Wisconsin Family Ties (WFT) will work to strengthe the capacity and capability of famiies to ast as agents of change in transforming the chodren's mental health system in Wisconsin. The goals of WFT's WStatewide Family Netowrk activities are to: 1 strengthe relationshps between all who are interested in the health and wellbing of children with serious emotional disturbances(SED), 2. help fmailies develop strong leadership skills and business acumen, 3. provide effective services to families that are respnsive to their needs, and 4. enhance the readiness of youth to participate in systmes change activities. The Statewide Family Network project will serve children to age 18, or young adults to age 26 needing transitional services to the adult mental health system, who have a serious emotioal disturbance. Second goal for Wisconsin's project is to improve data quality. The HSRS, is Wisconsin's primary source for descriptive, outcome, and service encounter data for mental health consumers. The HSRS data will be subject to a comprehensive review of its reliability and validity. Finally, Wisconsin plans to increase its capacity for data analysis and reporting to maximize the use of its data. | |

Overall, project goals are to: 1) Build on existing state level-infrastructure to support the work of the WCC project; 2) Develop a local Child Wellness Council for a comprehensive child wellness system; 3) Build upon/sustain evidence-based programming and increase the capacity of the community; and 4) Enhance and promote child wellness in all areas of development. | |

| Grassroots Empowerment Project Inc. (GEP) intends to develop a Statewide Consumer Network by conducting 8 regional "Leadership Academies," which will support mental health consumers in their active and meaningful participation in mental health/substance abuse service system transformation at the state and local levels. The leadership academies will include in-depth training with ongoing technical and educational support. GEP will develop a multimedia curriculum to provide consumers with the information and tools necessary to become active participants in mental health system transformation. In addition, GEP intends to sponsor annual 2-day forums, bringing mental health consumers and state staff who are involved in mental health policy making stakeholders of state public mental health policy groups to discuss the need and benefits of involving consumers in policymaking, to identify mental health system transformation issues of importance to consumers, and to create linkages between consumers and policy groups. The goal of the project is to increase the number of skilled consumers working as equal partners in Wisconsin mental health service transformation. | |

This five-year project will engage a minimum of 800 people living with HIV/AIDS with mental health problems and their family member/partners in a comprehensive continuum of mental health services focused on the following outcomes: reduction of symptoms of depression/anxiety, reduction of substance abuse and HIV risk behaviors, and improved health and well being. The cooperative approach of this application coordinates HIV primary and specialty medical care, behavioral health services, scientifically driven program evaluation, psychosocial case management services, AIDS social services and health education. | |

Through SAMHSA's Project LAUNCH, the Red Cliff Band of Lake Superior Chippewa, a federally recognized tribe, will create a supportive, family-driven accessible system of care comprised of a unique weaving of traditional values and evidence-based practices to enhance the social, emotional, physical and behavioral wellness of children aged 0 - 8 and their families A Council on Young Child Wellness, with representation from families, the community, traditional healers, Early Childhood, Public School, Physical Health, Mental Health, Substance Abuse Treatment, Domestic Violence and Social Services will create a shared vision of child wellness. Harvard's Project on Effective Interventions School Readiness Pathway, coupled with local community knowledge, will identify existent service system gaps. To enhance cross-agency collaboration, increase knowledge of child development and engage families, 200 family-serving personnel will be trained in culturally-adapted Touchpoints. Project LAUNCH goals are 1) Our Children are Developmentally On-track; 2) Our Families are Strong, Nurturing and Connected; 3) Our System of Care is Connected, Informed and built upon Traditional Wisdom; and 4) Our Actions Today will Strengthen the next Seven Generations (Sustainability). Expected project outcomes include improved child health and school readiness; developmentally-appropriate screenings across settings; increased access to prevention and early intervention programs and pediatric and adult mental health services; increased connectedness between providers, families, and schools; increased integration of physical health, mental health and traditional wisdom and, by project end, a sustainable system of care. |

The Lawrence University Campus Suicide Prevention Project (LUCSPP) is a comprehensive approach to lowering suicide risk factors and enhancing protective factors, among Lawrence students. The two goals of LUCSPP are to (I) Strengthen Systemic and Sustainable Structures to effectively address the mental health needs of students particularly those at high risk; and (II) Change Campus Culture to reduce stigma, reduce suicide risk factors ingrained in the academic and social culture on campus, and promote awareness and use of mental health services. Our objectives are: (1) to implement project activities in a transparent and inclusive manner, drawing on perspectives and expertise of broad and diverse on- and off-campus constituencies; (2) to develop new and enhance existing training programs for campus personnel and students to recognize, respond to, and refer distressed students, support students affected by suicidal behavior of others, and respond in culturally competent ways; (3) to strengthen internal and external networking infrastructures; (4) to develop comprehensive, culturally appropriate educational outreach approaches tailored to the unique needs of our campus; (5) to monitor, evaluate, assess, and report on program activities to assure sustainability. Our students are socioeconomically diverse; hail from nearly every state; and 12% are international, representing over 50 countries. We will target the entire student body through comprehensive culturally competent approaches that address risk factors and promote life skills, resiliency, and social connectedness, particularly among students from diverse sub-populations at higher risk. We will reach work with key community mental health stakeholders to identify common concerns and share suicide prevention efforts, particularly with other local college campuses; existing and potential partners in crisis pre-., inter-, and postvention; and other organizations serving at-risk youth and young adults. |

Project Name: CMN Suicide Prevention Project College of Menominee Nation, a Tribal College located in Keshena WI, proposes to develop a suicide prevention program and offer prevention services currently unavailable to our high risk populations. CMN serves several high-risk populations including Native Americans and Veterans. Currently the College does not provide any counseling or preventative services, although it works with local agencies to provide students the services they need. SAMHSA funds would allow the College to focus on better meeting the needs of these populations. Population Focus: Native Americans and Veterans Proposed Strategies/Methods: CMN proposes to accomplish all required SAMHSA activities, targeting all students at both its Keshena and Oneida campuses. This is approximately 1000 students annually. In general, the project will assist CMN develop a campus suicide prevention/education program from the ground up. |

The council will achieve these goals by implementing multiple strategies such as conducting the search survey, establish a youth involved media campaign , circulate cultural sensitive information in the community and conduct youth and adult focus groups in the community. These collaborated efforts with key stakeholders will allow the council to continue its history of success. | |

The purpose of the Sober Truth on Preventing Underage Drinking (STOP) Act grant program is to prevent and reduce alcohol use among youth in communities throughout the United States. The STOP Act grant program will encourage existing local community coalitions to develop, assess, and implement effective strategies to prevent and reduce underage drinking. Strategoies may include: changing local attitudes and norms, and re-evaluating existing laws and policies. (1) Grantee must participate in national evaluation activities of the STOP grant program. (2) STOP Grantees must use the Strategic Prevention Framework (SPF), a five step evidence based process for community planning and decision-making. The five step rocess includes: needs assessment, capacity building, planning, implementation and evaluation. (3) STOP grantees must plan and implement a comprehensive approach inclusive of multiple strategies as emphasized in the 2007 Surgeon General's Call to Action to prevent and Reduce Underage Drinking located online at: http://www.surgeongeneral.gov/topics/underage drinking/calltoaction.pdf Emphasis should be given to environmental strategies that incorporate prevention efforts aimed at changing or influencing community conditions, standards, institutions, structures, systems and policies. In addition, grantees must select strategies that lead to long term outcomes. (4) STOP grantees must enhance, not supplant, effective local community initiatives for preventing and reducing alcohol use among youth. For current Drug Free Community grantees, STOP ACT foods can not be used to supplant or replace activities that are presently being supported by Drug Free Comunity funds, and , separate DFC and STOP ACT accouting systems must be maintained for the purposes of reporting. |

AIDS Resource Center of Wisconsin in Milwaukee, WI has received a 5 year Strategic Prevention Framework (SPF) grant to provide SA, HIV and Hepatitis prevention services to minority populations and minority reentry populations. ARCW and leading partners' project purpose is to reduce and eliminate the occurrence of SA, HIV and Hepatitis infection among the minority and re-entry populations of the Milwaukee- Waukesha MSA. ARCW's leading partners will be Wisconsin Correction Services, the state's leading Community service provider working with re-entry individuals and two minority community-based organizations, 16th Street Community Health Center and La Casa de Esperanza, to reach minority individuals. ARCW and its partners will work to reduce SA, including alcohol use, methamphetamine use, and other injection drug use, that can result in sex and drug use behaviors placing individuals at-risk for HIV and Hepatitis. With 20 years of experience providing prevention services to individuals at- risk, ARCW will extend prevention programming to reach a greater number of at-risk individuals. Upon completion & approval of the needs assessment & strategic plan, ARCW and its partners will implement prevention interventions that include: Providing SA, HIV, and Hepatitis prevention outreach through 10,800 contacts annually. Providing HIV and Hepatitis testing & referral services to individuals in the re-entry and minority communities, conducting 2,800 HIV and 1,800 Hepatitis tests annually. Screening 1,168 re-entry and minority individuals annually for substance abuse and make appropriate referrals for SA treatment. Conducting ulti-session SA, HIV and Hepatitis prevention education and counseling among the re-entry and minority populations reaching 880 individuals annually. Conducting capacity building assistance annually to 48 organizations serving re-entry minorities & minority individuals to provide SA,HIV & Hepatitis prevention. |

The State of Wisconsin will enhance delivery of substance abuse screening, brief intervention, referral, and treatment (SBIRT) services, in primary healthcare settings across the state. Implementation will adhere to the Chronic Care Model and recommendations by the Institute of Medicine. The project will emphasize development and ongoing improvement of durable systems that provide culturally competent services in a standardized manner. Wisconsin Initiative on Promoting Healthy Lifestyles (WIPHL) will sustain and enhance the delivery of SBIRT services for adolescents and adults, initially in 22 clinical sites affiliated with five umbrella organizations, then in additional sites of those organizations, and then throughout Wisconsin. "Champions" at each clinical site will help select two implementation team leaders. After training, the leaders will use a workbook to guide their teams in implementing and improving SBIRT services in monthly plan-do-study-act (PDSA) cycles. WIPHL staff will support clinical site efforts with monthly conference calls, semi-annual meetings, and site visits. WIPHL site staff will systematically administer brief, multi-health behavior screens, using electronic medical record systems, written forms, or automated telephone systems. Patients with positive brief substance abuse screens will undergo fuller screening and receive additional SBIRT services from on-site counselors, who receive 3 weeks of initial training, ongoing supervision, and regular feedback. Counseling protocols will be based on the trans-theoretical model of change, motivational interviewing principles, and additional strategies to enhance cultural competence. |

WIser Choice will continue to serve: 1) the general adult population, with a special emphasis on 2) families with children, and 3) criminal justice population: a) inmates re-entering Milwaukee from prison, and b) offenders on probation or parole who face revocation proceedings and imprisonment, and who can be safely supervised in the community. With ATR-2, WIser Choice will now also cover the entire criminal justice continuum from pre-disposition (diversion), to sentencing (diversion and courts), to community alternatives to confinement (alternative to revocation and prison), to release from confinement (jail and prison re-entry). Currently ARC Community Services provides gender/cultural sensitive primary AODA treatment to approximately 150 women annually in three programs; two day treatment programs, the Center for Women and Children (CWC) and Healthy Beginnings (HB), and one intensive outpatient program, Integrated AODA and Work Services (JAWS). ARC-ROFBT targets substance abusing low income women ages 18 and older, who struggle with issues of trauma, relationship issues, mental health concerns, criminal justice system involvement, and low levels of education and marketable job skills. A large number of the target population are minority group members with children who have had prior treatment episodes. | This project expands programming to African American women with substance use disorders (SUDs) who are at high risk of HIV infection/ transmission. The project will address psychosocial and empowerment issues that contribute to both SUDs and HIV risk behavior. HIV testing, counseling, education, and service referrals (for HIV positive women) will also be provided. Services will: improve the level of functioning related to substance abuse and sobriety; improve the stability of the family environment; improve mental health functioning and self care; and reduce the risk of HIV infection. The therapeutic approach will be Motivational Interviewing with interventions based on the motivational level of each woman for both substance use and HIV risk behaviors. Services include: Case Management, Alcohol and Other Drug Abuse (AODA) Treatment, Medical Services, Mental Health and Trauma Services, Services to Support Mothers with Children, Employment and Financial Self-Sufficiency, and HIV Prevention and Referral. The evaluation will examine the extent to which approximately 45 women annually, and 240 over five yeas, will: show a decrease in substance use; show a commitment to recovery; establish a drug-free family environment; show improvement in financial self-sufficiency; decrease mental health symptoms; show good mental health functioning and self care; show a decrease in PTSD symptoms; show an increase in the ratio of protected to unprotected sex; show an avoidance of other HIV risk behaviors; and possess sexual relationship skills. | |

| The Meta House Preserving & Reuniting Families in Recovery (PRFR) project will provide residential programming for primarily African-American women with substance use and co-occurring mental health disorders who are pregnant or postpartum (PPW). The project will strengthen families by providing services to mothers, their minor children, children's fathers, mothers' partners, and family members of mothers and children. Meta House will expand services to PPW and enhance services to non-resident children, fathers, partners, and family members. The mothers typically have co-occurring mental health disorders, histories of trauma and various forms of violence, and serious physical health problems. Approximately 120 women, 240 children, 52 partners, 65 fathers, 63 family members will be served over three years. A family-centered treatment model will be implemented to provide gender- and culturally-appropriate comprehensive services, as well as the Self-In-Relation model, supported by family strength-based practices as motivational interviewing and decision-making. The onsite comprehensive trauma-informed parenting program implements EBPs and builds on its relationship with Children's Court and Child Welfare by targeting mothers having SA, high-risk pregnancies, and/or children in foster care or who would be placed in care if not for Meta House. Family engagement/reunification programming includes family education and support, fathers parenting, and a EBP youth program, Becoming a Responsible Teen. All services and provider network systems will be coordinated using a Single Coordinated Care Plan. | |

| Milwaukee County Circuit Court targets high risk/high need non-violent offenders with significant substance abuse problems who are charged with a felony or are chronic misdemeanants, are willing to participate in treatment, and who would otherwise face a District Attorney's Office recommendation for incarceration. The program uses a collaborative team approach to identify, screen, assess, case plan, and monitor individuals eligible for Treatment Court services. The project has three major goals: Goal 1: Successfully divert 120 offenders from further involvement in the criminal justice system through an adult drug treatment court program that combines intensive supervision with clinical treatment and recovery support services provided in an environment emphasizing personal accountability and responsibility; Goal 2: Improve Drug Treatment Court program outcomes for 45 participants through targeted use of medication-assisted treatment; and Goal 3: Improve the effectiveness of the system infrastructure for Drug Treatment Court participants with co-occurring disorders by advancing the integration of the substance abuse treatment and mental health services systems. | |

| The purpose of the Mi-WISH Project is to develop a recovery-oriented integrated system of care to provide homeless (primarily African American) adults with substance abuse disorders or co-occurring substance abuse disorders and mental health disorders with treatment and recovery support services; and to help them develop a stable income so that they can sustain housing. Mi-WISH will serve individuals who are brought in on an emergency detention basis by the police to Psychiatric Crisis Services (PCS), and subsequently referred to an acute inpatient psychiatric unit or detoxification center. This population is not receiving services due to their chronic symptoms and inability to maintain housing. Upon discharge from the hospital or detoxification services, participants will be placed in community housing with supportive services; which will be paid for temporarily by Homeless Prevention and Rapid Re-Housing Program (PHRP) funds, while employment is being secured or mainstream benefits accessed. The BHD project will be based on four evidenced-based practices shown to be effective with this population: 1) Integrated Treatment for Co-Occurring Disorders; 2) Comprehensive Case Management; 3) Supported Employment ; and 4) SSI/SSDI Outreach and Recovery (SOAR). | |

| The Milwaukee Behavioral Health Division (BHD) proposes to build on the success of its Access to Recovery-funded re-designed service delivery system, to expand and enhance its services for 150 adult offenders reentering Milwaukee from prison. The Milwaukee Linking Individuals to a New Chance (MI-LINC) project will build on existing linkages between BHD and the Wisconsin Department of Corrections (DOC) to serve (primarily African American) adults with substance abuse disorders. The DOC Reentry Director will facilitate access to two institutions (one male, one female) from which participants meeting project admission criteria will be identified. BHD will provide a comprehensive screen on a reach-in basis to these individuals. The Division of Community Corrections (DCC) will dedicate several field agents to handle supervision for all MI-LINC participants, to simplify communication with BHD, project Recovery Support Coordinators (RSC's) and treatment providers. | |
