Title: FY 2023 Treatment, Recovery, and Workforce Support Grant

Mi Camino: Your Pathway to Career Success will be a bilingual/bicultural supported employment program integrated with behavioral health treatment that seeks to address the educational and vocational needs of Latinx individuals with substance use disorders or co- occurring mental health disorders in order to enter the workforce, maintain employment, and develop career pathways that support long-term success in recovery. Mi Camino integrates employment support services into each job seeker's personal care plan. Job seekers work with their care team to map out their skills, strengths, interests, and needs, and meet career goals. Mi Camino will serve 40 unduplicated clients in year 1 and 70/year for Years 2-5, for a total of 320 unduplicated clients over five years. Casa anticipates 100% of clients will have a substance use disorder and 70% will have a co- occurring mental health condition; 40% will be monolingual Spanish speakers and all will be below the national poverty line. The goals of this project are: 1. Complete vocational/educational assessment and service planning for Latinx with SUDs and CODs; 2. Create and deliver a culturally-focused Leadership Development Program that supports Latinx with CODS in becoming leaders and working in the Latino recovery community; 3. Increase access to educational and vocational opportunities through the establishment of Community Partnerships; 4. Improve access to integrated mental health and substance use treatment and primary care for Latinos with SUD and COD entering the workforce, and 5. Enhance organizational training plan to ensure the successful implementation of projects by reinforcing existing competencies and developing new skills. Mi Camino's success will be measured through the number of assessments completed, development of peer support professionals, vocational training services provided, training peer recovery professionals on culturally focused Leadership Development Program, and engagement in our Supported Employment services, such as individual vocational assessment and support, job search based on individual strengths, skills, and preferences, help with resumes, cover letters, and interview skills. Clinical staff will utilize evidence-based practices to administer SUD and mental health treatment, such as Integrated Dual Disorder Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, Medication-Assisted Treatment, Intensive Case Management, Supported Employment, and Assertive Outreach. Additionally, Mi Camino staff receives ongoing professional development training to reinforce existing competencies and develop new skills.

Through the SAMHSA Treatment, Recovery, and Workforce Support Grant, WestCare Tennessee (WCTN) will implement evidence-based programming to support individuals with substance use disorder (SUD) or co-occurring disorders (COD) to live independently and participate in the workforce, including individuals who are formerly incarcerated or justice-involved. PROJECT NAME: WCTN Next Chance Workforce Program Populations to be served: The proposed WestCare Tennessee (WCTN) Next Chance Workforce Program population of focus are adult (18+) male and female individuals in Jefferson and Cocke County Tennessee with substance use disorder (SUD) or co-occurring disorders (COD), including individuals who are formerly incarcerated or justice-involved, as well as those referred by local Drug Court judges, Rural Medical Services, local American Job Centers, and faith-based organizations and institutions. Strategies/Interventions: In partnership with the East Local Workforce Development Area Board, the East Tennessee Human Resource Agency (ETHRA), Jefferson and Cocke County Sheriff’s Offices, and Rural Medical Services, WCTN Next Chance Workforce Program activities will include vocational training, building relationships with employers, case management, and other employment and recovery support services that will support clients in attaining and successfully maintaining employment and their recovery. Project goals and measurable objectives, including the number of people to be served annually and throughout the lifetime of the project: Goal 1: The WCTN Next Chance Workforce Program will provide trauma informed evidence-based services to support individuals in Jefferson & Cocke Counties, TN in SUD or COD treatment and recovery, to live independently and participate in the workforce. Objective 1.1: Provide comprehensive recovery-oriented workforce support services to 210 adults by September 2028 (30 adults in years 1 and 5; 50 adults in years 2-4). Objective 1.2: By September 2028, 80% of the clients enrolled will successfully complete the program. Objective 1.3: By September 2028, 80% of the clients will receive case management, individual and group counseling, care coordination, MAT linkage and other services as needed as evidenced by the clinical record. Objective 1.4: By September 2028, 80% of program completers will be substance free at discharge, and 70% will remain substance free at 6-months post discharge as evidenced by the GPRA and urinalysis. Objective 1.5: By September 2028, 80% of program completers will have stable living arrangements at discharge, and 70% of those will maintain their living arrangements at 6-months post intake as evidenced by the GPRA. Objective 1.6: By September 2028, 80% of the clients completing services will have improved social connectedness at discharge, and 70% will maintain these improvements at 6-months post intake as evidenced by the GPRA. Objective 1.7: By September 2028, 80% of clients completing services having anxiety, depression, or trauma symptoms at admission will exhibit fewer symptoms at discharge, and 70% of those will maintain the improvements or show additional decreases at 6-months post admission assessed by the PCL-5 and GPRA. Goal 2: The WCTN Next Chance Workforce Program will identify impacts of SUD/COD on the workforce in Jefferson and Cocke Counties, TN and reduce SUD/COD stigma and expand collaboration between stakeholders to increase employment opportunities for the recovering population in the community. Objective 2.1: By September 2028, 90% of clients will receive prevocational training services directly linked to the employment opportunities in the Jefferson and Cocke Counties, TN. Objective 2.2: By September 2028, 80% of program completers will be employed at discharge, and 70% will remain employed at 6-months post discharge, as evidenced by the GPRA.

Summary. Via the project, CITC will implement 13 main activities, including case management; SUD treatment and recovery services; career planning services; linkages to COD treatment and recovery, SMI treatment, supported employment, and supportive services; CLAS standards trainings and implementation; therapeutic workplace implementations; and workforce investment board coordination. Population to Be Served. The project’s target population will be adults (18+ years) who have substance use disorders (SUDs) or co-occurring disorders (CODs) and are seeking gainful employment opportunities in the project’s catchment area. The project’s catchment area will be the Anchorage Metropolitan Statistical Area (MSA), which is composed of the Municipality of Anchorage consolidated city-borough and the Matanuska-Susitna borough. The Anchorage MSA’s estimated population is 399,352 residents, who represent over 50% of Alaska’s total population. Strategies. The project will facilitate evidence-based behavioral healthcare and workforce development services for the project’s target population. Via the project, CITC will guide at least 150 individuals to recovery, self-sufficiency, and gainful employment. Additionally, via the project, CITC will improve its capacity to provide comprehensive and high-quality behavioral healthcare and workforce development services. Goals and Objectives. Via the project, CITC will achieve 3 goals and 8 objectives. Abridged versions of those goals and objectives are provided below. Goal 1. Improve SUD and COD treatment and recovery outcomes for adults who have SUDs or CODs in the Anchorage MSA. Objective 1.1. Each year, provide weekly SUD counseling sessions to 30 unduplicated adults. Objective 1.2. Each year, provide linkages to COD treatment and recovery services to 20 unduplicated adults. Objective 1.3. Each year, provide linkages to supportive services to 30 unduplicated adults. Goal 2. Improve employment outcomes for adults who have SUDs or CODs in the Anchorage MSA. Objective 2.1. Each year, provide career planning services to 30 unduplicated adults. Objective 2.2. Each year, complete and have signed a memorandum of agreement for therapeutic workplace implementation and job opportunities. Objective 2.3. Each year, provide linkages to supported employment services to 10 unduplicated adults. Goal 3. Improve CITC’s capacity to provide workforce development services to adults who have SUDs or CODs in the Anchorage MSA. Objective 3.1. In the first year, complete and have signed a memorandum of agreement for short-term prevocational training services and for support for active workforce development projects in the Anchorage MSA. Objective 3.2. Each year, complete a SUD and COD workforce assessment. The Centers for Families and Children (d/b/a “The Centers”) will provide treatment, recovery, and workforce support services to Cuyahoga County residents with Substance Use Disorder (SUD) or Co-Occurring Disorder (COD), with a focus on populations that have historically had limited access to quality healthcare services and that have been most impacted by poverty and unemployment. These populations include African American, Latinx, those with limited English proficiency, and immigrant populations. As a collaborative effort between our SUD, Supported Employment, and El Barrio Workforce Development staff, the SUD & Employment Support Program team will provide SUD treatment and recovery services, workforce development, job training, and individual placement supported/supported employment to help clients obtain sustaining employment. The Centers, in collaboration with community stakeholders, proposes to serve 1,125 individuals over five years (125 in year one; 175 in year two; 225 in year three; 275 in year four; and 325 in year five). Through the SUD & Employment Support Program, our goal is to provide treatment and recovery services, job readiness skills, employment coaching, and supportive services to these individuals. Objectives to meet this goal include: • 70% of SUD and COD clients will be linked to the SUD & Employment Support Program within year one of the start of the grant. • At least 10 clients will be trained in El Barrio’s Peer Support track within year one of the start of the grant. • Ensure 50% of program clients show job retention 30 days from obtaining employment. • Obtain 260 job development contact opportunities within year one of the start of the grant. • 75% of program staff will be retained in the SUD & Employment Support Program within year one of the start of the grant.
