Description
The purpose of this program is to pilot an approach to comprehensive healthcare for racial and ethnic medically underserved people experiencing unsheltered homelessness through the delivery of portable clinical care delivered outside that is focused on the integration of behavioral health and HIV treatment and prevention services. Recipients will be expected to take a syndemic approach to healthcare delivery through utilization of low barrier substance use disorder (SUD) treatment; mental healthcare; HIV and viral hepatitis testing and treatment; HIV prevention including condom, Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP) distribution; and harm reduction services.
Eligibility
Eligible applicants are States and Territories (American Samoa, Guam, Marshall Islands, Micronesia, Northern Mariana Islands, Palau, Puerto Rico, Virgin Islands), including the District of Columbia, political subdivisions of States (i.e. county, city, state), Indian tribes, or tribal organizations (as such terms are defined in section 5304 of title 25), community-based public and private non-profit entities, or programs operated by or in accordance with a contract or award with the Indian Health Service, or other public or private non-profit entities.

Proposed budgets cannot exceed $666,666 in total costs (direct and indirect) in any year of the proposed project. Annual continuation awards will depend on the availability of funds, grantee progress in meeting project goals and objectives, timely submission of required data and reports, and compliance with all terms and conditions of award.
