The Clayton Judicial Circuit is an enhancement of the adult drug court and will assist nonviolent offenders with successful rehabilitation from the use of drugs and/or alcohol. The population to be served is high-risk and/or high-needs nonviolent offenders, based on a risk/needs assessment using LSI-R, who have a minimum of two prior felony convictions. The proposed strategies/interventions are to use the Matrix Model, Relapse Prevention Therapy, Thinking for a Change, and Cognitive Behavioral Therapy, combined with sanctions and incentives, intensive drug testing, and community supervision. The goals and objectives of the project are to 1) improve treatment outcomes for the target population of non-violent drug-addicted criminal offenders; 2) eliminate or reduce alcohol and other drug abuse by increasing abstinence among offenders through the use of drug testing in the field during home surveillance visits, at treatment, court, and probation; and 3) reduce recidivism of the target population. The project will include evidence-based principles and practices to maximize program capacity, enhance treatment services, and improve participant education levels. To date, the program has not had any client needing or requesting medically assisted treatment (MAT). The program's treatment provider will assist in locating a qualified physician to oversee the treatment of any client requiring MAT. The program, including the treatment provider will work alongside the prescribing physician to ensure that treatment is continued as long as medically necessary.

The Perry County Drug Court (PCDC) Service Enhancement Project seeks to enhance substance use treatment and mental/physical health services offered within the existing adult Drug Court, only an estimated 11% of those in need receive treatment. Perry County is located in eastern Kentucky, a very rural and agricultural area that has increasing substance abuse and criminal justice problems with few available resources. The area has been designated as a Promise Zone by President Barrack Obama, acknowledging the pervasive poverty in the area and establishing the area as a priority for funding. At the present funding level, PCDC participants often do not receive appropriate levels of care. The PCDC has been operational for 11 years, but funding is limited. PCDC targets defendants with high need for substance use disorder treatment who are at high risk of criminal recidivism. In Kentucky Drug Courts (KDC), emphasis is given to the Ten Key Components and Best Practice Standards. Goals/objectives include: 1) enhancing services to develop an effective, comprehensive system of care to help individuals with substance abuse problems involved within the criminal justice system; 2) enhancing and strengthening treatment services to increase abstinence and decrease recidivism; and 3) ensuring project implementation/evaluation adhere to objectives and facilitate continuous quality improvement. Project objectives include: utilizing Assertive Community Treatment (ACT) and increasing staffing support to Kentucky River. PCDC will provide access to MAT. BJA funding for this project will allow for the purchase of services identified by the ACT team as critical to meet participant's treatment goals, but are not currently available including both mental/physical health services (i.e., HIV testing/treatment, smoking cessation), including services located in the region but not operated by Kentucky River. The project will serve 48 persons annually.

The McCracken County Drug Court (MCDC) Service Enhancement Project seeks to enhance substance use treatment and mental/physical health services offered within the existing adult Drug Court. With too few residential beds and intensive outpatient treatment slots, only an estimated 11% of those in need receive treatment. McCracken County is located in western Kentucky, a primarily rural and agricultural area that has increasing substance abuse and criminal justice problems with few available resources. At the present funding level, MCDC participants often do not receive appropriate levels of care. The MCDC has been operational for 11 years, but funding is limited. MCDC targets defendants with high need for substance use disorder treatment who are at high risk of criminal recidivism. The Bernalillo County Metropolitan Court Enhancement project seeks to enhance services for DWI/Recovery Court, Urban Native American Healing to Wellness Court (HWC) and the Community Veterans Court (CVC) clients. The DWI/RC and HWC target high-risk/ high need, non-violent offenders who have at least two, but not more than five, DWI convictions. The DWI/RC offers special tracks for those with co-occurring substance use disorders and mental illness, as well as Spanish speakers. The HWC targets self-identified, Native American adult residents and the CVC targets US military veterans, members of the National Guards, Reservists and active duty member suffering from substance abuse, co-occurring disorders, diagnosable and treatable behavioral health issues and charged with a misdemeanor including DWI charges. The proposed enhancements to the DWI/RC, the HWC and the CVC are designed to: 1) reduce the wait list for psychiatric services; 2) improve service coordination through expanded case management and mentor services; 3) expand drug testing as a recovery support tool; 4) expand program assessment for quality assurance; and 5) provide medication-assisted treatment services. The overarching goal of the project is to enhance the treatment and recovery services available to clients enrolled in the Bernalillo County Metropolitan DWI/RC, HWC and the CVC so they are evidence-based and culturally appropriate. To reach this goal, client-level objectives are to streamline case management and mentor services, enhance the current drug testing system to better serve individuals, provide immediate access to medication-assisted treatment and expand access to psychiatric services. Two hundred and twenty (220) clients will be served each year.

The Harnett County Veterans Treatment Court will enhance the adult drug court to assist nonviolent offenders with successful rehabilitation from the use of drugs and/or alcohol. The population to be served is Veteran. Forty-five clients will be served with SAMHSA funding annually and forty-five clients will be served with BJA funding annually. The proposed interventions are motivational interviewing and cognitive behavioral therapy. The goals of this enhancement are to: 1) provide crisis response and crisis stabilization services to all requesting Veterans. This goal will be accomplished through seven best practice standards related to the target population, historically disadvantaged groups, roles and responsibility of the judge, incentives , sanctions, and therapeutic adjustments, substance abuse treatment, multidisciplinary team, and census and caseload; 2) provide mandated substance use disorders (SUDs) outpatient treatment and/or evidence-based psychotherapy and corresponding psychiatric medication management. This goal will be accomplished through three objectives and four best practice standards related to substance abuse treatment, complementary treatment and social services, drug and alcohol testing, and monitoring and evaluation; 3) provide peer support services to each participating Veteran. This goal will be accomplished through the best practice standard related to complementary treatment and social services; 4) offer evidence-based skill-building practices. This goal will be accomplished through the best practice standard related to complementary treatment and social services; 5) decrease stressors related to daily living (e.g., addiction, financial management, housing, education, employment). This goal will be accomplished through two objectives and the best practice standard related to complementary treatment and social services. All 10 NADCP Adult Drug Court Best Practice Standards will be addressed.

The Fayette County Drug Court (FCDC) will assist adult nonviolent offenders with successful rehabilitation from the use of drugs and/or alcohol. In partnership with community agencies, this project will expand residential services and enhance wraparound services. This project will use evidence-based strategies and interventions. Expanded services will provide additional gender-specific residential treatment and recovery at two premier programs - the Hope Center, Inc. and Chrysalis House, Inc. Hope Center will expand residential services for an additional 23 FCDC participants annually, providing gender-specific, evidence-based residential recovery services. Chrysalis House, a licensed provider of substance abuse treatment and services for women and their families, will expand its evidence-based services for 20 FCDC participants annually. The total to be served with expanded residential services for this project is 43 each year. FCDC will also offer enhanced services, including employment services, relationship and communication skills training, dental services, and others, to 100 drug court participants annually. Most Fayette County Drug Court clients (nearly two-thirds) are male, and over 1/3 are women; most are ages of 25-44; and about 42% are Black, and 97% are non-Hispanic. The target population will mirror the arrestee population, including both felony and misdemeanor high risk/high needs offenders. To enter the program, clients must meet legal screening criteria, clinical screening criteria based on the Addiction Severity Index (ASI) and the risk and needs assessment. Fayette County Drug Court has access to MAT through community providers; and individuals who are receiving MAT are eligible for the FCDC.

The Polk County Drug Court in Polk County, Florida is an enhancement that will address the acute needs of high risk/high need adult clients diagnosed with a substance abuse disorder and assessed as requiring immediate treatment services, specifically residential treatment and/or medication assisted treatment. Drug Court participants in Polk County face a multiplicity of barriers to access, including poverty, limited transportation, lack of social support and the impact of trauma. The proposed project aims to address barriers to access with an enhanced continuum of services, including recovery support services and transportation assistance. Fifty individuals per year will participate in the project. To enhance access to treatment, the project aims to: a) provide transportation assistance, particularly to those living in the rural regions of the county; b) provide increased case management and recovery support services to help participants dismantle life's stressors; c) provide funding for residential treatment services; d) provide access to opiate and alcohol replacement therapy; e) enhance case monitoring and coordination by the Drug Court treatment team; and f) increase training and implementation of trauma-informed care strategies. The enhancements will incorporate evidence-based principles demonstrated in the NADCP Adult Drug Court Best Practice Standards. The following outcomes are anticipated as a result of the program enhancements: a) participant success/graduation from the Drug Court will increase 12% to 15%; b) participant relapse rates while in the program will decrease from current levels; c) participant recidivism for drug/alcohol related offenses will decrease from current levels; and d) participant psycho-social functioning will improve as a result of participation in the program. It is anticipated these efforts will improve the quality of life for all of Polk County's residents as well as improve the lives of Polk County's most vulnerable citizens.

The Las Vegas Municipal Court will enhance the adult drug courts by promoting participants' recovery from substance abuse and mental health disorders which contribute to their repeat criminal behavior. The Municipal Court operates 6 specialty court dockets, each serving a different subpopulation. They include: 1) Women in Need of Change (WIN) Court, which serves women who have a history of repeated arrests related to drugs and prostitution; 2) Habitual Offenders Prevention and Education (HOPE) Court, which serves habitual, typically homeless offenders; 3) Youthful Offender (YO) Court; which serves offenders between the ages of 18 and 24; 4) Driving Under the Influence (DUI) Court, which targets repeat DUI offenders or first time offenders with high blood alcohol levels at the time of arrest; 5) Veterans Court, which serves Veterans of all ages diagnosed with substance use and/or co-occurring disorders, and connects them with Veterans health and other benefits; and 6) Mental Health Court, which serves high-risk, high-need individuals with co-occurring substance abuse and mental health disorders. Participation in a Specialty Court program results in access to long-term drug treatment and counseling services, to the extent clinically indicated for each person's diagnosis. It provides access to recovery support services that address a wide range of criminogenic needs, as well as aftercare and recovery planning. An estimated 40 additional individuals will be served annually by SAMHSA funding, and 20 with BJA funds. Objectives include: 1) retention - 50% of participants will successfully complete court ordered requirements for graduation; 2) reduction in substance use - participants will demonstrate a 70% reduction in substance use; 3) recidivism - 75% of participants will not re-offend within 3 years of program graduation. This will be accomplished by adhering to all of the NADCP Best Practice Standards. MAT is available for participants.

The Denver Adult Drug Court (DADC) will collaborate with probation and treatment partners on the RESTART Expansion Project to increase the number of offenders that are served on the Felony DUI track of the drug court program. Colorado enacted a new law on August 5, 2015, that makes an individual's fourth or subsequent DUI or DWAI arrest a felony offense. This project will allow for a total of 60 offenders to be served over the life of the grant. The Denver District Attorney's Office screens offenders arrested on felony DUI charges for entry into this hybrid track. An offender that is screened and deemed eligible for the program moves rapidly through the plea and sentencing process to allow for early treatment intervention. A risk/needs assessment is completed prior to sentencing and an individualized treatment plan is quickly developed. This high-risk high-need population will benefit from evidence-based intensive outpatient treatment and supervision including additional services designed to reduce substance abuse and discourage intoxicated driving. The goals of the enhancement are to 1) build a sustainable collaborative infrastructure to ensure cross-system coordination among stakeholders; 2) expand the capacity of the RESTART track; and 3) improve outcomes for program participants, 80% of participants will successfully complete program requirements and graduate, 100% of participants will show a reduction in alcohol and drug use after 6 months, and 80% of participants who complete the RESTART track of the DADC will not reoffend with a new alcohol related driving offense. RESTART strives to comply with all of the NADCP Adult Drug Court Best Practice Standards. The treatment partner for this grant project, the University of Colorado Addiction Research and Treatment Services, offers medication assisted treatment (MATs) and MATs are encouraged by the RESTART team.

Miami-Dade County will support the Opioid Response Partnership (Partnership). This Partnership will act as a service enhancement for Miami-Dade's Adult Drug Court and its substance use disorder treatment partners to effectively address the gaps in services for an increasing number of nonviolent offenders using opioids/opiates, and to better ensure their successful stabilization and rehabilitation. The population served will be those defendants charged with possessing or purchasing opioids/opiates or other illegal drugs but who are determined to have an addiction to opioids/opiates. Eighty clients will be served with BJA funding each year of the grant. Of those 80, an average of fifty (50) clients will be served through the proposed Outpatient Detox Program with SAMHSA funding each year of the grant. The proposed interventions are: 1) adult drug court case management dedicated to the opioid/opiate population; 2) providing outpatient detoxification and medication-assisted treatment maintenance through Jackson Health System's Jackson Outpatient Detox Program; 3) providing residential and outpatient treatment, and other social services through South Florida Behavioral Health Network, and other community providers. The goals and objectives of the project are to: 1) expand case management services; and 2) improve access to opioid/opiate-focused substance use disorder treatment and services facilitating stabilization and rehabilitation by creating a streamlined system of referral and follow-up for detox and treatment between the Adult Drug Court, Jackson Outpatient Detox Program, South Florida Behavioral Health Network, and other community providers, providing outpatient detox and medication-assisted treatment through Jackson Outpatient Detox Program, and providing residential and outpatient treatment and referrals for social services through SFBHN, and other community providers. This will be accomplished through NADCP Adult Drug Court Best Practice Standards.
