Project Description The Native Family Wellness Partnership represents an unprecedented partnership to focus on a major gap in the provision of culturally competent and comprehensive mental health services and system of care (SOC) for American Indian and Alaska Native (AIAN) children, ages birth to 21, and their families in the San Joaquin Valley, California. The Tule River Indian Tribe, a federally recognized tribal government and reservation established in 1873, and the Fresno American Indian Health Project (FAIHP), an American Indian nonprofit organization that is recognized by the Indian Health Service (IHS) as an Urban Indian Health Program under Title V of the Indian Health Care Improvement Act, PL 94-437 will work together to build an integrated, coordinated, family-driven, youth-guided system of care that spans the urban areas and tribal territories in the surrounding communities of the Valley. This project is expected to serve at least 30 families during the first year of the project as services ramp up and a solid foundation is set. The goal is to increase service enrollment by at least 10% each year as the project is brought to scale. The goals and objectives of the project address the following key areas of focus: (1) provide permanent supportive housing; (2) maintain housing long term; (3) enroll participants in mainstream benefits programs that include Medicaid, SSI,SSDI, TANF, SNAP, and veteran benefits; (4) facilitate behavioral health recovery (substance use and mental health disorders); (5) promote physical wellness and (6) foster collaboration within state and local government. MH4R will provide comprehensive integrative and person centered services by practicing Housing First, which values housing as the primary need that must be addressed first. Outreach, engagement and rapid re-housing services will be offered to qualified individuals, along with the leveraging of mainstream benefits and programs that include Medicaid, SSIISSDI Outreach, Access, and Recovery (SOAR), and the Projects for Assistance in Transition from Homelessness (PATH) program.
