This report presents the 2021 data and findings from the National Substance Use and Mental Health Services Survey (N-SUMHSS).
Reports in the Annual Release
Currently, the Annual Releases consist of four main products:
- Annual Report: These reports are produced every year on key findings from the N-SUMHSS
- Detailed Tables: These tables present in-depth findings on the location, characteristics, and utilization of substances and mental health services.
- State Profiles: Each state profile gives summary information on substance use disorder and mental health treatment facilities at the national and state level. This includes facility characteristics, services offered, client numbers, and a mapping of locations in the state.

1. Annual Report
This report gives results from the 2021 N-SUMHSS. N-SUMHSS is an annual survey of substance use disorder and mental health treatment facilities in the United States. The N-SUMHSS is sponsored by the Center for Behavioral Health Statistics and Quality (CBHSQ) of the Substance Abuse and Mental Health Services Administration (SAMHSA). The N-SUMHSS collects data on the location, characteristics, service provision, and utilization of substance use disorder and mental health treatment facilities.
2. Detailed Tables
The Annual Detailed Tables have in-depth findings on the location, characteristics, and function of substance use disorder and mental health services. This information is collected by the N-SUMHSS. The data is intended for use by behavioral health services providers; researchers; and federal, state, and local governments. It gives information on the number and characteristics of public and private substance use disorder and mental health treatment facilities nationwide.

The 2021 N-SUMHSS annual report includes data on key operational characteristics and services provided by substance use disorder and mental health treatment facilities in the United States, its territories, and D.C.
The 2021 N-SUMHSS annual report data were collected from 20,622 eligible substance use disorder and mental health facilities across 50 states, 7 territories, and D.C. These data include information from the following U.S. territories: American Samoa, the Commonwealth of the Northern Mariana Islands, the Federated States of Micronesia, Guam, Palau, Puerto Rico, and the U.S. Virgin Islands.
Data from all substance use disorder and mental health treatment facilities were included in the 2021 report with the following exceptions: Department of Defense (DoD) military mental health treatment facilities; individual private practitioners or small group practices not licensed as a substance use disorder and/or mental health clinic or center; jails or prisons; facilities that do not primarily provide specialty mental health treatment services; and halfway houses. For more details on the N-SUMHSS inclusion and exclusion criteria, please refer to Appendix B of the 2021 N-SUMHSS Annual Report.
Facilities that provide both types of services are included in analyses specific to substance use (SU) and in analyses specific to mental health (MH). Some analyses also include a category for facilities that provide both types of services (SU/MH).These categories are not mutually exclusive since SU/MH facilities are also categorized as SU and MH facilities.
