This project will continue the State's effort to build infrastructure to collect data and report the remaining Mental Health Block Grant Uniform Reporting System Developmental Measures. Grant efforts will focus on (1) local provider training to improve data quality, (2) implementation of web-based technology using DS2K + data standards to collect, report, and improve accessibility of data, and (3) strengthening internal and external database linkages. Project outcomes will include consistent data definitions, timely capture of data, improved measure of service outcomes and client change, improved data quality, and enhanced ability to analyze and report on developmental measures such as school attendance, school performance, and involvement with the criminal justice system. The project outcomes will be evaluated based on the ability to produce the data required for URS and other desired reporting. The project will also be evaluated in terms of its ability to produce data that is useful to and is used by system stakeholders.

The Mental Health Center of Dane County Adolescent Trauma Treatment Program will improve the quality, effectiveness, and availability of therapeutic services for adolescents who have experienced traumatic events by identifying, developing, improving, providing and evaluating effective adolescent trauma treatments and services. The program will focus on the relationship between adolescent trauma and substance abuse. The Center will develop integrated early identification services for traumatized adolescents who are also involved in substance abuse across the Dane County adolescent service system. In order to improve access to these services, the program will also provide training and consultation on child/adolescent trauma to a wide array of governmental and community agencies to improve assessment and intervention services. The program will share the lessons of its efforts through local, state and national collaborations.

Link for Life is a multidisciplinary collaborative effort of Marquette University faculty, staff, and administration. The program will focus on the issues of suicide and those mental health issues that impact suicide by providing suicide prevention training and education to administration, faculty, staff, and the general student body. The specific goals of this project are: 1) To raise awareness, across campus and among parents, about suicide among young people and to increase knowledge about mental illness. 2) To use a nationally recognized suicide prevention model (Question, Persuade, Refer [QPR]) to train Gatekeeper Trainers. 3) To enhance partnerships between the university and its students, as well as community mental health providers. 4) To review existing intervention efforts employed on other campuses. 5) To extensively evaluate he project under the guidance of a Project Evaluator to ensure the activities are as effective as possible and to reduce the potential loss of lives.

Wisconsin will utilize a mentoring and expert consultation approach to develop culturally sensitive, cross systems and consumer inclusive projects in 10 communities with elevated risk of youth suicide and then disseminate information on best practices through regional sharing sessions, a toolkit, a list serve and a statewide conference. This process will create an infrastructure and increased capacity to support the development of further projects. Wisconsin will educate all project sites on identifying at-risk groups in their communities but will focus particular attention on the following three target populations with elevated risk for suicide: Native American youth, youth who are deaf/hard of hearing, and youth in rural areas. Project sites will be selected through a competitive application among the 55 local entities, including four tribes, which submitted letters of interest. Project sites will be required to include the major youth-serving systems as part of their local coalitions, as well as including parents, mental health consumers, public health and primary care, faith communities and other stakeholders. Involvement of survivors of suicide will be strongly encouraged.

Milwaukee Health Services, Inc. (MHSI), is a private non-profit Community Health Center, serves as lead agency of a community-based collaboration with the AIDS Resource Center of Wisconsin and Stanford University to improve access to culturally competent mental health services for people living with HIV/AIDS in southeast Wisconsin. This five-year project will engage a minimum of 800 people living with HIV/AIDS with mental health problems and their family member/partners in a comprehensive continuum of mental health services focused on the following outcomes: reduction of symptoms of depression/anxiety, reduction of substance abuse and HIV risk behaviors, and improved health and well being. The cooperative approach of this application coordinates HIV primary and specialty medical care, behavioral health services, scientifically driven program evaluation, psychosocial case management services, AIDS social services and health education.

The University of Wisconsin Oshkosh Counseling Center will develop a Comprehensive Suicide Prevention and Mental Health Program, which will fundamentally transform the University’s approach to students experiencing mental and behavioral health problems which lead to depression, substance abuse, suicide attempts and academic failure. This collaborative effort will better meet the needs of at risk populations that are currently underserved and often inadequately treated. Preventing college student suicide is a daunting challenge. The Wisconsin Medical Journal (2004) reports that, at 5.7 per 100,000, suicide is the leading cause of death among Wisconsin’s 15-24 year olds (a rate 36% higher than the national average). UWO will develop a Comprehensive Safety and Mental Health Partnership which will develop clear communication structures and policies which ensure an active coordinated response that maintains safety. We will also: develop a social norms campaign to reduce barriers to help seeking and increase student knowledge of resources train campus staff and student leaders, and weave mental/behavioral health assessment into the fabric of services provided across campus. We will target populations identified by the Suicide Prevention Resource Center as high risk and for whom the nature of this campus creates barriers to accessing care. The target populations are: students of color, international students; lesbian, gay, bisexual, transgender and questioning (LGBTQ); non-traditional age, first generation; academically failing, alcohol abusers; or depressed will make our students more academically successful and improve retention.

The rural code coalitions mission is to take a community wide grass roots approach to reducing substance abuse among youth, particularly youth ages 13-20. This coalition was started by new Paradigm Partners whose mission is to strengthen their learning communities through resource sharing, capacity building, and innovation teaching strategies and collaborate efforts. While the strategies are different , ultimately the goal of both the rural CODE coalition and New Paradigm Partners is to increase student achievement. The coalition covers approximately 2000 square miles with a population under 5000, serving the communities of Birch wood, Shell Lake, Weyerhaeuser, New Auburn, Winter, and Lac Courte OReilles Reservation all of which arel small, low wealth, rural communities located in the Norwest region of Wisconsin.

The Delinquency Prevention Council serves Jefferson County, Wisconsin, a mid -sized suburban area with a population of 78,300 which is 10 percent Latino. The goals of the program are to reduce alcohol and marijuana use among youth and over time among adults by addressing factors that minimize the risk of substance abuse, and to strengthen governments to support the efforts of the council to prevent and reduce substance abuse among youth, specifically alcohol and marijuana use. The council will achieve these goals by implementing multiple strategies such as conducting the search survey, establish a youth involved media campaign , circulate cultural sensitive information in the community and conduct youth and adult focus groups in the community. These collaborated efforts with key stakeholders will allow the council to continue its history of success.

The Concerned Citizens Coalition on Alcohol and other Drug Abuse serves the great Kenosha County area with a population of 154,234, this is 89% urban and 11% rural. African American and Hispanic residents account for 17% of ethnic groups with 1% of less for others. Coalition objectives are to prevent and reduce substance abuse, reduce underage access to alcoho,l implement science based model programs to increase youth resiliency and refusal skills, build coalition capacity, increase community readiness toward ATOD prevention, and sustain interventions.

AIDS Resource Center of Wisconsin in Milwaukee, WI has received a 5 year Strategic Prevention Framework (SPF) grant to provide SA, HIV and Hepatitis prevention services to minority populations and minority reentry populations. ARCW and leading partners' project purpose is to reduce and eliminate the occurrence of SA, HIV and Hepatitis infection among the minority and re-entry populations of the Milwaukee- Waukesha MSA. ARCW's leading partners will be Wisconsin Correction Services, the state's leading Community service provider working with re-entry individuals and two minority community-based organizations, 16th Street Community Health Center and La Casa de Esperanza, to reach minority individuals. ARCW and its partners will work to reduce SA, including alcohol use, methamphetamine use, and other injection drug use, that can result in sex and drug use behaviors placing individuals at-risk for HIV and Hepatitis. With 20 years of experience providing prevention services to individuals at- risk, ARCW will extend prevention programming to reach a greater number of at-risk individuals. Upon completion & approval of the needs assessment & strategic plan, ARCW and its partners will implement prevention interventions that include: Providing SA, HIV, and Hepatitis prevention outreach through 10,800 contacts annually. Providing HIV and Hepatitis testing & referral services to individuals in the re-entry and minority communities, conducting 2,800 HIV and 1,800 Hepatitis tests annually. Screening 1,168 re-entry and minority individuals annually for substance abuse and make appropriate referrals for SA treatment. Conducting multi-session SA, HIV and Hepatitis prevention education and counseling among the re-entry and minority populations reaching 880 individuals annually. Conducting capacity building assistance annually to 48 organizations serving re-entry minorities & minority individuals to provide SA,HIV & Hepatitis prevention.

The target population for the coalition is the county's 38,000 youth ages 5-18. The goals of the program are : Goal 1) prevent or delay first use of tobacco, alcohol ,and marijuana by youth ; limit the access of underage youth to alcohol ; reduce high levels of marijuana by youth.

This coalition serves Westfield and Montello School District(s) Combined districts have an ethnic makeup of 93.7% White, 4.5 % Hispanic , and 1.8% combined American Indian , African American, and Asian/Pacific Islander. The goals of the program are to: 1) Establish and strengthen collaboration among communities and agencies to support the efforts of community coalitions; and to prevent and reduce substance abuse among youth. To achieve these goals, the coalition plans to implement information dissemination, environmental strategies, community based processes and prevention Education with an emphasis on Early Education.

The Bad River Reservation in northwestern Wisconsin , the largest Chippewa Reservation in the state, encompasses an area along 17miles of Lake Superior shoreline in Ashland County. The 124,334- acre reservation was established in 1854. Total tribal membership (on and off reservation) consists of 6,374. The seat of tribal government is located in Odanah, approximately 18miles east of Ashland. The bad river Family Preservation seeks to serve tribal youth ages 11-15. The focus of this prevention programming will be on reducing substance abuse among youth and strengthening collaboration and relationships with various organizations and sectors of the community changing community attitudes. The coalition will make an effort to recruit , hire, and train Native American staff and volunteers to become involved in both the planning and implementation phases of this program.

The Positive Alternative Coalition (PAC) serves all of the Vilas and portions of Oneida Counties Wisconsin, a rural area with a population of 30,301 of which 9% is represented as American Indian. PAC is housed in the central office of Great Lakes Inter Tribal Council, a consortium of the 11 American Indian tribes of Wisconsin. The PAC service area includes three school districts, covering 1300 square miles and drawing students from 19 small rural communities. The goals of the program are to reduce substance abuse by: facilitating and enhancing collaboration among all age groups and cultures in the community; targeting multiple drugs; and establishing and strengthening collaboration among rural communities, community coalitions, and non profit organizations, federal, state and local tribal governments of the Great Lakes Inter-tribal Council.

The Department of Health and Family Services, State of Wisconsin requests federal funding for each year of a three-year grant period through the Strengthening Treatment Access and Retention-State Implementation program. The Wisconsin Department of Health and Family Services proposes to implement Plan-Do-Study-Act process improvement projects in at least 15 outpatient substance abuse treatment providers having at least 150 admissions to improve access to and retention in services. At the state level, the Department will work to reduce fiscal, regulatory, and other barriers to implement provider process improvement projects, thereby increasing access to and retention in services.

Project Fresh Light will bring new energy, emphasis and effectiveness to the treatment, services and supports received by Wisconsin's adolescents with substance abuse and co-occurring disorders and their families by improving the provider network, integrating service administration, resolving funding and regulatory barriers and improving data management. First, the State will develop a community of practice that supports and sustains the use of culturally competent, research-supported and outcome-focused adolescent treatment. Second, the Department of Health and Family Services will create an interdepartmental workgroup of system experts to create solutions to improve the fiscal and regulatory climate in supporting adolescent substance abuse services. Third, the Department of Health and Family Services will allocate a full-time state level position to coordinate efforts to improve service administration and delivery at both the State and county levels. Fourth, the Bureau of Mental Health and Substance Abuse Services and the Office of Justice Assistance will establish an information management and evaluation system to provide reliable evaluation and quality assurance measures for this project.

The program will establish an afternoon/evening Day Reporting Center for substance abusing youth aged 14-17 who are returning to the community after being sentenced to incarceration or detention. Racine County intends to serve 300-400 youth by providing alcohol and drug counseling, academic tutoring, career development, independent living skills and family therapy.

The State of Wisconsin will enhance delivery of substance abuse screening, brief intervention, referral, and treatment (SBIRT) services, in primary healthcare settings across the state. Implementation will adhere to the Chronic Care Model and recommendations by the Institute of Medicine. The project will emphasize development and ongoing improvement of durable systems that provide culturally competent services in a standardized manner. Wisconsin Initiative on Promoting Healthy Lifestyles (WIPHL) will sustain and enhance the delivery of SBIRT services for adolescents and adults, initially in 22 clinical sites affiliated with five umbrella organizations, then in additional sites of those organizations, and then throughout Wisconsin. "Champions" at each clinical site will help select two implementation team leaders. After training, the leaders will use a workbook to guide their teams in implementing and improving SBIRT services in monthly plan-do-study-act (PDSA) cycles. WIPHL staff will support clinical site efforts with monthly conference calls, semi-annual meetings, and site visits. WIPHL site staff will systematically administer brief, multi-health behavior screens, using electronic medical record systems, written forms, or automated telephone systems. Patients with positive brief substance abuse screens will undergo fuller screening and receive additional SBIRT services from on-site counselors, who receive 3 weeks of initial training, ongoing supervision, and regular feedback. Counseling protocols will be based on the trans-theoretical model of change, motivational interviewing principles, and additional strategies to enhance cultural competence.

Meta House, Inc. will enhance and expand its gender-appropriate treatment services for homeless women with co-occurring disorders and their children. The Meta Housing program will partner with two other Milwaukee housing programs for women to form the Housed in Recovery (HIR) program. Services include: maintain recovery commitment, improve mental health functioning, increase housing stability, and improve stability of the family. It has a gender and culturally responsive treatment predicated on the distinctive characteristics of the female physiology and women's roles, socialization and relative status within the larger culture.
