Project Description The Henrico Area Mental Health & Developmental Services (HAMHDS) will partner with The Daily Planet Healthcare Center to provide primary care services at two locations in Henrico County, Virginia. A Family Nurse Practitioner and Medical Assistant will provide care to clients, establishing a health home at HAMHDS. The focus of the project will be to engage clients, especially those experiencing Severe Mental Illness, in routine primary care. Approximately 500 individuals will be served each year. Project Description The purpose of the Primary and Behavioral Health Care Integration (PBHCI) project is to provide coordinated and integrated services through the co-location of primary and specialty care medical services in a community-based behavioral health setting. Integrated services to be provided through the PBHCI project include: comprehensive care management, care coordination, health promotion and wellness programming, comprehensive transitional care, individual and family support services, referral to community and social support services and ongoing assessment. The targeted population includes total of 1,461 unduplicated severe and persistent mental illness (SPMI) clients over the four-year grant period: 292 will be enrolled in the first year, 731 will be enrolled in second year, 1,169 will be enrolled in the third year, and 1,461 will be enrolled in the fourth year. The demographics of this population are expected to be: 40% African American, 56% White, and 4% Other. Women will make up 66% while 34% will be males. The goal is to improve the physical health status of adults with SPMI and those with co-occurring substance use disorders who have or are at risk for co-morbid primary care conditions and chronic diseases. Project Description The South Carolina Department of Mental Health (SCDMH) is proposing the following project, which will co-locate primary and specialty care medical services in three additional community mental health clinics of Catawba CMHC. The project will demonstrate measureable improvements in the physical health status of adults with serious mental illnesses and those with co-occurring substance use disorders who have or are at risk for co-morbid primary care conditions and chronic diseases. Catawba Community Mental Health Center (CCMHC) serves a three county area in northern South Carolina (SC), which includes York, Chester and Lancaster Counties. CCMHC currently serves a total of 2,437 patients (1,734 adults), and in FY 2014 served a total of 5,082 adult consumers (duplicated count). The targeted population for this project would be active adult clients (consumers) of Catawba CMHC. The objective of the proposed project is to improve the health status of adults with SMI and those with co-occurring substance use disorders, to enhance the consumer experience of care through improved access and quality, and to reduce the cost of care. The primary goals of the project are to initiate a screening program, to develop a wellness program, to provide primary care to 175 consumers in the first year and 720 clients in year 4, to reduce ER use by 10% in year 1 and by 25% in year 4; and to provide enhanced case management to these consumers. These goals will be accomplished through a partnership with Community Medicine Foundation (CMF). CMF is a private, non-profit, Federally Qualified Health Center (FQHC) founded in 1992. Finally, our proposal leverages years of work building primary and behavioral health partnerships in Fairfax County. Project Description Juneau Primary and Behavioral Health Care Integration Project (JPBHCI). Project Description Milwaukee Center for Independence (MCFI) will implement the Milwaukee Whole Health Project (MWHP), which will provide coordinated and integrated primary and behavioral health care services to 407 unduplicated Milwaukee County residents with Serious Mental Illness (SMI) over the four-year grant period. The project will be provided in the context of MCFI's Whole Health Therapies (WHT) PBHCI clinic, which opened in January 2014. MWHP will be delivered by MCFI in close cooperation with the Milwaukee County Behavioral Health Division (BHD) Community Access to Recovery Services (CARS). Through CARS contracts and Medicaid reimbursements, MCFI will leverage an additional $6,288,000 in non-grant funds over four years to implement the project. Those who have the most complex and high-risk set of medical and behavioral health conditions will receive the highest level of care management, to be delivered by an RN.
