, evaluating results. 3) Disseminate training statewide and across service levels to enhance the quality of trauma intervention for children/families 4) Convey information about trauma issues to the public. 5) Serve as a resource to help other communities and providers. 6) Sustain trauma-focused treatment and services and the Center after the grant ends.

The Delaware Suicide Prevention Coalition's (DSPC) Project LIFE is a comprehensive, statewide suicide prevention initiative targeting youth ages 10-24. By creating new programming and leveraging existing resources, the DSPC aims to reduce negative behaviors and enhance resiliency in youth most at-risk for suicide. Project LIFE takes a public health and community-based approach to suicide prevention by identifying the broader patterns of suicidal behavior through groups and populations. Following the Guiding Principles of the National Strategy for Suicide Prevention, DSPC is designed to be a catalyst for social change, with the power to transform attitudes, policies and services. The goals of Project LIFE are to prevent suicidal behaviors by enhancing resiliency, reduce the impact of suicide and suicidal behaviors on individuals, families and communities, and improve access to and availability of prevention services for vulnerable, high-risk individuals. Literature suggests using a system-wide community approach in recognizing and referring youth to an appropriate service has been shown to markedly reduce rates of suicide. The DSPC has mobilized over thirty state and community-based agencies to develop multiple strategies to prevent youth suicide. Project LIFE focuses on using evidence-based Best Practices in the field because prevention programs that address risk and protective factors at multiple levels are more effective.

The grantee will: (1) reduce substance abuse among youth and over time, among adults by addressing factors in the community that increase the risk of substance abuse and promote factors to minimize the risk of substance abuse; (2) establish and strengthen citizen participation and collaboration among communities, nonprofit agencies, and federal, state, local, and tribal governments to support community efforts to deliver effective substance use prevention strategies for youth; (3) use the Strategic Prevention Framework of evidence based prevention strategies to assess needs, build capacity, plan, implement and evaluate community prevention initiatives; and (4) assess and report on the effectiveness of community prevention initiatives to reduce age of onset of any drug use, frequency of use in the past 30 days, increased perception of risk or harm, and increased perception of disapproval of use by peers and adults.

Delaware proposes an initiative to: 1) integrate mental health and substance abuse treatment through the development of a state level infrastructure to provide an organizational framework for planning and implementing integrated services; and 2) pilot enhancement activities that will inform the development of a comprehensive , integrated approach to serving persons with co-occurring disorders throughout Delaware. Infrastructure goals proposed for this project are: 1) statewide screening for co-occurring disorders across four systems; 2) training and credentialing of professionals; 3) implementation of evidence-based practice (EBP) integrated dual diagnosis (IDDT) treatment in four pilot sites; and 4) evaluation of services. Four service pilot sites and activities will be used to examine the effectiveness of the EBP IDDT service model and an enhanced training credentialing program will be used to test the feasibility and effectiveness of treatment delivery and infrastructure enhancement strategies being developed.

The Connect to Success Services in Supportive Housing Program will provide longitudinal services to homeless persons who have substance use and mental health conditions, including immediate access to a range of housing options. Wrap-around supportive services, including integrated primary medical, substance abuse, and mental health treatment will be delivered by a mobile intensive outpatient team. The project will serve 220 homeless persons over the life of the grant, primarily adult males who are African-American and chronically homeless or at-risk for chronic homelessness.

|
Brandywine Counseling's Safety Net Services proposes to enhance and expand substance abuse treatment, pretreatment, and HIV/AIDS services in Wilmington, Delaware. The target populations are women, and ex-offenders returning to the community in the past two years. Each year 2500 outreach contacts will result in pre-treatment services to reduce client risk. 600 clients will be tested for HIV, 300 will enroll in Safety Counts, and 75 will enter treatment. The goal is to provide a safety net for any person at high risk for HIV, regardless of where they are in the continuum of readiness for substance abuse treatment. The project will partner with Christiana Care Health Services and other community-based organizations to provide medical care.These collaborations will result in a program capable of meeting the complex needs of the target population.
