Templates to Help You Get Started
SAMHSA has restructured the way we announce our grant funding opportunities for
FY 2006. Most announcements will be developed from a standard “template”
for one of the following program areas: Services; Infrastructure; Best
Practices; or Service to Science. Requirements specific to each grant
program will be added to the standard template. In preparation for
developing a grant application, prospective applicants are encouraged to
familiarize themselves with the basic format and requirements of the templates
prior to the announcement of the funding opportunity.
-
Formatting Requirements
Your application must adhere to the following formatting requirements. Failure
to do so will result in your application being screened out and returned to you
without review. In addition to these formatting requirements, there may be
specific programmatic requirements. Please check the specific funding
announcement before preparing your application.

SAMHSA Grant Application Forms
The list below describes the forms required for the SAMHSA grant application
process. In most cases, these forms are maintained by other Federal
offices. The Federal forms include detailed instructions for completing
them.
