The Colorado Department of Education (CDE) has identified three local education agencies with unique contexts and populations to partner with in building a locally driven system for the state: Aurora Public Schools (APS), Fountain-Fort Carson School District (FFCSD), and Thompson School District. The AWARE goals include: 1) building state infrastructure and capacity through partnerships, policy, shared data coordinating resources and intensive coaching for data-driven decision making; 2) building LEA infrastructure and capacity through partnerships, policy, shared data systems, coordinating resources and intensive systems coaching for data-driven decision making; 3) utilizing implementation science to deliver research-based programs and strategies to build students' social, emotional and behavioral wellbeing while reducing behavioral health risks; 4) increasing awareness of behavioral health issues, reducing the stigma and increasing behavioral health promotion; and 5) securing sustainable funding mechanisms for comprehensive school behavioral health services.

The three collaborating Florida AWARE school districts serve more than 300,000 students from very diverse backgrounds. Short Title: NITT-Healthy Transitions (NITT-HT) The project will serve three populations of focus: 1) 16-25 year olds at risk of developing a serious mental health condition; 2) 16-25 year olds who have already been identified as having a serious mental health condition; and 3) the community-at-large. The goals are to: 1) make policy and funding changes at the state and local level to improve cross-system collaboration, service capacity, and workforce expertise; 2) create, implement, and expand research- supported services and supports that are culturally competent and youth-guided; 3) provide for continuity of care between child and adult behavioral health systems; and 4) involve family and community members in this process. The Florida Youth Transition to Adulthood project will be implemented through state and local partnership. DCF will contract with Central Florida Behavioral Health Network (CFBHN), a full service Managing Entity (ME) that manages State child and adult behavioral health treatment, prevention, and social service contracts in west central Florida, to manage grant activities. CFBHN will pilot evidence-based services for this population in Hillsborough, Lee, and Collier counties and collaborate with DCF and state and local stakeholders to address policy and procedural issues relating to the transition of young people with serious mental health conditions to adulthood.

The three divisions are Montgomery, Pulaski, and Fairfax County Public Schools. Mental Health First Aid and Youth Mental Health First Aid will be provided to at least 750 people. There are 18,856 students at the pilot schools. There are three goals: 1. Develop a multi-tiered model that integrates a comprehensive systems approach for addressing the mental health needs of children and youth that can be piloted with the three selected school divisions. 2.
