University of Kentucky's Increasing Networks for Campus Awareness to Suicide and Emergencies (UK-TN-CASE) seeks systematically to create a safer and more caring campus community, to assist those at-risk for suicidal behavior, and to support those who are concerned regarding the welfare of members of the community. This project will be accomplished by using a public health model, which expands on current campus practices to create a systemic program of suicide prevention, and postvention that integrates service, policy and referral networks. Major aspects of the program include: 1. Campus Network and Policy Development in which a campus advisory group will be formed with representation from more than 15 campus departments, offices and academic units and eight community consultants, with the goal of creating a comprehensive, regularly-assessed and evidence-based campus suicide prevention plan; 2. Health Communications and Stigma Reduction that aims to utilize social marketing and educational programs for students, their families, and community members which will decrease stigma and encourage help-seeking; and 3. Gatekeeper and Clinical Training that seeks to expand gatekeeper training so that it is systematic in targeting specific populations of students, faculty and staff, and ensures that campus professionals and students in professional programs receive appropriate clinical training. Evaluation and data collection efforts will aid in providing evidence of whether grant activities are furthering best practices.

CATTI is a multidisciplinary, University-Community collaboration housed at the University of Kentucky's Center for the Study of Violence against Children. This project builds on an already nationally-recognized child assessment and treatment center in Kentucky, and proposes to develop a clinical nucleus for clinical training, and dissemination of trauma informed evidence-based practices (TI-EBPs) in four rural to urban areas in Kentucky. The CATTI project will provide three interventions that have been identified as best practice approaches, and provide a continuum of care to traumatized children: Parent Child Interaction Therapy (PCIT) for ages 2-12), Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) for ages (3-17), and Abuse-Focused Cognitive Behavioral Therapy (AF-CBT) (for school aged children).

Kentucky Partnership for Families and Children, Inc.'s (KPFC) vision is to improve the quality of life for children, youth, and young adults facing emotional, social, and behavioral challenges and to encourage the healthy functioning of their families through education, advocacy and outreach. As the family organization in Kentucky, KPFC collaborates with many state-level systems to build a strong "family-driven" and "youth-guided" system of care.
Through this next grant cycle, KPFC will continue to strengthen organizational relationships by collaborating with a multittude of public agencies and private organizations across the state on committees, projects, and collaboratives.
KPFC will teach leadership skills among youth that have SED and their families through trainings across the state, a quarterly newsletter, a resource request line, referrals to support groups, and the annual Youth/Parent Leadership Conference. KPFC will prepare these parent leaders and youth leaders to more fully participate at the community, regional, and state-levels as a family and youth voice to ensure that Kentucky has a "family-driven" and "youth-guided" system of care.

Building upon the strong interagency infrastructure of the Kentucky IMPACT Program and the school-based mental health model established through the Bridges Project, Kentuckians Encouraging Youth to Succeed (KEYS) will facilitate the implementation of a continuum of positive behavior interventions and supports in targeted schools in the North Central region of Kentucky to address the mental health and substance use needs of its youth. Recognizing the unmet needs of youth with co-occurring mental health and substance use disorders, KEYS will place particular emphasis upon employing evidence-based strategies to effectively identify and treat this challenging population. Additionally, KEYS will address identified gaps in the available service array for youth with serious emotional disabilities, including early childhood, crisis stabilization, day treatment, adolescent substance abuse, intensive home-based, and respite. A comprehensive evaluation system will be in place to support continuous improvement of the model and to support sustainability and statewide replication of the system of care.

The Commonwealth of Kentucky faces significant challenges in comprehensively addressing the needs of its children with severe emotional disturbances and their families, particularly young children age birth through five. Funds from this cooperative agreement will be used to further develop Kentucky's system of care for its youngest children and their families. A system of care is an organizational philosophy and framework that involves collaboration across agencies, families, and youth for the purpose of improving access and expanding the array of coordinated community-based, culturally and linguistically competent services and supports for children and youth with a serious emotional disturbance and their families. Guided by a theory of change, Kentucky's implementation plan will integrate the Kentucky IMPACT and Early Childhood Mental Health Programs to enhance the statewide System of Care for children age birth through five with severe emotional disabilities (SED) and their families. The State Interagency Council for Services to Children with an Emotional Disability (SIAC) will serve as the governing body for this initiative and will expand its membership to include representatives who represent agencies and/or programs who serve young children with disabilities and their families.

The purpose of Kentucky Suicide Prevention in Youth - a Collaborative Effort (SPYCE) is to raise awareness, enhance quality interventions, and utilize effective research and data collection methodologies to reduce the number of suicides completed by the youth in KY. Many activities will be implemented statewide, while others will focus within one community mental health center region to be chosen through an RFP process. SPYCE's goals are to: (1) increase knowledge of suicide warning signs, risk factors, and protective factors; (2) enhance suicide crisis intervention availability, quality, and public awareness of service; (3) increase local community collaboration in suicide prevention; (4) establish and increase support for suicide survivors through postvention and prevention; (5) increase resiliency among youth at risk; (6) enhance existing emergency department services for youth and their families; and (7) provide additional training in clinical suicide risk management to medical/mental health professionals and paraprofessionals across the state.

The Strategic Prevention Framework State Incentive Grants are used to advance community-based programs for substance abuse prevention, mental health promotion, and mental illness prevention. The SPF SIG implements a five-step process known to promote youth development, reduce risk-taking behaviors, build on assets, and prevent problem behaviors. The five steps are: (1) conduct needs assessments; (2) build state and local capacity; (3) develop a comprehensive strategic plan; (4) implement evidence-based prevention policies, programs and practices; and (5) monitor and evaluate program effectiveness, sustaining what has worked well.
These grants will allow the programs to provide leadership, technical support and monitoring to ensure that participating communities are successful. The success of the grants will be measured by specific measurable outcomes, among them: abstinence from drug use and alcohol abuse, reduction in substance abuse-related crime, attainment of employment or enrollment in school, increased stability in family and living conditions, increased access to services, and increased social connectedness.
Kentucky's Strategic Prevention Framework Incentive Grant project will build a data-driven, comprehensive, interagency prevention service system to reduce substance abuse and its related problems. The State will make extensive use of geographic information system (GIS) technology to focus community goal-setting and enable targeted resource allocation.

Westcare Kentucky, Inc. provides a 60-bed, licensed residential substance abuse treatment program for adult males in highly rural Eastern and Southeastern Kentucky at the Hal Rogers Appalachian Recovery Center in Pike County. Westcare will serve 100 adult males annually with SAMHSA funds, using the Operation UNITE voucher system in this therapeutic community setting where over 225 individuals with alcohol and other drug problems are served each year. Clients stay in treatment for a minimum of 90 days. The grant will enhance existing services to residents through cultural and gender-relevant evidence-based treatment. Clients, many with a history of criminal justice involvement, receive residential treatment as an alternative to more costly and less therapeutic incarceration, hospitalization and homelessness.

The Christian County Drug Court will enhance adult drug court services to the target population of adult offenders. Nearly 75% of drug court clients are male, and the majority is between the ages of 25-44. About one third are African American, and 84% and non-Hispanic. Most clients are unemployed and 76.5% do not have a GED or high school diploma. The Christian County Drug Court Services Enhancement project will use evidence-based strategies and interventions to provide a more comprehensive mix of services. The drug court intensive outpatient program (IOP) will expand its hours from seven to 20 hours per week, thus providing more structure for adults in Phase I of drug court who are not employed. An evidence-based employment intervention designed for adult drug court participants, and shown to be effective in increasing legal employment among the target population, will be added. A case manager will be hired to be assigned to the Drug Court IOP and wraparound services, including vouchers for licensed childcare and transportation, to address barriers to treatment participation among the target population, will be added. The services will be for 60 clients annually, a total of 180 over the three year grant.

Chrysalis House, Inc. requests SAMHSA funding for 3 years to provide comprehensive, high-quality residential substance abuse (SA) treatment to 20 low-income women (age 18 and over) annually (60 women in total) who are pregnant or postpartum and 60 of their newborn children, as well as outpatient services to 90 minor children (age 17 and under), and targeted outreach to 2,340 women who have limited access to quality health services. The target population also includes fathers of the children, partners of the women, and other extended family members of the women and children in treatment, when their inclusion in non-residential treatment services is deemed to be appropriate and beneficial. The project aims to ensure project implementation and evaluation adhere to stated objectives and facilitate continuous quality improvement; and improve treatment outcomes for women, infants, and the family unit. The project will expand and enhance the existing comprehensive continuum of care offered to women and their families throughout the state by Chrysalis House in Lexington, Kentucky. Using effective SA practices as identified by SAMHSA, Chrysalis House will implement holistic, state-of-the art clinical and service delivery approaches that are gender-specific and culturally-appropriate for the women and culturally- and age- appropriate for their minor children and other family members. Moreover, the comprehensive family-based treatment approach addresses the biological, psychological, and social aspects of women and their families. Chrysalis House has Memoranda of Agreement with over 50 community service network providers to offer the full range of PPW services to targeted clients.

Independence House in Corbin, Kentucky is seeking SAMHSA funding to provide long-term residential treatment for pregnant and postpartum women (PPW) with substance dependence. The new project is nestled within an eight-county community mental health center (Cumberland River Comprehensive Care Center). This project will serve 110 women, 60 children, and 110 other family members over a three year period. By actively engaging a myriad of services of Cumberland River, Independence House will link clients to a vast network of community services, including prenatal health, education, employment training and supported employment, and medication for co-occurring disorders when indicated and safe for the fetus or child. In addition, Independence House will provide family education and direct services to family members of the women and children who need and request services through Independence House or the provider network within Cumberland River Comprehensive Care Center. Independence House builds on over 20 years of successful short-term residential substance abuse (SA) treatment for women. The length of stay, which may be up to one year, will be individualized based on client need, family environment, and client consent. This three-year project will establish the only long-term residential SA treatment program dedicated to serving PPW in the State. While the State has had recovery support programs, Independence House will be the first licensed treatment program to exclusively serve this population.

The Hope Center, Inc. Wellness Recovery and Management (WARM) Project will target adult homeless men who are members of high-risk, underserved populations impacted by HIV and substance abuse. The specific high-risk target population for the proposed project includes primarily African-American and Latino/Hispanic adults who are homeless. The project will also target men who have been released from prisons and jails within the past two years and men who inject drugs, including men who have sex with men (MSM) and at-risk non-injecting MSMs. Additionally, the project will target men who are dually-diagnosed with a co-occurring mental disorder or mental illness. They will employ a number of evidence-based practices, including eight CSAT Treatment Improvement Protocols (TIPS), NIDA's Community-Based Outreach Model, the Kelley Foundation's Recovery Dynamics, and the NIV Thinking for a Change curriculum. The project objectives are to: 1) provide enhanced outreach and screening at its five Lexington sites to 250 homeless men annually, 2) provide enhanced outreach to 1200 men annually at the Hope Center Shelter, 3) provide expanded outreach/pretreatment targeting minorities through collaboration with St. Paul and St. Matthew AME Church and Bluegrass Community Health Center, 4) screen 100% of homeless men for HIV-risk and complete rapid HIV-testing for 80% of those men, and 5) facilitate the provision of medical case management for 100% of individuals who test HIV- positive with confirmation.

|
Assertive Community Living for Appalachian Dual Diagnosed Adults uses an evidence-based integrated services delivery method as the cornerstone for treatment, coupled with recovered-focused assertive community treatment and health services. The project aims to provide integrated mental health and substance abuse treatment through assertive community treatment and culturally sensitive care for up to 120 Appalachian adults (per year), aged 18 and over, who have co-occurring substance abuse and severe mental illness. The ethnicity of the region in Appalachia is 98% white and the project expects to treat this population with a small percentage of Latino, Native American, African American or Asian. The population is socio-economically disadvantaged.
