4. enhance the readiness of youth to participate in systmes change activities.
The Statewide Family Network project will serve children to age 18, or young adults to age 26 needing transitional services to the adult mental health system, who have a serious emotioal disturbance.

Wisconsin has automated reports for the Data Tables it submits by accessing data in a mental health/substance abuse data warehouse it created with the help of previous Data Infrastructure grant funds. Second goal for Wisconsin's project is to improve data quality. The HSRS, is Wisconsin's primary source for descriptive, outcome, and service encounter data for mental health consumers. The HSRS data will be subject to a comprehensive review of its reliability and validity. Finally, Wisconsin plans to increase its capacity for data analysis and reporting to maximize the use of its data.

Milwaukee Health Services, Inc. (MHSI), is a private non-profit Community Health Center, serves as lead agency of a community-based collaboration with the AIDS Resource Center of Wisconsin and Stanford University to improve access to culturally competent mental health services for people living with HIV/AIDS in southeast Wisconsin. This five-year project will engage a minimum of 800 people living with HIV/AIDS with mental health problems and their family member/partners in a comprehensive continuum of mental health services focused on the following outcomes: reduction of symptoms of depression/anxiety, reduction of substance abuse and HIV risk behaviors, and improved health and well being. The cooperative approach of this application coordinates HIV primary and specialty medical care, behavioral health services, scientifically driven program evaluation, psychosocial case management services, AIDS social services and health education.

Link for Life is a multidisciplinary collaborative effort of Marquette University faculty, staff, and administration. The program will focus on the issues of suicide and those mental health issues that impact suicide by providing suicide prevention training and education to administration, faculty, staff, and the general student body. The specific goals of this project are: 1) To raise awareness, across campus and among parents, about suicide among young people and to increase knowledge about mental illness. 2) To use a nationally recognized suicide prevention model (Question, Persuade, Refer [QPR]) to train Gatekeeper Trainers. 3) To enhance partnerships between the university and its students, as well as community mental health providers. 4) To review existing intervention efforts employed on other campuses. 5) To extensively evaluate he project under the guidance of a Project Evaluator to ensure the activities are as effective as possible and to reduce the potential loss of lives.

Wisconsin will utilize a mentoring and expert consultation approach to develop culturally sensitive, cross systems and consumer inclusive projects in 10 communities with elevated risk of youth suicide and then disseminate information on best practices through regional sharing sessions, a toolkit, a list serve and a statewide conference. This process will create an infrastructure and increased capacity to support the development of further projects. Wisconsin will educate all project sites on identifying at-risk groups in their communities but will focus particular attention on the following three target populations with elevated risk for suicide: Native American youth, youth who are deaf/hard of hearing, and youth in rural areas. Project sites will be selected through a competitive application among the 55 local entities, including four tribes, which submitted letters of interest. Project sites will be required to include the major youth-serving systems as part of their local coalitions, as well as including parents, mental health consumers, public health and primary care, faith communities and other stakeholders. Involvement of survivors of suicide will be strongly encouraged.

Through SAMHSA's Project LAUNCH, the Red Cliff Band of Lake Superior Chippewa, a federally recognized tribe, will create a supportive, family-driven accessible system of care comprised of a unique weaving of traditional values and evidence-based practices to enhance the social, emotional, physical and behavioral wellness of children aged 0 - 8 and their families
A Council on Young Child Wellness, with representation from families, the community, traditional healers, Early Childhood, Public School, Physical Health, Mental Health, Substance Abuse Treatment, Domestic Violence and Social Services will create a shared vision of child wellness. Harvard's Project on Effective Interventions School Readiness Pathway, coupled with local community knowledge, will identify existent service system gaps. To enhance cross-agency collaboration, increase knowledge of child development and engage families, 200 family-serving personnel will be trained in culturally-adapted Touchpoints.
Project LAUNCH goals are 1) Our Children are Developmentally On-track; 2) Our Families are Strong, Nurturing and Connected; 3) Our System of Care is Connected, Informed and built upon Traditional Wisdom; and 4) Our Actions Today will Strengthen the next Seven Generations (Sustainability). Expected project outcomes include improved child health and school readiness; developmentally-appropriate screenings across settings; increased access to prevention and early intervention programs and pediatric and adult mental health services; increased connectedness between providers, families, and schools; increased integration of physical health, mental health and traditional wisdom and, by project end, a sustainable system of care.

The purpose of the Sober Truth on Preventing Underage Drinking (STOP) Act grant program is to prevent and reduce alcohol use among youth in communities throughout the United States. The STOP Act grant program will encourage existing local community coalitions to develop, assess, and implement effective strategies to prevent and reduce underage drinking. Strategoies may include: changing local attitudes and norms, and re-evaluating existing laws and policies.
(1) Grantee must participate in national evaluation activities of the STOP grant program.
(2) STOP Grantees must use the Strategic Prevention Framework (SPF), a five step evidence based process for community planning and decision-making. The five step rocess includes: needs assessment, capacity building, planning, implementation and evaluation.
(3) STOP grantees must plan and implement a comprehensive approach inclusive of multiple strategies as emphasized in the 2007 Surgeon General's Call to Action to prevent and Reduce Underage Drinking located online at: http://www.surgeongeneral.gov/topics/underage drinking/calltoaction.pdf Emphasis should be given to environmental strategies that incorporate prevention efforts aimed at changing or influencing community conditions, standards, institutions, structures, systems and policies. In addition, grantees must select strategies that lead to long term outcomes.
(4) STOP grantees must enhance, not supplant, effective local community initiatives for preventing and reducing alcohol use among youth. For current Drug Free Community grantees, STOP ACT foods can not be used to supplant or replace activities that are presently being supported by Drug Free Comunity funds, and , separate DFC and STOP ACT accouting systems must be maintained for the purposes of reporting.

AIDS Resource Center of Wisconsin in Milwaukee, WI has received a 5 year Strategic Prevention Framework (SPF) grant to provide SA, HIV and Hepatitis prevention services to minority populations and minority reentry populations.
ARCW and leading partners' project purpose is to reduce and eliminate the occurrence of SA, HIV and Hepatitis infection among the minority and re-entry populations of the Milwaukee- Waukesha MSA.
ARCW's leading partners will be Wisconsin Correction Services, the state's leading Community service provider working with re-entry individuals and two minority community-based organizations, 16th Street Community Health Center and La Casa de Esperanza, to reach minority individuals. ARCW and its partners will work to reduce SA, including alcohol use, methamphetamine use, and other injection drug use, that can result in sex and drug use behaviors placing individuals at-risk for HIV and Hepatitis. With 20 years of experience providing prevention services to individuals at- risk, ARCW will extend prevention programming to reach a greater number of at-risk individuals.

Providing HIV and Hepatitis testing & referral services to individuals in the re-entry and minority communities, conducting 2,800 HIV and 1,800 Hepatitis tests annually. Screening 1,168 re-entry and minority individuals annually for substance abuse and make appropriate referrals for SA treatment. Conducting ulti-session SA, HIV and Hepatitis prevention education and counseling among the re-entry and minority populations reaching 880 individuals annually. Wisconsin proposes to build on the highly successful implementation of its ATR-funded Wisconsin Supports Everyone's Recovery Choice (WIser Choice) program in Milwaukee County by significantly expanding: 1) the number of total individuals served (38% more will be served than during the first round of ATR), and 2) the scope of the criminal justice population served to include the entire corrections continuum. WIser Choice will continue to serve: 1) the general adult population, with a special emphasis on 2) families with children, and 3) criminal justice population: a) inmates re-entering Milwaukee from prison, and b) offenders on probation or parole who face revocation proceedings and imprisonment, and who can be safely supervised in the community. With ATR-2, WIser Choice will now also cover the entire criminal justice continuum from pre-disposition (diversion), to sentencing (diversion and courts), to community alternatives to confinement (alternative to revocation and prison), to release from confinement (jail and prison re-entry). The program will continue to be implemented by the Office of Governor Jim Doyle in collaboration with multiple state, Milwaukee County, and community partners.

The State of Wisconsin will enhance delivery of substance abuse screening, brief intervention, referral, and treatment (SBIRT) services, in primary healthcare settings across the state. Implementation will adhere to the Chronic Care Model and recommendations by the Institute of Medicine. The project will emphasize development and ongoing improvement of durable systems that provide culturally competent services in a standardized manner. Wisconsin Initiative on Promoting Healthy Lifestyles (WIPHL) will sustain and enhance the delivery of SBIRT services for adolescents and adults, initially in 22 clinical sites affiliated with five umbrella organizations, then in additional sites of those organizations, and then throughout Wisconsin.
"Champions" at each clinical site will help select two implementation team leaders. After training, the leaders will use a workbook to guide their teams in implementing and improving SBIRT services in monthly plan-do-study-act (PDSA) cycles. WIPHL staff will support clinical site efforts with monthly conference calls, semi-annual meetings, and site visits. WIPHL site staff will systematically administer brief, multi-health behavior screens, using electronic medical record systems, written forms, or automated telephone systems. Patients with positive brief substance abuse screens will undergo fuller screening and receive additional SBIRT services from on-site counselors, who receive 3 weeks of initial training, ongoing supervision, and regular feedback. Counseling protocols will be based on the trans-theoretical model of change, motivational interviewing principles, and additional strategies to enhance cultural competence.

Currently ARC Community Services provides gender/cultural sensitive primary AODA treatment to approximately 150 women annually in three programs; two day treatment programs, the Center for Women and Children (CWC) and Healthy Beginnings (HB), and one intensive outpatient program, Integrated AODA and Work Services (JAWS). ARC-ROFBT targets substance abusing low income women ages 18 and older, who struggle with issues of trauma, relationship issues, mental health concerns, criminal justice system involvement, and low levels of education and marketable job skills. A large number of the target population are minority group members with children who have had prior treatment episodes. In addition to well-established Dane County referral sources, ARC has established linkages with Middleton Veterans Administration Hospital in Madison, Wisconsin to prioritize outreach and service delivery to female veterans suffering from issues of trauma and substance abuse.

The Meta House Preserving & Reuniting Families in Recovery (PRFR) project will provide residential programming for primarily African-American women with substance use and co-occurring mental health disorders who are pregnant or postpartum (PPW). The project will strengthen families by providing services to mothers, their minor children, children's fathers, mothers' partners, and family members of mothers and children. Meta House will expand services to PPW and enhance services to non-resident children, fathers, partners, and family members. The mothers typically have co-occurring mental health disorders, histories of trauma and various forms of violence, and serious physical health problems. Approximately 120 women, 240 children, 52 partners, 65 fathers, 63 family members will be served over three years. A family-centered treatment model will be implemented to provide gender- and culturally-appropriate comprehensive services, as well as the Self-In-Relation model, supported by family strength-based practices as motivational interviewing and decision-making. The onsite comprehensive trauma-informed parenting program implements EBPs and builds on its relationship with Children's Court and Child Welfare by targeting mothers having SA, high-risk pregnancies, and/or children in foster care or who would be placed in care if not for Meta House. Family engagement/reunification programming includes family education and support, fathers parenting, and a EBP youth program, Becoming a Responsible Teen. All services and provider network systems will be coordinated using a Single Coordinated Care Plan.

Horizons Inc. is a grassroots non-profit provider of transitional residential AODA and HIV prevention services in Milwaukee, Wisconsin.The proposed project to expand Horizons' Community Connections Program (CCP) will increase access to integrated outpatient AODA treatment and HIV services for a minimum of 600 justice involved women who are less likely to access treatment through formal systems. A minimum of 125 women offenders will receive integrated outpatient AODA treatment, HIV education and wraparound services each year. A minimum of 50 partners and 25 children of women will also be reached each year through the addition of evidence-based programs focused on AODA and HIV prevention. A minimum of 200 women and partners will receive rapid HIV testing each year. This project expands programming to African American women with substance use disorders (SUDs) who are at high risk of HIV infection/ transmission. The project will address psychosocial and empowerment issues that contribute to both SUDs and HIV risk behavior. HIV testing, counseling, education, and service referrals (for HIV positive women) will also be provided. Services will: improve the level of functioning related to substance abuse and sobriety; improve the stability of the family environment; improve mental health functioning and self care; and reduce the risk of HIV infection. The therapeutic approach will be Motivational Interviewing with interventions based on the motivational level of each woman for both substance use and HIV risk behaviors. Services include: Case Management, Alcohol and Other Drug Abuse (AODA) Treatment, Medical Services, Mental Health and Trauma Services, Services to Support Mothers with Children, Employment and Financial Self-Sufficiency, and HIV Prevention and Referral. The evaluation will examine the extent to which approximately 45 women annually, and 240 over five yeas, will: show a decrease in substance use; show a commitment to recovery; establish a drug-free family environment; show improvement in financial self-sufficiency; decrease mental health symptoms; show good mental health functioning and self care; show a decrease in PTSD symptoms; show an increase in the ratio of protected to unprotected sex; show an avoidance of other HIV risk behaviors; and possess sexual relationship skills.
