Project Description The Recovery-Oriented Data Action Network (Project "RODAN") seeks to (1) develop improved engagement and person-centered planning practices that involve data collection using a recovery oriented tool (the Mental Health and Recovery Measure), and (2) build a consumer-directed and operated personal Electronic Health Record System to capture data to inform all aspects of our target population, recovery services and associated outcomes. Through Project RODAN, the Recovery Institute of Southwest Michigan will serve as a catalyst for organizational/structural change and create a locus of responsibility for the need for development of quality engagement, person-centered planning, and information management systems that can meet the service needs of an ever growing target population in the face of Medicaid expansion. The project will serve an estimated 80 persons who are seeking services monthly who are over the age of 18 residing in Kalamazoo County, Michigan, or neighboring counties and who have self-disclosed as having a serious and persistent mental illness. Activities will include practice reform via development of an improved peer-driven engagement and person-centered planning process; and internal process for reviewing goal-service-outcomes matches at the individual and organizational level; an EHR build that accurately captures demographics, services and outcomes for individuals; measurement of recovery outcomes for individuals receiving services over time; and development and implementation of protocols for decision-making regarding enhancements or changes for the peer-run, recover-oriented service array in the face of Medicaid expansion, as well as the development and wide-spread dissemination of toolkits, guidebooks, and curricula, and translation into Spanish, to members of the peer-run network in Michigan.... Project Description Healthy Mind, Healthy Body: A Peer Approach is designed to provide a conceptual connection between mental and physical wellness for consumers and providers of mental health services in Montana, to educate the general public on that connection, and to integrate peer support into the mental health system as part of making those connections. This project will serve veterans, youth, American Indians, and other mental health consumers in Montana. We are not concentrating on any clinical characteristics because this project, and all our work, is inclusive of all people with mental illness, diagnosed or self-diagnosed, and most of our work, including that for this project, takes place outside of a clinical setting. In Year One, MPN will concentrate our efforts in two rural communities (5,000 population and fewer), and in Bozeman, Helena, and Great Falls, expanding as the project moves forward. MPN will develop and conduct participatory educational workshops with identified populations in targeted communities as mentioned above. The goal of these workshops is to get participants to see the connections between physical and mental issues and wellness, and then empower them to work towards physical and mental health and recovery. Often, even health professionals are not aware of the mental health ramifications and effects of various illnesses or treatments, or of the physical effects of some mental health diagnoses. Consumers likewise may not know how their mental or physical health issues affect the other. This project is to change how people in Montana see mental health services and to integrate peer support into the mental health system.
