Kentucky Consumer Advocate Network proposed to educate, support and provide advocacy services for consumers. The organization proposes to develop drop in centers, educate state leaders about recovery, provide recovery education to family members and expand education and outreach to hospitals and mental health facilities. During the project KYCAN proposes to expand membership, increase the number of county affiliates and the number of consumers employed on the project, and recognize the collaborative efforts of partners in recovery efforts.

CATTI is a multidisciplinary, University-Community collaboration housed at the University of Kentucky's Center for the Study of Violence against Children. This project builds on an already nationally-recognized child assessment and treatment center in Kentucky, and proposes to develop a clinical nucleus for clinical training, and dissemination of trauma informed evidence-based practices (TI-EBPs) in four rural to urban areas in Kentucky. The CATTI project will provide three interventions that have been identified as best practice approaches, and provide a continuum of care to traumatized children: Parent Child Interaction Therapy (PCIT) for ages 2-12), Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) for ages (3-17), and Abuse-Focused Cognitive Behavioral Therapy (AF-CBT) (for school aged children).

Kentucky Partnership for Families and Children, Inc.'s (KPFC) vision is to improve the quality of life for children, youth, and young adults facing emotional, social, and behavioral challenges and to encourage the healthy functioning of their families through education, advocacy and outreach. As the family organization in Kentucky, KPFC collaborates with many state-level systems to build a strong "family-driven" and "youth-guided" system of care. Through this next grant cycle, KPFC will continue to strengthen organizational relationships by collaborating with a multittude of public agencies and private organizations across the state on committees, projects, and collaboratives. KPFC will teach leadership skills among youth that have SED and their families through trainings across the state, a quarterly newsletter, a resource request line, referrals to support groups, and the annual Youth/Parent Leadership Conference. KPFC will prepare these parent leaders and youth leaders to more fully participate at the community, regional, and state-levels as a family and youth voice to ensure that Kentucky has a "family-driven" and "youth-guided" system of care.

Building upon the strong interagency infrastructure of the Kentucky IMPACT Program and the school-based mental health model established through the Bridges Project, Kentuckians Encouraging Youth to Succeed (KEYS) will facilitate the implementation of a continuum of positive behavior interventions and supports in targeted schools in the North Central region of Kentucky to address the mental health and substance use needs of its youth. Recognizing the unmet needs of youth with co-occurring mental health and substance use disorders, KEYS will place particular emphasis upon employing evidence-based strategies to effectively identify and treat this challenging population. Additionally, KEYS will address identified gaps in the available service array for youth with serious emotional disabilities, including early childhood, crisis stabilization, day treatment, adolescent substance abuse, intensive home-based, and respite. A comprehensive evaluation system will be in place to support continuous improvement of the model and to support sustainability and statewide replication of the system of care.

The purpose of Kentucky Suicide Prevention in Youth - a Collaborative Effort (SPYCE) is to raise awareness, enhance quality interventions, and utilize effective research and data collection methodologies to reduce the number of suicides completed by the youth in KY. Many activities will be implemented statewide, while others will focus within one community mental health center region to be chosen through an RFP process. SPYCE's goals are to: (1) increase knowledge of suicide warning signs, risk factors, and protective factors; (2) enhance suicide crisis intervention availability, quality, and public awareness of service; (3) increase local community collaboration in suicide prevention; (4) establish and increase support for suicide survivors through postvention and prevention; (5) increase resiliency among youth at risk; (6) enhance existing emergency department services for youth and their families; and (7) provide additional training in clinical suicide risk management to medical/mental health professionals and paraprofessionals across the state.

In collaboration with the University of Louisville WINGS Clinic, the Kentucky Department of Corrections and Volunteers of America, the Connectimmunity Project will expand and enhance the Creating Lasting Family Connections substance abuse prevention curriculum to integrate and increase access to HIV and Hepatitis prevention services. The project will focus on minority children and youth ages 12 - 17 years and include males and females reentering the community from correctional facilities.

The Strategic Prevention Framework State Incentive Grants are used to advance community-based programs for substance abuse prevention, mental health promotion, and mental illness prevention. The SPF SIG implements a five-step process known to promote youth development, reduce risk-taking behaviors, build on assets, and prevent problem behaviors. The five steps are: (1) conduct needs assessments; (2) build state and local capacity; (3) develop a comprehensive strategic plan; (4) implement evidence-based prevention policies, programs and practices; and (5) monitor and evaluate program effectiveness, sustaining what has worked well. These grants will allow the programs to provide leadership, technical support and monitoring to ensure that participating communities are successful. The success of the grants will be measured by specific measurable outcomes, among them: abstinence from drug use and alcohol abuse, reduction in substance abuse-related crime, attainment of employment or enrollment in school, increased stability in family and living conditions, increased access to services, and increased social connectedness. Kentucky's Strategic Prevention Framework Incentive Grant project will build a data-driven, comprehensive, interagency prevention service system to reduce substance abuse and its related problems. The State will make extensive use of geographic information system (GIS) technology to focus community goal-setting and enable targeted resource allocation.

The purpose of the Volunteers of America of Kentucky, Inc.'s Grace House Program continues to be to expand and enhance services to address a well-documented service need within the existing continuum of care available to Louisville, KY women substance abusers who are pregnant, postpartum, and/or parenting children to age 17. A full range of services that have been shown to be most effective for this treatment population will be provided to address the special needs of clients in a safe residential setting, including substance abuse, medical, employment and case management services. Treatment will be phased and individualized, including, group/individual therapy, parenting training, family therapy, case management, and life-skills training. Specific program goals are to eliminate/reduce substance abuse use/relapse; eliminate/reduce HIV/AIDS risky behaviors; improve psychosocial and family functioning improve health and development of infants/children; improve mothers' quality of life indicators; improve safety/stability of family, reduce criminal behaviors. We will serve an additional 35 women and an estimated 105 number of children up to age 17.

Louisville, KY continues to experience an increase in HIV case rates within the minority community, with a minority case rate in excess of 25 per 100,000. The scope of this project includes: increasing the number of counselors; developing African American or other minority alcohol and drug abuse counselors; utilizing outreach to increase treatment enrollment; implementing a motivational approach to increasing client motivation to enter treatment; and utilizing ongoing evaluation to improve service provision to clients.

Northern Kennedy Drug Court plans to serve 65 juveniles, ages 14 to 18, per year for three years (total of 195 juveniles over three years) in hopes of increasing abstinence, decreasing recidivism, improving life skills, expanding and maintaining resource bases, and increasing community support for drug court operations. The target population is 97% White and 2% African American. The project aims to use a combination of substance abuse treatment, case management, individual, group and family therapy, and wraparound services to accomplish its goals.

The Kentucky Youth First Project is focused on enhancing infrastructure supports for adolescent substance abuse treatment in a rural State that includes an Appalachian area. The project will establish a full-time Adolescent Treatment Coordinator, a liaison with the newly formed Kentucky Office of Drug Control Policy, and specialist staff to help develop better trained and qualified treatment providers. With an overall purpose of serving more adolescent clients and improving the quality of care, this project has three specific goals: (1) to enhance linkages among public and private agencies to better identify, refer, intervene with and treat adolescents who have substance use disorders; (2) to increase training and clinical consultation supports with clear target expectations for adolescent treatment providers by encouraging the use of best practices; and (3) to increase the uses for adolescent specific client-level data on characteristics of adolescents entering treatment as well as follow-up outcomes on clients who have received treatment. These data will be used to strengthen and coach clinical providers in using data to guide treatment efforts and to compare outcomes for adolescents with those of adults. The project will develop memoranda of agreement between key linkage agencies and will strengthen training and consultation resources so that there is a greater clinical presence regarding adolescent treatment in State-funded programs. In addition, the project will use intake and follow-up data to help clinicians and policy makers better understand the characteristics of clients entering treatment and their needs.

The "Family Futures" project will focus on women with substance abuse problems who also face custody problems with their young children (up to age 5). The project will use Drug Court supervision coupled with intensive treatment and case management to address substance abuse problems, co-occurring disorders, and parenting issues. The target population will be 105 mothers and 154 children over the course of the three year grant; 30 mothers and 44 children in the first year, 40 mothers and 59 children in the second year, and 35 mothers and 51 children in the third year, totaling 259 clients overall. The proposed ethnic breakdown is: 60% White; 35% African American; and 5% Hispanic.

The Hope Center's proposed project will provide outreach, stabilization, substance abuse treatment, and recovery (wraparound) services to homeless women with substance abuse or co-occurring disorders. The proposed project represents a collaborative effort between the Hope Center and Chrysalis House, a licensed substance abuse provider, and over the 5 year period the project will provide residential recovery and treatment services to 150 women annually.

Chrysalis House, Inc. will expand and enhance substance abuse treatment services to pregnant and postpartum women and their children. Chrysalis House uses holistic, evidence-based practices that are gender-specific and culturally appropriate for women and age appropriate for their minor children. This approach is comprehensive and addresses the biologic, psychological, social, and spiritual issues that families bring to treatment. Chrysalis House provides expert care for low income women who are age 18 and over, substance dependent, and with limited access to quality health services. The goal of the proposed project will be to provide residential substance abuse treatment comprehensive services to an additional 20 women and 20-60 children for a one-year period. The program will be enhanced with further emphasis on family of origin issues, healthy relationships, and parent coaching.

Assertive Community Living for Appalachian Dual Diagnosed Adults uses an evidence-based integrated services delivery method as the cornerstone for treatment, coupled with recovered-focused assertive community treatment and health services. The project aims to provide integrated mental health and substance abuse treatment through assertive community treatment and culturally sensitive care for up to 120 Appalachian adults (per year), aged 18 and over, who have co-occurring substance abuse and severe mental illness. The ethnicity of the region in Appalachia is 98% white and the project expects to treat this population with a small percentage of Latino, Native American, African American or Asian. The population is socio-economically disadvantaged.
