Mental health intervention is the step necessary to stop the progression from troubled youth to risk-taking behavior to suicide attempt. The Berrien County Health Department, in collaboration with the Benton Harbor Area Schools and the Berrien County Mental Authority (Riverwood Center) will enhance the identification and referral of high-risk adolescents into school based primary care centers for mental health treatment in Benton Harbor, Michigan. Youth in Benton Harbor are at high risk for contemplating and committing suicide. The Berrien County program seeks to improve the link between adolescents at risk to mental health services provided through the school based health clinic at Benton Harbor High School, and through the ChildNet collaborative at Benton Harbor middle schools, by expanding the identification and referral system for at-risk teens. The program will also extensively evaluate the existing program to substantiate its success through outcomes data and analysis. The identification and referral system will be enhanced through four goals: 1) Continue to maintain the high quality mental health services at the school settings, 2) Improve the referral system so that 100% of youth at risk are identified and treated, 3) Empower the administration of Benton Harbor Area Schools to effectively improve the mental health system at the schools, and 4) Fully evaluate the referral system, treatment and outcomes of the program to validate its effectiveness. The expansion of the current model will employ three over-arching strategies 1. Skill-building for school personnel and allied health, families and students 2. Strong referral relationships 3. Preventative direct services by culturally competent, trained mental health clinicians.

The escalation of mental health difficulties and the steady level of suicide completions among college students prompt the call for effective university-based prevention efforts. This comprehensive suicide prevention plan for Western Michigan University seeks to modify aspects of the Air Force Suicide Prevention Program (2001) to a non-military, academic environment. The Air Force program reduced the incidence of completed suicides and attempts among Air Force Personnel and is considered "highly effective" according to SAMHSA. The elements adapted include the utilization of a community-based approach; the use of community leaders to carry out and support the program; the dissemination of information regarding risk factors, symptoms, and referral sources for depression and suicide; and the promotion of help-seeking behavior among students at risk. Proposed activities include the training of campus leaders; the use of culturally sensitive social marketing activities to promote community responsiveness to mental health problems and promote help seeking; and the use of the Web-based technology to educate the community. Special emphasis will be placed upon reaching underrepresented students through the use of their social connections and student groups.

The Southwest Michigan Children's Trauma Assessment Center (CTAC) serves traumatized children through comprehensive assessments and subsequent interventions. To address the potential combined harmful effects of trauma and prenatal alcohol exposure CTAC offers a unique neurodevelopmental trauma assessment that includes fetal alcohol assessment and diagnosis. Following the assessment CTAC offers family meeting to discuss assessment results, pharmacological consultations in home occupational therapy interventions, participation in school meeting, and court testimony. CTAC will build on its current services and will: (1) develop school models that maximize the learning potential and improve social functioning for traumatized children, (2) provide extensive training of diverse professionals, in order to strengthen child responsive systems of care for children, and (3) develop an alexithymia instrument for children.

Kalamazoo Wraps is a partnership inclusive of families, youth and representatives from all child and family system stakeholders in Kalamazoo Michigan. With Kalamazoo Community Mental Health and Substance Abuse Services as the fiduciary, Kalamazoo Wraps will expand and broaden service capacity utilizing culturally competent practices, ensure individualized service plans, and empower children with serious emotional disturbances and their families. The following four priority populations, who have traditionally been the most challenging to serve towards successful and sustained positive outcomes, are children/youth with SED, who have: 1) multiple contacts and/or placements within the Juvenile Justice system; 2) complex and long standing learning, behavioral and/or neurobiological issues; 3) co-occurring substance use disorder; and/or 4) involvement with the child welfare system. All of the children/youth mentioned will receive specific interventions. Minimally, 100 children and their families annually will receive project services steeped in system of care values.

The Transforming Youth Suicide Prevention in Michigan (TYSP-Mi) Program will engage programs/individuals to expand suicide prevention in the state and more strongly emphasize primary prevention and early intervention. The Program will 1) provide technical assistance and grants to local communities for coordinated service delivery; 2) develop a cadre of trainers in established gatekeeper and professional programs; and 3) build a strong state infrastructure. Awareness activities include implementing a health communication campaign to reduce suicide among 19-24 year olds who are not in an academic setting; coordinating a Train-the-Trainers Program to improve statewide access to gatekeeper trainers and to trainers qualified to teach the new clinical core competencies workshop for behavioral health professionals; and convening the TYSP-Mi Program Work Group to provide oversight and expert input for the Program.

Integrating Multiple Paths Achieves Comprehensive Children's Treatment (IMPACCT), a collaborative of child-serving agencies in Ingham County, Michigan seeks to create an integrated, effective, efficient, and transformed system of care for youth with severe emotional disturbance (SED) that will reduce the need for out-of-home placements and psychiatric hospitalizations, and assist youths and families to achieve the highest possible level of functioning by providing coordinated, individualized, strength-based, family-driven, youth guided, and culturally and linguistically competent care. IMPACCT proposes to build on current strengths; provide a unified structure for community-wide collaboration and system reform; and establish a common assessment and outcomes measure for children and families entering mental health, juvenile justice, and child welfare systems. IMPACCT will develop a 6-year strategic plan for system reform to meet the needs of all youth with SED in our community. IMPACCT will utilize training and technical assistance, social marketing, ongoing evaluation, innovative funding mechanisms, best practice models, and evidence-based interventions to meets its goals.

This project will continue the State's effort to build infrastructure to collect data and report the remaining Mental Health Block Grant Uniform Reporting System Developmental Measures. Grant efforts will focus on (1) local provider training to improve data quality, (2) implementation of web-based technology using DS2K + data standards to collect, report, and improve accessibility of data, and (3) strengthening internal and external database linkages. Project outcomes will include consistent data definitions, timely capture of data, improved measure of service outcomes and client change, improved data quality, and enhanced ability to analyze and report on developmental measures such as school attendance, school performance, and involvement with the criminal justice system. The project outcomes will be evaluated based on the ability to produce the data required for URS and other desired reporting. The project will also be evaluated in terms of its ability to produce data that is useful to and is used by system stakeholders.

An innovative Integrated Collaborative Chronic Care program addresses the mental and physical health needs of an urban population of persons living with HIV/AIDS that is primarily African American and uninsured/underinsured. The program draws from integrated and chronic care models to engage patients in early and ongoing assessment for mental health conditions leading to culturally relevant and comprehensive treatment in an HIV primary care clinic. A multi-disciplinary team of primary and mental health providers along with each individual client establishes a culturally relevant treatment plan engaging the client to achieve self-management skills to improve mental and physical health. Components include screening for mental health disorders, and establishing an integrated treatment plan including the patient as an active participant.

Community Health Awareness Group, (CHAG) will offer a range of HIV-related mental health services geared to the cultural norms and expressed symptoms of HIV positive African Americans with or without a DSM diagnosis. Delivered by therapists trained in HIV and indigenous staff skilled in reaching the target, the program will improve service integration, maximize availability of needed services and minimize fragmentation and frustration for the clients. Comprehensive psychological evaluations will be conducted by a mental health therapist to determine level of care needed and psychiatric evaluations will be scheduled as needed. A trained HIV case manager will also conduct a complete biopsychosocial assessment in concert with the therapist. Both will incorporate information from other sources with client consent. Weekly individual therapy will be provided in conjunction with ongoing group therapy treatment. Ongoing case management will address all assessed service needs.

The Strategic Prevention Framework State Incentive Grants are used to advance community-based programs for substance abuse prevention, mental health promotion, and mental illness prevention. The SPF SIG implements a five-step process known to promote youth development, reduce risk-taking behaviors, build on assets, and prevent problem behaviors. The five steps are: (1) conduct needs assessments; (2) build state and local capacity; (3) develop a comprehensive strategic plan; (4) implement evidence-based prevention policies, programs and practices; and (5) monitor and evaluate program effectiveness, sustaining what has worked well. These grants will allow the programs to provide leadership, technical support and monitoring to ensure that participating communities are successful. The success of the grants will be measured by specific measurable outcomes, among them: abstinence from drug use and alcohol abuse, reduction in substance abuse-related crime, attainment of employment or enrollment in school, increased stability in family and living conditions, increased access to services, and increased social connectedness. The Michigan Strategic Prevention Framework State Incentive Grant will support and enhance efforts to identify, coordinate and leverage resources to address prevention challenges. A crosscutting theme in this effort will be enhancing the effectiveness of prevention services through the increased use of evidence-based strategic planning models and intervention strategies.

The goal of this grant is to enhance and strengthen the capacity and capability of the Michigan Methamphetamine Control Strategy Task Force and, therefore, enable our state to employ a statewide multi-agency infrastructure for the purpose of planning and implementing strategies effective at preventing, reducing or delaying the use or spread of methamphetamines in high risk communities in the state of Michigan. The Task Force was formally convened in September 2002, by the Michigan Department of Community Health, Office of Drug Control Policy (MDCH/ODCP), and consists of a group of leaders from various disciplines across the state, to further develop a statewide strategy and implement programming aimed at attacking methamphetamine use and production.

SHAR has been providing substance abuse treatment to the indigent population since 1969, and currently provides residential, outpatient, and aftercare services to more than 1,000 people each year.

STAND Program The Third Circuit Court of Michigan's Specialized Treatment for Alcohol and Narcotic Dependency (STAND) program is designed to assist juveniles to reduce substance use and abuse and related non-violent delinquent behavior by utilizing a structure of strength based treatment services; and intervention, court supervision and community support in order to enhance public safety and return productive and responsible citizens to their families and community. Funds are being sought to: 1) increase the number of youth who can participate in the STAND program; 2) strengthen substance abuse treatment effectiveness through the use of evidence-based assessment and treatment models; and 3) improve the quality of life for youth at home, in school and in the community. Each year, the project proposes to serve 110 youth through the three drug court teams. Outcomes for this project include reduced marijuana use, reduced recidivism, improved school attendance and performance and improved social and family relations.
