Governor Christine Gregoire of the State of Washington is leading Partnerships for Recovery with the full support and participation of the Director of every Department and Division serving people with mental illness in the State of Washington. With consumers and family members as equal partners, Partnerships for Recovery has launched a deep transformation effort to achieve the goals of the President’s New Freedom Commission for all people in the State of Washington. The template for Partnerships for Recovery is the President’s New Freedom Commission Report. However what will emerge in Washington State will be unique to the needs of the consumers and families of Washington. Partnerships for Recovery is building the infrastructure to an on-going process of planning, action, learning and innovation that will result in measurable improvements in the lives of both young and old throughout the State. Key elements of the initiative will include a social marketing initiative to reduce the stigma of mental illness, increase awareness, strengthening of the statewide infrastructure for consumer and family support and development of a comprehensive approach to insure participation of consumers as service providers. Reduction of ethnic and geographic disparitiesAdoption of a strengths-based, consumer-driven care planning modelImplementing training and fiscal and regulatory incentives for the expanded use of evidence- based recovery focused practices. The Statewide Action for Family Empowerment of Washington (SAFE WA) is a coalition of twelve family organizations that provides support to parents and caregivers, and Health 'N Action, a youth group that provides direct support to youth. SAFE WA proposes to provide training and technical assistance supports to these organizations and unite their voices to establish a clear, common voice that decision and policy makers will hear.

SAMHSA funds will supplement state funds to adopt and test the Transition to Independence Process (TIP) developed by H.B. Clark, and for at risk youth, will develop a Transitional Assertive Community Treatment team (ACT), as modeled by D. Bridges.

Yakima County in Washington State will implement a collaborative Nurse-Family Partnership program utilizing the Olds' model of nurse home visiting. An infant mental health speciality component will be added. The purpose of the program is to decrease risk factors by modifying behaviors of mothers targeting 5 primary domains: personal health, environmental health, maternal role development, maternal life course, and friend and family support. Outcomes for improved birth outcomes, decreased child abuse and neglect, decreased child abuse and neglect, decreased dependence on public economic support, and decreased mental health problems of the mother ande infant are expected.

The Yakima County Jail Diversion Program will serve chronically and acutely mentally ill adults ages 18 and older who come into contact with the Yakima County or City of Yakima law enforcement, court or jail systems. The program will utilize a multi-point jail diversion model to avoid or dramatically reduce the length of incarceration for acutely and chronically mentally ill adults. The model will divert individuals from jail at three contact points. The first diversion point will be the initial law enforcement contact (pre-booking). Chronically or acutely mentally ill adults (including those with co-occurring disorders) may be diverted prior to booking to a voluntary plan and voluntary mental health services. The second diversion point will be at arraignment (post-booking). The target population who have been arrested and who are identified as potential candidates through a mental health assessment would be considered for diversion. The third diversion point will be at adjudication (post-booking/post adjudication). Acutely or chronically mentally ill individuals with complex mental health or co-occurring disorders and who have need of more complex or long-term interventions are targeted at this diversion point. In order to implement the diversion model outlined above, Yakima County will re-direct and expand its existing jail mental health/substance abuse services to a diversion focus. Yakima County will partner with Central Washington Comprehensive Mental Health.

The Puyallup Tribal Health Authority (PTHA) Child and Adolescent SIG project will develop the infrastructure for full implementation of a wraparound service delivery model, which is culturally appropriate and cost effective for American Indian/Alaska Native (AI/AN) youth that are struggling with substance abuse and mental health problems, such as serious emotional disturbances, in the Pierce County service delivery area. The PTHA will use the grant funds to build the infrastructure to promote, support, and sustain the mental health and substance abuse treatment intervention capabilities for the target population.

This project will continue the State's effort to build infrastructure to collect data and report the remaining Mental Health Block Grant Uniform Reporting System Developmental Measures. Grant efforts will focus on (1) local provider training to improve data quality, (2) implementation of web-based technology using DS2K + data standards to collect, report, and improve accessibility of data, and (3) strengthening internal and external database linkages. Project outcomes will include consistent data definitions, timely capture of data, improved measure of service outcomes and client change, improved data quality, and enhanced ability to analyze and report on developmental measures such as school attendance, school performance, and involvement with the criminal justice system. The project outcomes will be evaluated based on the ability to produce the data required for URS and other desired reporting. The project will also be evaluated in terms of its ability to produce data that is useful to and is used by system stakeholders.

The project will implement best practice alternatives to seclusion and restraint in Washington State. This will be accomplished through a multifaceted program of training and changes in policies and practices at all three of the state's public inpatient psychiatric hospitals. Western State Hospital and Eastern State Hospital care for adults, older adults and forensic populations; the Child Study and Treatment Center cares for youth under age 18, including forensic and non-forensic patients. The goal of this project is to implement best practice alternatives in 100% of the state hospital wards and units, resulting in the complete elimination of seclusion and restraint episodes. Hospital leadership attendance will participate in workshops offered by the National Technical Assistance Centers, followed by diffusion of training for all staff throughout the facilities. Training includes development of facility- specific plans that address the variety of interventions needed to reduce seclusion and restraint, including interventions addressed to leadership and organizational change, effective use of data, workforce development, use of seclusion/restraint reduction tools, consumer empowerment, and debriefing techniques. A process and outcome evaluation will be conducted by an experienced research institute in close consultation with the national evaluation center at HSRI and with the hospitals' advisory boards to evaluate the effects of the project and to assess process strengths and weaknesses and make ongoing improvements in project operations.

The Lummi Nation is seeking funding to support a Coast Salish on Prevention of Addictions tentatively scheduled for September of 2005. A Coast Salish Gathering is a meeting the Tribal People, communites and governments servicing Coast Salish Peoples. Coast Salish peoples live along the coast of the area that is now known as Oregon, Washington States and the province of British Columbia. The approach to developing a gathering is traditionally dictated so that there will be a cross section of the community who will attend. This includes clients, interested family members of the client, professional services providers, interested Tribal members from the Lummi Nation and from other Coast Salish Tribal Communities.

Neighborhood House proposes Project HANDLE (HIV/AIDS) Network Development and Life-skills Experience) to prevent and reduce HIV and substance abuse (SA) among high-risk Cambodian refugee community in Seattle/King County (KC). HANDLE activities include culturally appropriate peer education and intensive services to high-risk youth including tutoring, prevention classes and job skills. Washington State has the third largest number of Cambodian refugees (15,00) in the US with most living in Seattle/KC. The Cambodian American youth of Seattle/KC are at very high risk for HIV/SA because of the favorable attitudes toward drug use, fighting and other and-social behaviors, the HIV/AIDS epidemic in Cambodia is and sexual promiscuity. Local needs assessment data show that compared to American youth, Cambodian American youth are nearly three times more likely to have sexual intercourse and 93% of the Cambodian-American youth do not talk to their parents about HIV/STIs infection, sex or reproductive health problems. The incidence of reported AIDS cases in 2001 among Asians in Seattle is double the national rate for Asian/Pacific Islanders (Seattle/KC Public Health Department, 2001). Of the 37 HIV non-AIDS cases in Seattle King County, 19% are Cambodian refugees. The goals of HANDLE are to: 1) reduce high-risk behaviors and increase protective behaviors for high-risk Cambodian youth and their families; 2) increase parent's bicultural parenting skills and knowledge of HIV/SA; 3) increase community knowledge and awareness about HIV/SA; and 4) create Cambodian network and resources for HIV/SA. In collaboration with Cambodian Women's Association and other partners, our activities include intensive services to 50 high risk Cambodian youth and their parents including HIV/SA education classes, year-round youth employment and tutoring, parenting classes, community volunteering and youth cultural activities.

The Strategic Prevention Framework State Incentive Grants are used to advance community-based programs for substance abuse prevention, mental health promotion, and mental illness prevention. The SPF SIG implements a five-step process known to promote youth development, reduce risk-taking behaviors, build on assets, and prevent problem behaviors. The five steps are: (1) conduct needs assessments; (2) build state and local capacity; (3) develop a comprehensive strategic plan; (4) implement evidence-based prevention policies, programs and practices; and (5) monitor and evaluate program effectiveness, sustaining what has worked well. These grants will allow the programs to provide leadership, technical support and monitoring to ensure that participating communities are successful. The success of the grants will be measured by specific measurable outcomes, among them: abstinence from drug use and alcohol abuse, reduction in substance abuse-related crime, attainment of employment or enrollment in school, increased stability in family and living conditions, increased access to services, and increased social connectedness. Washington State's Strategic Prevention Framework will provide the resources necessary to enhance existing assessment processes, implement and evaluate evidence-based strategies to address critical needs based on epidemiological data, and establish reporting procedures that track progress toward preventing substance abuse and related problems, including mental illness, delinquency, and violence.

In partnership with the Washington Department of Corrections, and several public and private non-profit service providers, Clark County's Department of Community Services and Corrections will establish a continuum of services for young adult offenders designed to ensuring successful community reentry and reducing the occurrence of relapse and recidivism. The Clark County model will be organized around a Program for Assertive Community Treatment and will utilize two research-based dual diagnosis treatment regimens: the Matrix Model and Seeking Safety. A minimum of 60 participants per year will benefit from the intensive group interaction and support in a structured treatment setting afforded by the Matrix and Seeking Safety programs. The project will incorporate a broad range of services to effectively address all risk factors for the young adults ages 18-24, including housing and targeted education and employment assistance. All services will be provided using Washington State's version of the Wrap-Around Approach, Individualized and Tailored Care.

The Division of Alcohol and Substance Abuse (DASA) proposes to utilize funding from the CSAT Adolescent Substance Abuse Treatment Coordination grant to develop a Statewide infrastructure that fosters cross system planning, and knowledge and resource sharing to enhance the existing adolescent substance abuse treatment system. Specific grant funded activities include: - Develop and staff a Statewide Leadership Council (SLC) on Adolescent Substance Abuse Treatment to direct, coordinate and oversee efforts to integrate substance abuse treatment services across all systems statewide. Council membership will include State leaders from child welfare, mental health, juvenile justice, education, and health. Youth and family consumers will also be recruited to participate. - Develop a subcommittee structure that reports to the SLC and involves statewide collaboration to: improve certification and licensure standards for programs, facilities, and counselors; enhance training opportunities, resources and materials related to evidenced based practices; and develop strategies to identify resources and gaps related to unmet needs. - Create a new full time staff position with the authority and leadership support necessary to assess and facilitate ongoing, self-sustaining, and cross system planning and development. - Conduct a needs assessment and prepare a State strategic plan to guide and sustain the development of coordinated substance abuse treatment for youth residing in Washington State. - Create and sustain training resources, materials and opportunities, especially in the areas of screening and referral and implementation of evidence-based practices. - Strengthen and sustain the role and visibility of the existing adolescent substance abuse provider association. - Develop, promote, and coordinate with family recovery support organizations.
