National Survey on Drug Use and Health: Comparison of 2009-2010 and 2010-2011
Model-Based Prevalence Estimates (50 States and the District of Columbia)

NOTE: The methodology used to conduct significance testing for State estimates between the 2009-2010 and the 2010-2011 NSDUHs and to calculate p values is provided after Table 26.

Revised Mental Illness Estimates: The Substance Abuse and Mental Health Services Administration (SAMHSA) has been publishing State estimates of the prevalence (both percentages and estimated counts) of past year serious mental illness (SMI) and any mental illness (AMI) among adults aged 18 or older since the release of the 2008-2009 State small area estimation (SAE) report. Estimates were based on a model developed in 2008. In 2013, SAMHSA developed a more accurate model for the 2012 data. This revised model incorporates the NSDUH respondent's age and indicators of past year suicide thoughts and depression, along with the variables that were specified in the 2008 model (Kessler-6 [K6] questions on psychological distress and an abbreviated set of questions on impairment in carrying out activities from the World Health Organization Disability Assessment Schedule [WHODAS]), leading to more accurate estimates of SMI and AMI. The SMI and AMI tables in this document include estimates that have been revised based on the new model. Additionally, note that the 2010-2011 small area estimates for these two measures were produced using an updated 2010 sample weight (i.e., one that used 2010 population counts based on the 2010 census rather than the 2000 census in the poststratification adjustment) and updated SAE model predictors for both 2010 and 2011. Other mental health measures in these tables, such as major depressive episode (MDE) and serious thoughts of suicide, were not affected. Methodology Used to Estimate the Bayes Posterior Probability of No Change
between the 2009-2010 and 2010-2011 Small Area Estimates

Comparisons between State small area estimates displayed in Tables 1 to 26 in this document are based on the 2009 through 2011 National Surveys on Drug Use and Health (NSDUHs). The moving average State prevalence estimates for the overlapping 2009-2010 and 2010-2011 time periods were obtained from independent applications of the survey-weighted hierarchical Bayes (SWHB) methodology; that is, the 2010-2011 models were fit independently of the previously fitted 2009-2010 models. This independent analysis approach was followed because there was no desire to revise the previously published 2009-2010 estimates. Moreover, the same fixed predictor variables were used in the 2009-2010 and 2010-2011 models, but annual updates were made when more current versions became available (for details, see Section B.3 in the "2010-2011 National Survey on Drug Use and Health: Guide to State Tables and Summary of Small Area Estimation Methodology" at https://www.samhsa.gov/data/NSDUH/2k11State/NSDUHsae2011/Index.aspx). The age group-specific fixed predictor variables were defined at five levels (namely, the person level, census block group level, tract level, county level, and State level). Also, each age group model had 51 State-level random effects and 300 "within-State" area-level random effects.

Note that [variance v of the natural logarithm of Theta 1 hat] and [variance v of the natural logarithm of Theta 2 hat] used here to calculate [variance v of the estimate of the log-odds ratio, lor hat sub s and a] are the same variances used in calculating the previously published 2009-2010 Bayesian confidence intervals and the 2010-2011 Bayesian confidence intervals, respectively.

The correlation between [natural logarithm of Theta 1 hat] and [natural logarithm of Theta 2 hat] was obtained by simultaneously modeling the 2009, 2010, and 2011 NSDUH data. This simultaneous modeling approach was adopted based on the results of the validation study1 conducted for measuring change in the 1999-2000 and 2000-2001 State estimates. For this simultaneous model, 4 age groups (12 to 17, 18 to 25, 26 to 34, and 35 or older) by 3 years (2009, 2010, and 2011), that is, 12 subpopulation-specific models, were fitted, each with its own set of fixed and random effects. In this case, the general covariance matrices for the State and within-State random effects were 12 × 12 matrices corresponding to the 12 element (age group × year) vectors of random effects. Note that the survey-weighted, Bernoulli-type log likelihood employed in the SWHB methodology was appropriate for this simultaneous model because the 12 age group × year subpopulations were nonoverlapping. The correlation [[natural logarithm of Theta 1 hat], [natural logarithm of Theta 2 hat]] was approximated by the correlation calculated using the posterior distributions of [natural logarithm of pi 1 sub s and a divided by 1 minus pi 1 sub s and a] and [natural logarithm of pi 2 sub s and a divided by 1 minus pi 2 sub s and a] from the simultaneous model.

Note that the small area estimates for serious mental illness (SMI) and any mental illness (AMI) shown in Tables 23 and 24 were revised in October 2013 based on the new 2012 model variables (for details, see Section B.11 in the "2010-2011 National Survey on Drug Use and Health: Guide to State Tables and Summary of Small Area Estimation Methodology" at https://www.samhsa.gov/data/NSDUH/2k11State/NSDUHsae2011/Index.aspx). A decision was made by the Substance Abuse and Mental Health Services Administration (SAMHSA) not to produce revised correlations based on the revised variables. It was expected that the correlations would not be too affected by changes to the modeling procedures used to produce SMI and AMI estimates. Hence, in the calculation of p values mentioned below, the correlations for these two outcomes were based on calculations done using the 2008 model variables; however, updated estimates and variances based on the 2012 model variables were used.

To calculate the p value for testing the null hypothesis of no difference [Log-odds ratio lor is equal to zero], it is assumed that the posterior distribution of lor is normal with [Mean is equal to estimate of the log-odds ratio, lor hat sub s and a] and [Variance is equal to variance v of the estimate of the log-odds ratio, lor hat sub s and a].
