| Through this system of care initiative, Hamilton County's Journey to Successful Living (Journey) will serve youth and young adults, ages 14-21, with serious mental health challenges, who are transitioning to adulthood. These youth will be involved with the mental health and substance abuse system, child welfare system, juvenile justice system, and/or mental retardation and developmental disability system. Journey will build upon an existing system of care, collaborations and infrastructure, by adding partners from schools, social services, vocational rehabilitation, housing, primary care and the business community to create a broad based, sustainable systemic change inclusive of policy reform and infrastructure development that will be maintained after federal funding has ended. To accomplish this, Project Journey will establish a sustainable system of care that will: 1) be youth-focused, family driven and culturally and linguistically competent, with sensitivity to the overrepresentation of African American youth; 2) transform the current infrastructure by developing new policies, protocols and tools that facilitate comprehensive service planning designed to address needs of youth/young adults transitioning to adulthood and their families; 3) ensure the best, most efficacious treatment practices are available through expansion of non traditional services and use of evidence- based and promising practices; 4) increase workforce competency through comprehensive training and technical assistance; 5) build a cross system data information system to better inform planning and practice; and 6) incorporate a comprehensive evaluation to improve the infrastructure and quality of the project, and to improve sustainability after the grant ends | |

| This project proposes to adapt two evidence-based interventions, Parent-Child Interaction Therapy (PCIT) and Trauma - and Grief- Focused Component Therapy for Adolescents (TGCT-A) to address trauma-related symptoms and behaviors and to promote resiliency in two underserved and unique populations: 1) young children of military families and 2) adolescents in juvenile justice settings (JJS) and residential treatment centers (RTC). The two groups include children at risk for or displaying trauma-related problems such as Posttraumatic Stress Disorder, depression, anxiety, and/or behavior problems due to parental deployment(s); and adolescents with complex traumas such as child maltreatment, and family or community violence. For each group, the project will adapt the evidence-based intervention, including providing training protocols and resources, implementation strategies, and train-the-trainer toolkits, culminating in a web-based training in the third year of the grant. Beginning with pilot sites in Ohio, Ft. Hood and Tripler, the project will serve scores of children, families and adolescents in the first year, expanding to serve hundreds in years two and three and indirectly serving thousands of military families and youth in juvenile justice settings (JJS) and residential treatment centers (RTC) as the project's protocols, toolkits, and trained trainers disseminate the adapted models. | |

The Ohio Federation of Children's Mental Health proposes to strengthen the infrastructure of the Federation in order to increase its ability to support for Ohio's children, youth and families to enhance mental wellness and build resilency. This project, The Ohio Federation: Strengthening Foundation to Empower Family voice, will service all Ohio children, youth and their families who have a serious mental health disturbance. Achievement of these goals will increase the system capacity to support effective mental health services by 1) developing a grass-roots ground/swell of concerned and informed consumers of mental health services, 2) educating consumers and their families on issues related to being good consumers of mental heatlh services, 3) educating policy-influencing and policy -making stakeholders on key system transformation issues, 4) focusing attention and providing leadership to the growing concern of youth successfully transitioning into adulthood. |

The Clermont County Mental Health and Recovery Board, in partnership with families, youth and child-serving agencies, will implement a System of Care (SOC) comprehensive strategy providing promotion, prevention, early identification, and treatment interventions for emotional/behavioral problems impacting children aged 3-21 and their families, increasing the number of children/families served by 30 percent. The design of the SOC, named FAST TRAC, will be child-driven, family-focused, community-based, and culturally/linguistically competent. The goal is to intervene earlier in the child/family's life to more effectively address and resolve issues that lead to severe problems when they are not recognized or treated, while also identifying and responding to cultural/linguistic service barriers. Family and Children First, a partnership of government agencies and community organizations that plans and coordinates services to multi-need children and families through a "cluster" process, will provide oversight for FAST TRAC and assure direct access to, and coordination with, appropriate services. The SOC would provide the funding, new and enhanced services, infrastructure supports, and outcome data needed to assure an effective, child- and family-responsive, and evaluation-informed service delivery system. |

| Mental Health Services for Homeless Persons, Inc. (MHS) proposes to serve traumatized Children of ages 4-18 years who are referred to the Department of Children and Family Services (DCFS) of Cuyahoga County. MHS and DCFS staff members would work with the National Center for Child Traumatic Stress (NCCTS), and the Treatment and Service Adaptation (TSA) Centers to develop and implement a screening tool for use by members of the DCFS intake staff to identify at-risk children. During the course of the grant period, MHS would provide training for all 750 DCFS staff members and 100 DCFS supervisors, to help them integrate trauma-informed practices into their work. MHS would also utilize the talent and resources of the NCTSN to provide presentations on youth traumatic stress to professional and community groups, and to prepare and submit research findings for publication in professional journals. | |

CFC expects that 600 intake visits, 48 Wellness Education Sessions, 4500 Health Screenings will occur in the first year and 144 Wellness Education Sessions and 13,500 Health Screenings over the life of the project. | |

In the past two decades, Ohio's public mental health system has led the way to achieve reform and promote recovery. Now Ohio will build on that track record to achieve Mental Health Transformation. With strong leadership, the active involvement of all shareholders and support from the Mental Health System Transformation Grant, Ohio will knit together a wide range of resources and collaborations to achieve deeper change. Ohio approaches transformation as individuals and families approach recovery- as a meaningful process of internal change that requires strong external support. Ohio's resources include strong mental health leadership; effective advocacy groups; a demonstrated commitment to recovery, resiliency and cultural competence; nationally recognized approaches to promoting best/evidence-based practices, assessing outcomes, and evaluation research. Via this grant, Ohio will fully orient the mental health system and other systems to recovery, resiliency and culturally competent practices. Our goal and outcome is clear: children, families and adults will live their lives fully in their communities. Youth and adult consumers and families will be engaged in activities that affect their recoveries and service experiences, including person-centered planning, peer support and recovery/resiliency-oriented policy making. With the TSIG grant, Ohio will develop several new program approaches (i.e., trauma care for children and adults, older adult services and mental health supports for offenders reentering communities); integrate current policies and programs to emphasize person-centered planning, peer support and cultural competence; and develop the infrastructure to ensure effective collaboration and communication among all shareholders. |

| Nationwide Children's Hospital program will address the needs of children and adolescents who have experienced severe psychiatric disorders and complex trauma. The project will develop a regional trauma-informed service system and substantially expand the number of youths in the target population who receive evidence-based treatment. The program goals are fourfold. First, they will provide trauma-related training and technical support to local community agencies. One hundred twenty five administrators and providers from ten child and family serving agencies will be trained in trauma-informed services and evidence-based treatment. Second, they will promulgate the use of evidence-based treatment for complex trauma by implementing three interventions: Dialectical Behavioral Therapy (DBT), Trauma Focused Cognitive Behavioral Therapy with Selective Serotonin Reuptake Inhibitor (SSRI) medication treatment, and Care Management. The program will service 125 youths/families in Year 1, and 275 youths/families per year for the remaining two years. They will serve a large number of Medicaid clients and African-American families. Third, they will secure the provision of local resources to assist in the commitment of the region to implement these practices. Fourth, they will study systematically the need for cultural adaptations to these treatment approaches and implement these adaptations if warranted. | |

| Southeast, Inc. is proposing to meet the HIV/AIDS-related mental health needs of African Americans and other persons of African descent in Franklin County, Ohio through Project HIP (Health Is Primary). Project HIP will provide mental health treatment and other supportive services to a total of 480 individuals over the five year life of the grant. Treatment services will be provided by an interdisciplinary team, including a team leader, psychiatrist, RN, advanced practice psychiatric nurse, peer specialist, pastoral counselor, case manager, pharmacist and counselor to provide services in the community. Clients may have a mental health diagnosis or may have mental health needs that do not meet the criteria for a mental health diagnosis. This project will provide a full range of mental health services that address HIV-related mental health needs. | |

| The Ohio State University Campus Suicide Prevention Program (OSU-CSPP) seeks to develop a comprehensive, effective, culturally responsive, technologically advanced, and sustainable system of suicide prevention at the Columbus and five Regional Campuses. A broad and diverse network of 72 campus and community partners are already committed to the project and will create a systematic and coordinated effort, where suicide prevention is seen as a shared campus responsibility. There is high-level and broad-based commitment to this project, with the President of the University already committing his support and leadership, and campus partners from 63 different units and programs from the academic and service sides of the university, as well as student partners. The campus and community partners have identified individuals to spearhead their unit's contributions to suicide prevention. In the proposed project, 22 campus partners from 11 different campus offices have pledged at least 5% of their time as an in-kind match. Major components of the program include (1) continue to engage a broad and diverse group of campus and community partners; (2) collect and integrate new and existing data regarding suicide risk on campus; (3) develop coordinated policies and procedures for crisis management across university partners; (4) develop an extensive, culturally sensitive, and technologically advanced training system in suicide prevention, with particular attention to targeted groups; (5) use technology and e-messaging, in addition to traditional formats, to expand suicide prevention education and anti-stigma campaigns to all students; (6) develop a Comprehensive Suicide Plan for OSU; (7) evaluate all components of the project, through process, performance, and outcome measurements; and (8) disseminate information widely to other campuses, through professional organizations, and through SPRC Best Practice and SAMHSA Evidence-Based registries. | |

The purpose of the Sober Truth on Preventing Underage Drinking (STOP) Act grant program is to prevent and reduce alcohol use among youth in communities throughout the United States. The STOP Act grant program will encourage existing local community coalitions to develop, assess, and implement effective strategies to prevent and reduce underage drinking. Strategoies may include: changing local attitudes and norms, and re-evaluating existing laws and policies. (1) Grantee must participate in national evaluation activities of the STOP grant program. (2) STOP Grantees must use the Strategic Prevention Framework (SPF), a five step evidence based process for community planning and decision-making. The five step rocess includes: needs assessment, capacity building, planning, implementation and evaluation. (3) STOP grantees must plan and implement a comprehensive approach inclusive of multiple strategies as emphasized in the 2007 Surgeon General's Call to Action to prevent and Reduce Underage Drinking located online at: http://www.surgeongeneral.gov/topics/underage drinking/calltoaction.pdf Emphasis should be given to environmental strategies that incorporate prevention efforts aimed at changing or influencing community conditions, standards, institutions, structures, systems and policies. In addition, grantees must select strategies that lead to long term outcomes. (4) STOP grantees must enhance, not supplant, effective local community initiatives for preventing and reducing alcohol use among youth. SARDI, the applicant, housed in the Department of Community Health at Wright State's University (WSU) School of Medicine in Dayton, OH has received a 5 year Strategic Prevention Framework (SPF) grant to provide substance abuse prevention and HIV and Hepatitis prevention services to minority populations and minority reentry populations. SARDI is seeking funds for the Community Health Initiative (CHI) to reduce substance abuse, HIV/AIDS and Hepatitis risk factors primarily among at-risk African American males and females, including ex-offenders. Relevant community needs of the target group and the subsequent identification and implementation of prevention strategies will be accomplished by a collaborative of providers and consumers. The program will include outreach strategies to the target group, including a mobile health van capable of providing screening, testing, and appropriate referrals for African American neighborhood residents. The Community Health Initiative will integrate process and outcome evaluation activities inform and guide annual planned project goals and objectives with consumers. Local and national leaders with specific expertise in assessment, training, planning, program selection and implementation will assist with infrastructure development, to systemically prevent substance abuse, HIV/AIDS and HEP among African American men and women, including ex-offenders returning to their neighborhoods. |

The number of offenders coming through the Court has increased and the treatment programs are not able to meet the demands without increasing their resources. There are currently 100 drug court referrals waiting to receive a chemical dependency assessment and another 100 clients who have been assessed and are waiting for an opening in the intensive outpatient level of care. The project will create an expansion to and enhancement of services to add to its already successful range of treatment services to the drug court offender. Treatment capacity will be expanded annually by 130 intensive outpatient clients and enhancements through the implementation of an evidence based assessment/pre-treatment model and clinical oversight and supervision to the program. Greater Cincinnati Behavioral Health Services proposes to expand and strengthen treatment services for chronically homeless public inebriates. The Paths to Recovery project will link housing, treatment, medical care, and support services. Using a "low-demand housing" approach, all services will be tailored to support each person's unique "path to recovery". This will be achieved through a collaborative partnership with: Alcoholism Council, ( a SUD treatment provider) Over-the-Rhine Community Housing, a housing organization that provides resident-centered, affordable housing, and; The Health Care Connection, a federally qualified health center that offers medical, and dental services. Seamless service linkage will facilitate implementation of evidence-based practices designed to improve access to, and retention in, housing, as well as linkage to treatment and other services including: Housing First, Strengths-Based Case Management, Motivational Interviewing, and Integrated Dual Disorder Treatment. Paths to Recovery will serve chronically homeless public inebriates. The majority are: male (85.5%), African American (70.0%), between 31-50 years of age (61.6%), and have a co-occurring condition (54%). Daily alcohol use, history of alcohol dependence, and multiple failed attempts at substance abuse treatment characterize the target group. Most have significant physical health needs and have also experienced trauma related to chronic homelessness and substance use. |

Each year the Center will provide case management, social network support, and HIV/Hepatitis education to 100 African American target members at highest risk for substance dependence and/or HIV. Evidence-based approaches will include: 1) Strength-based Case-management to improve outcomes related to treatment entry/engagement, substance use, perceived needs, physical/mental health, housing, employment, and criminal justice involvement; 2) Social Network intervention to address risk factors among target groups' family and friends and improve recovery support, and; 3) VOICES/VOCES, to improve knowledge and perceived risk related to HIV. Wright State University through the Consumer Advocacy Model (CAM) program, an outpatient alcohol, drug, and mental health provider for persons with disabilities, proposes to address these needs through the creation of an e-therapy/community networks approach for Deaf persons referred to SUD treatment in Ohio. Over a period of three years, a minimum of 50 persons per year will be served through the state collaborative project that will both enhance the treatment experience and expand the number of Deaf individuals served. The proposed model will be based on knowledge currently available on best practices for this population, and it will include several components: 1. Individuals will be served in the "least restrictive environment" throughout Ohio in a combination of locally-available treatment that is augmented by e-therapy delivered via the CAM Program located in Dayton, Ohio. 2. Standard practices and levels of care will be enhanced through e-therapy that will supplement and strengthen the treatment episode, with an emphasis on learning about sustaining recovery. Group and individual counseling and support, and case management will be offered via electronic means, primarily though video conferencing and video phone technology. 3. All Deaf-specific services offered by CAM will be provided by individuals who come from a background in "Deaf culture", such as persons who are Deaf, children of Deaf adults (CODA), or Deaf services career professionals. Consumer feedback and guidance from a project Advisory Board will guide program content and service delivery. Enhanced services will be coordinated by a treatment plan shared with the primary community provider and CAM. Collaboration PUSH (People United to Stop HIV/AIDS) aka C-PUSH, will serve African American women and their children living in East Cleveland who fall through all safety nets, including TANF because they have been sanctioned and/or have utilized all other available resources. The targeted area is 93% African American. The Cleveland prevalence rate for females is 245.8 per 100,000. 47.4% of the Heads of Households in this area (primarily African American women) live below the poverty rate. Young, cannabis using offenders exhibit consistently low success rates within the context of Drug Court. Southeast, Inc.'s proposed outcomes for the project include: o 90% of clients will be able to identify triggers for substance use and understand negative impacts of substance use as measured by a post-treatment questionnaire; o 90% of clients will understand the impact of substance use and prescription medications as measured by a post-treatment questionnaire; o 60% of clients will be linked to and receive primary healthcare as documented in clinical records; o 90% of clients will be able to identify personal recovery goals; o 40% of clients who are homeless at intake will achieve housing as documented in clinical records; o 60% of clients with criminal justice system involvement will have decreased involvement with the system measured by fewer arrests in the six months after starting treatment than in the six months prior to starting services; o 80% of clients will participate in an element of a recovery support system; and o The program will experience a 15% drop in attrition between the first appointment and discharge from the program. |

| The Mental Health and Recovery Services Board of Stark County (MHRSB) proposes to create SHELTER, an integrated services program that serves homeless transitional youth (ages 18-25) and serial inebriates with mental illness and co-occurring substance abuse disorders. The SHELTER Project utilizes the evidence-based practices of Strengths-Based Case Management, Motivational Interviewing, and Transitions to Independence. It is anticipated that 50 individuals per year will be linked with scattered site housing and a holistic continuum of community-based services for a total of 250 individuals over the five year project period. | |
