Project Description The Homeless 2 Home Behavioral Health Project for Pennsylvania (H2H-PA)- Enhancement will assist one hundred individuals who are experiencing homelessness and have substance use disorders, seriously mental illness (SMI) or co-occurring mental health and substance use disorders in Bucks and Delaware Counties, using evidence-based practices through an interdisciplinary team that has proven its housing support model's efficiency and effectiveness under CABHI State FFFY13. H2H-PA-Enhanacement will serve two populations: 1) Individuals who experience chronic homelessness and have substance use disorders, SMI, or co-occurring mental health and substance use disorders; and/or 2) Veterans who experience homelessness/chronic homelessness and have substance use disorders, SMI, or co-occurring mental health and substance use disorders. Critical Time Intervention (CTI), SSI/SSDI Outreach, Access and Recovery Project (SOAR), and Peer Support are the evidence-based strategies that will be deployed to assist CABHI Enhancement program participants. The goals and measurable objectives of the Enhancement project include 1.) building upon statewide strategies to address planning and coordination of behavioral health and primary care services along with permanent housing to reduce homelessness; 2.) increasing the number of individuals residing in permanent housing who receive behavioral health treatment and recovery support services; and 3.) increasing the number of individuals places in permanent housing and enrolled in medicaid and other mainstream benefits.... Project Description The Arizona Department of Health Services/ Division of Behavioral Health Services (ADHS/DBHS) vision for the Cooperative Agreement to Benefit Homeless Individuals Enhancement (CABHI-States-Enhancement) project is to continue to improve health outcomes for individuals with substance use or co-occurring substance abuse and mental health disorders who experience chronic homelessness. The enhanced project scope will expand the catchment geographic service area statewide to include such counties as Maricopa, Pima, La Paz and Mohave. The enhanced project scope will include the continuation of supportive housing and a comprehensive array of evidence based services to serve a total of 35 people in the expanded service area while also improving statewide infrastructure and capacity to provide accessible, effective, comprehensive, coordinated/integrated, strength-based, person-centered, family-focused, culturally sensitive, and clinically sound and supervised behavioral health services through local, community-based behavioral health service providers based on the location and choice of the individual. Project Description The CABHI-States-Supplemental award to Illinois in FFY 2015 supports an expansion and enhancement of treatment services for chronically homeless persons with diagnosed MHDs in the absence of SUDs. The proposed SAMHSA CABHI-States-Enhancement funds will be used to support an additional expansion and enhancement of treatment services for chronically homeless persons with diagnosed SUDs or co-occurring SUDs and MHDs. Through the proposed award, an additional unduplicated count of 300 homeless persons with SUDs will be admitted to these expanded and enhanced treatment services during FFY 2016. These services will be provided by three Illinois Department of Human Services, Division of Alcoholism and Substance Abuse IDHS/DASA-licensed treatment providers, an Employment Specialist who will provide needed assistance to participating homeless persons. The infrastructure developments of the Illinois CABHI-States cooperative agreement began in FFY 2014 with the formulation of a Statewide Interagency Council, and the development of a statewide strategic plan that will be used to guide and monitor the Council's activities. These additional infrastructure development activities will be supported through a training and technical assistance contract with the Great Lakes Addiction Technology and Transfer Center (ATTC). HHC will (1) Establish a Public Health System Impact Coordination Team for its eight-county service area; (2) Increase the agency's level of integration from a SAMHSA Level 4 (basic on-site collaboration) to a Level 6 (fully transformed integrated practice); (3) Achieve all EHR Meaningful Use Standards across all five mental health clinics; (4) Incorporate a behavioral health home model through the creation of integrated care teams at all community mental health center sites; (5) Institute on-site primary care services at 60% of the community-mental health center locations, and (6) Provide behavioral health home services to 75% of the clients diagnosed with a severe-persistent mental illness. The project will result in (1) 6,000 clients having access to fully integrated care and a unified electronic health record; (2) 3,600 gaining access to on-site primary care; and (3) 1,000 clients diagnosed with a severe persistent metal illness receiving behavioral health home services. HHC is the major behavioral health and primary care safety net organization for eight counties in Southeast Ohio (Athens, Hocking, Jackson, Meigs, Perry, Ross, Vinton, and Washington).
