This project will continue the State's effort to build infrastructure to collect data and report the remaining Mental Health Block Grant Uniform Reporting System Developmental Measures. Grant efforts will focus on (1) local provider training to improve data quality, (2) implementation of web-based technology using DS2K + data standards to collect, report, and improve accessibility of data, and (3) strengthening internal and external database linkages. Project outcomes will include consistent data definitions, timely capture of data, improved measure of service outcomes and client change, improved data quality, and enhanced ability to analyze and report on developmental measures such as school attendance, school performance, and involvement with the criminal justice system. The project outcomes will be evaluated based on the ability to produce the data required for URS and other desired reporting. The project will also be evaluated in terms of its ability to produce data that is useful to and is used by system stakeholders.

Building upon the strong interagency infrastructure of the Kentucky IMPACT Program and the school-based mental health model established through the Bridges Project, Kentuckians Encouraging Youth to Succeed (KEYS) will facilitate the implementation of a continuum of positive behavior interventions and supports in targeted schools in the North Central region of Kentucky to address the mental health and substance use needs of its youth. Recognizing the unmet needs of youth with co-occurring mental health and substance use disorders, KEYS will place particular emphasis upon employing evidence-based strategies to effectively identify and treat this challenging population. Additionally, KEYS will address identified gaps in the available service array for youth with serious emotional disabilities, including early childhood, crisis stabilization, day treatment, adolescent substance abuse, intensive home-based, and respite. A comprehensive evaluation system will be in place to support continuous improvement of the model and to support sustainability and statewide replication of the system of care.

The Kentucky Seclusion and Restraint Reduction Project will implement a national initiative, "Creating Violence and Coercion Free Mental Health Treatment Environments" in five state inpatient psychiatric facilities. Central State Hospital is currently a pilot facility for the initiative, and the first goal is to complete these pilot activities. This includes the implementation of an Aggression Treatment Map, implementation of seclusion and restraint reduction curriculum for all clinical staff and the reduction of seclusion and restraint by 60 percent by end of year one of the grant project. The project will provide training in the Seclusion and Restraint Curriculum at the other three state hospitals and the state's forensic hospital and develop a facility implementation plan at Western State Hospital. Activities include establishment of the NAMI Family-to-Family Education Program, completion of training on Aggression Treatment Map, implementation of the pilot on an identified unit, and reduction of seclusion and restraint use per indicators by 60 percent of baseline by end of year one of grant period.

The Kentucky Medication Algorithm Training and Evaluation Project will implement the Medication Management Approaches in Psychiatry "Toolkit" in two communities (rural and urban) in Kentucky. This project will implement a comprehensive training plan as the foundation of implementation of the evidence-based practice. A project coordinator will work with the KyMAP steering committee to oversee the project along with other partners (three universities, two Regional MH/MR Boards and two state psychiatric hospitals). Training in the algorithm will be provided to five stakeholder groups: consumers, families and other supporters, practitioners and clinical supervisors, mental health program leaders, and public mental health authorities). Unique features of the training plan will include web-based support for initial training and continuing education, weekly follow-up, and a focused consumer and family education. The evaluation of the project will focus on: 1) planned implementation strategies and sequencing of implementation steps; 2) organizational factors; and 3) quality and effectiveness of the training program. Anticipated outcomes include successful implementation of the evidence-based practice in each of the two target communities, provision of initial and ongoing training in the schizophrenia algorithm, measurement of client and program level outcomes, and sustained movement toward a high degree of fidelity to the model.

The Strategic Prevention Framework State Incentive Grants are used to advance community-based programs for substance abuse prevention, mental health promotion, and mental illness prevention. The SPF SIG implements a five-step process known to promote youth development, reduce risk-taking behaviors, build on assets, and prevent problem behaviors. The five steps are: (1) conduct needs assessments; (2) build state and local capacity; (3) develop a comprehensive strategic plan; (4) implement evidence-based prevention policies, programs and practices; and (5) monitor and evaluate program effectiveness, sustaining what has worked well. These grants will allow the programs to provide leadership, technical support and monitoring to ensure that participating communities are successful. The success of the grants will be measured by specific measurable outcomes, among them: abstinence from drug use and alcohol abuse, reduction in substance abuse-related crime, attainment of employment or enrollment in school, increased stability in family and living conditions, increased access to services, and increased social connectedness. Kentucky's Strategic Prevention Framework Incentive Grant project will build a data-driven, comprehensive, interagency prevention service system to reduce substance abuse and its related problems. The State will make extensive use of geographic information system (GIS) technology to focus community goal-setting and enable targeted resource allocation.

In collaboration with the University of Louisville WINGS Clinic, the Kentucky Department of Corrections and Volunteers of America, the Connectimmunity Project will expand and enhance the Creating Lasting Family Connections substance abuse prevention curriculum to integrate and increase access to HIV and Hepatitis prevention services. The project will focus on minority children and youth ages 12 - 17 years and include males and females reentering the community from correctional facilities.

The Inter-Association Child Care Substance Abuse Conference For Child, Youth, And Family Work Professionals Of The Mid-Atlantic States Will Be Held In The Radisson Hotel, Valley Forge, PA On November 4-6, 2005 for 350 child and youth care workers, case workers, parole and probation officers, educators, juvenile correction staff, foster parents, program supervisors and directors, and administrators from within the human services and juvenile justice delivery system. The specific goals of the Conference, Through Knowledge Dissemination, will be to build a prevention knowledge base around alcohol, tobacco, and other drug (ATOD) issues That Are Deemed Critical, Advance St A Te-Of- The-Art Prevention Services, And Raise Awareness Among Direct Services Workers Of Me Role They Can Play In Advocacy And Policy Building For The Prevention Of ATOD Abuse Among The Children, Youth, And Families With Whom They Work. The Conference Will Focus On Culturally appropriate Strategies for Prevention and Early Intervention in Community-Based ATOD Abuse Programs.

The Healing Place program, a social model recovery program, has as its aim to attract clients into a program of habilitation/rehabilitation that will sustain their desire and will to recover from the physical, mental, emotional, and social detriments of alcohol and drug addiction, and to build meaningful lives. Grant funds will apply toward Women's Program at The Healing Place. Clients will be introduced to Recovery Dynamics, the curriculum that teaches them about alcoholism and drug abuse, and the 12-step program they will be following. They will also be introduced to Peer Mentors, who will shepherd them through the program. They will attend AA meetings, and accountability community meetings, as they make their way through the motivational track and into the recovery program. They will learn to understand and to implement the principles and essential elements of the program, to learn and develop the core values of the program, to complete the requirements of the program, and to develop a plan to move through a transitional phase, then on to independent living, successfully addressing their own individual program to continue their recovery and to address issues important to their transition back to the community, e.g. legal, financial, educational, vocational, housing, familial, and relational.

Seven Counties Services is a private, non-profit, community behavioral health center that has developed a system of care for youth ages 10 to 21 who abuse substances and their families in Jefferson County, Kentucky. The project provides many services (prevention, assessment, education groups, and intensive outpatient, inpatient, and residential treatment and limited aftercare) out of their main center, which are also accessible at the local community level and by mobile response.

Louisville, KY continues to experience an increase in HIV case rates within the minority community, with a minority case rate in excess of 25 per 100,000. The scope of this project includes: increasing the number of counselors; developing African American or other minority alcohol and drug abuse counselors; utilizing outreach to increase treatment enrollment; implementing a motivational approach to increasing client motivation to enter treatment; and utilizing ongoing evaluation to improve service provision to clients.

To enhance and expand Intensive Outpatient treatment and outreach services to 600 individuals that are afflicted by HIV/AIDS who may also have co-occurring substance abuse disorders. The two primary at risk populations are: 1) African-American men who are intravenous drug users (IDU), including men who have sex with men (MSM), and at risk non-IDU MSMs, and 2) women who are positive for HIV and at risk for the acquisition of HIV, including those with children.

Northern Kennedy Drug Court plans to serve 65 juveniles, ages 14 to 18, per year for three years (total of 195 juveniles over three years) in hopes of increasing abstinence, decreasing recidivism, improving life skills, expanding and maintaining resource bases, and increasing community support for drug court operations. The target population is 97% White and 2% African American. The project aims to use a combination of substance abuse treatment, case management, individual, group and family therapy, and wraparound services to accomplish its goals.

The Kentucky Youth First Project is focused on enhancing infrastructure supports for adolescent substance abuse treatment in a rural State that includes an Appalachian area. The project will establish a full-time Adolescent Treatment Coordinator, a liaison with the newly formed Kentucky Office of Drug Control Policy, and specialist staff to help develop better trained and qualified treatment providers. With an overall purpose of serving more adolescent clients and improving the quality of care, this project has three specific goals: (1) to enhance linkages among public and private agencies to better identify, refer, intervene with and treat adolescents who have substance use disorders; (2) to increase training and clinical consultation supports with clear target expectations for adolescent treatment providers by encouraging the use of best practices; and (3) to increase the uses for adolescent specific client-level data on characteristics of adolescents entering treatment as well as follow-up outcomes on clients who have received treatment. These data will be used to strengthen and coach clinical providers in using data to guide treatment efforts and to compare outcomes for adolescents with those of adults. The project will develop memoranda of agreement between key linkage agencies and will strengthen training and consultation resources so that there is a greater clinical presence regarding adolescent treatment in State-funded programs. In addition, the project will use intake and follow-up data to help clinicians and policy makers better understand the characteristics of clients entering treatment and their needs.

WestCare Kentucky (KY) Pike County Residential Treatment Project will establish a 25-bed, long-term, residential, therapeutic community for males (=18 years) diagnosed with substance use disorders. The startup project will operate in a recently donated treatment facility located in Elkhorn City, Pike County, KY, about 40 minutes from the county seat of Pikeville within the Fifth Congressional District. It is expected that 100 men will be assessed for treatment admission during the 12-month project period. This SAMHSA project will provide seed money to establish much needed long-term, residential, substance abuse treatment services. The proposed 25 long-term treatment slots will augment residential capacity by 27% in the Congressional District that now only has short-term treatment beds. WestCare KY was incorporated in 2004 to provide treatment for individuals and families that are affected by substance abuse

The "Family Futures" project will focus on women with substance abuse problems who also face custody problems with their young children (up to age 5). The project will use Drug Court supervision coupled with intensive treatment and case management to address substance abuse problems, co-occurring disorders, and parenting issues. The target population will be 105 mothers and 154 children over the course of the three year grant; 30 mothers and 44 children in the first year, 40 mothers and 59 children in the second year, and 35 mothers and 51 children in the third year, totaling 259 clients overall. The proposed ethnic breakdown is: 60% White; 35% African American; and 5% Hispanic.

Chrysalis House will provide residential substance abuse and comprehensive services to 72 women and 215 children over a three-year period. The project will expand the availability of residential substance abuse treatment for low-income pregnant or postpartum women, or other parenting women and their minor children who have limited access to quality health care.

The Hope Center's proposed project will provide outreach, stabilization, substance abuse treatment, and recovery (wraparound) services to homeless women with substance abuse or co-occurring disorders. The proposed project represents a collaborative effort between the Hope Center and Chrysalis House, a licensed substance abuse provider, and over the 5 year period the project will provide residential recovery and treatment services to 150 women annually.
