The Granite State Federation of Families for Children's Mental Health, in collaboration with the Alliance for Community Supports, Inc., will create, implement and evaluate a statewide, peer-to-peer program of mentoring and empowerment for youth with serious emotioal disturbances and their families, and will address our internal cpacity to track and use data more effectively. This initiative will be called "Family and Youth Network Development (FYND).
Historically, these two organizations have worked collaboratively to provide family and youth leadership and voice, as well as training and technical assistance functions to integrated initiatves of the state Departments of Education and Health and Human Services and other groups concerned with youth with serious emotional disturbances (SED) and their familes. Together they provide leadership training for youth with SED. Additionally, the Granite State Federation of Families, as the state's only organization dedicated to youth with SED and their families, provides education, support and mentoring to youth and families who are clients of the community mental health system through the Family Mentor Program. Our challenges lie in providing support to all NH famiilies, not just those engaged with their community mental health center, and in sustaning a pool of highly trained, active, and well supported youth leaders. We propose to address these challenges by enhancing and building on two existing programs and engaging technical assistance for increased capacity to gather and utilize data for quality improvements and growth.

This proposal has two parts: The first part will provide two trainings, one to counselors, health
service providers and selected others to become certified QPR gatekeeper trainers and the second to give counselors and nurse practitioners specialized skills in the assessment and management of suicidal clients. Those certified as QPR gatekeeper trainers will themselves train resident advisors and directors, orientation staff, fraternity and sorority members, coaches and other students and staff who have on-going, close interactions with students to recognize the signs of suicide, question and persuade the person to seek help, and to refer to appropriate resources.
The second part of the proposal provides for the development of a mandatory suicide assessment protocol for students who have attempted suicide, are openly threatening suicide, or who have been transported to the hospital for alcohol poisoning. It will provide release time for counselors to see suicidal student or serious alcohol abusers to assess their level of safety, look at coping mechanisms and devise a plan to help them stay in school safely, and to connect them with all appropriate resources. For those who have been transported for alcohol poisoning, it will include alcohol assessment and education sessions.

BBH plans to add new sources to its collection of client level data. BBH also plans press for the use of standard data definitions and the elimination of unknown data. Data quality will also be increased by contracting out the consumer survey. BBH will increase the number of data items collected by implementing collection of Supported Employment data. BBH will utilize estimation technology to complete various data items and to adjust for duplication. BBH will support increased use of data by the MH PAC. First, it will arrange for training for the State MH PAC and second, it will collaborate with the Council on using data to set service provision and consumer outcome targets.

The goal of The New Hampshire Project for Adolescent Trauma Treatment (PATT) is to implement and evaluate best practices for severely emotionally disturbed adolescents who have experienced trauma and who are served by the community mental health system in New Hampshire.Assessment and treatment for post-traumatic reactions and problems not now systematically provided in the 10 lead community mental health centers serving the State. Community agencies and providers currently lack the information and skills to provide best practices adolescent trauma services. PATT goals will be accomplished through a sequential process of: needs assessment; community outreach to local constituencies including consumers; training of community mental health providers with technical assistance from NCTSN partners; assessment of treatment effectiveness and planning for sustainability of service system change. PATT will educate, train and supervise community mental health child and family providers to (1) screen for and assess the emotional sequelae of trauma exposure (2) increase outreach to traumatized adolescents and their families and (3) implement best practices for trauma treatment across the NH mental health system.Our specific objective is to have best practices for traumatized adolescents adopted and available at each of the 10 NH community mental health centers by the end of the period of grant funding.

The Strategic Prevention Framework State Incentive Grants are used to advance community-based programs for substance abuse prevention, mental health promotion, and mental illness prevention. The SPF SIG implements a five-step process known to promote youth development, reduce risk-taking behaviors, build on assets, and prevent problem behaviors. The five steps are: (1) conduct needs assessments; (2) build state and local capacity; (3) develop a comprehensive strategic plan; (4) implement evidence-based prevention policies, programs and practices; and (5) monitor and evaluate program effectiveness, sustaining what has worked well.
These grants will allow the programs to provide leadership, technical support and monitoring to ensure that participating communities are successful. The success of the grants will be measured by specific measurable outcomes, among them: abstinence from drug use and alcohol abuse, reduction in substance abuse-related crime, attainment of employment or enrollment in school, increased stability in family and living conditions, increased access to services, and increased social connectedness.
The New Hampshire Strategic Prevention Framework Program will reduce substance abuse-related problems in communities by transforming the state system to one that continuously uses the five steps of the Strategic Prevention Framework to deliver appropriate and effective prevention services to prevent abuse of alcohol, tobacco, and illegal drugs.
