Short Title: NITT-Healthy Transitions (NITT-HT) The project will serve three populations of focus: 1) 16-25 year olds at risk of developing a serious mental health condition; 2) 16-25 year olds who have already been identified as having a serious mental health condition; and 3) the community-at-large. The goals are to: 1) make policy and funding changes at the state and local level to improve cross-system collaboration, service capacity, and workforce expertise; 2) create, implement, and expand research- supported services and supports that are culturally competent and youth-guided; 3) provide for continuity of care between child and adult behavioral health systems; and 4) involve family and community members in this process. The Florida Youth Transition to Adulthood project will be implemented through state and local partnership. DCF will contract with Central Florida Behavioral Health Network (CFBHN), a full service Managing Entity (ME) that manages State child and adult behavioral health treatment, prevention, and social service contracts in west central Florida, to manage grant activities. CFBHN will pilot evidence-based services for this population in Hillsborough, Lee, and Collier counties and collaborate with DCF and state and local stakeholders to address policy and procedural issues relating to the transition of young people with serious mental health conditions to adulthood.

The primary goal of this project is to reduce the incidence of suicidal behavior and alcohol/substance abuse among the Inupiaq youth of the region. To do this, the project will focus on coordinating the region's existing initiatives in youth wellness, suicide prevention, and substance and alcohol abuse while creating protocols to ensure that youth do not fall the gaps of services. Anticipated new interventions would include greater cross-agency and inter-agency referrals and information sharing; public awareness campaigns aimed at reducing the normality' of suicide and alcohol/substance abuse; community conversations on addressing the problem; cultural competence improvement for professionals and more training for community para-professionals to improve accessibility of services; and reduction of access to lethal means. Measureable objectives include: 1. Completion of two community readiness assessments. 2. Creation and implementation of 'high-risk' protocol that coordinates services and closes gaps. 3. Creation and implementation of a postvention protocol that coordinates services and closes gaps. 4. Completion of trainings for para-professionals and professionals. Because of the role of outreach and wellness activities we estimate that the number of young people to be served annually will be 700-1000 region wide.

Short Title: Native Connections With SAMHSA funding, the Native Village of Selawik will form the Selawik Wellness Coalition which will be made up of four initial task forces: Wellness Promotion; Suicide Prevention; Crisis Intervention; and the Postvention. The goals of the Selawik Wellness Coalition will be fourfold: 1) For Selawik residents to take ownership of the problem and of the solution through Wellness Promotion; 2) To educate residents on how to effectively and appropriately respond to people at risk of suicide; 3) To connect with other organizations and villages to create a region-wide alliance focused on increasing the quality of life in rural Alaska; and 4) To provide Selawik residents with immediate access to prevention, treatment, and recovery services when needed. The overall indicators of success will be a reduction in both the rate of suicide and suicide attempts in Selawik, Alaska. Short Title: Native Connections With SAMHSA funding, the Native Village of Selawik will form the Selawik Wellness Coalition which will be made up of four initial task forces: Wellness Promotion; Suicide Prevention; Crisis Intervention; and the Postvention. The goals of the Selawik Wellness Coalition will be fourfold: 1) For Selawik residents to take ownership of the problem and of the solution through Wellness Promotion; 2) To educate residents on how to effectively and appropriately respond to people at risk of suicide; 3) To connect with other organizations and villages to create a region-wide alliance focused on increasing the quality of life in rural Alaska; and 4) To provide Selawik residents with immediate access to prevention, treatment, and recovery services when needed. The overall indicators of success will be a reduction in both the rate of suicide and suicide attempts in Selawik, Alaska.
