A Peer Wellness Coach will assist participants and family members with navigation and facilitate access to services. SLO-HIP will provide medical services to at least 280 of its 2800 clients with serious mental illness in its first year, with increased goals 700, 1120 and 1400 clients in Years 2, 3 and 4, respectively. Project goals are to: 1) Improve integration of physical and behavioral health care by providing medical services on-site to SLOBHD patients through a partnership with CHC. 2) Reduce chronic disease among SLOBHD's adult, severely mentally ill patients through early detection, intervention, and prevention education. 3) Improve access to a full range of integrated health services by employing a peer wellness coach, with lived experience, to support SLO-HIP patients. 4) Enhance information sharing between providers through electronic health information exchange between diverse electronic health record systems. 5) Evaluate cost benefit of integrated physical and behavioral health care through data collection and performance management. 6) Develop a sustainable system of integration of services by identifying funding mechanisms and responsibilities. Project Description Community Network Services (CNS), through CNS' PBHCI project provides person- centered BHH services to persons with SMI, diagnosed with or at-risk for a serious medical condition. CNS co-locates and partners with PCPs to develop the processes, protocols, infrastructure, and capacity to integrate and coordinate behavioral health and primary and specialty medical services, within four locations, in Oakland County, MI. Adults 18-65 years old men and women who have SMI and/or co-occurring SUD, enrolled with OCCMHA as a CNS consumer, and have at least one general medical condition, including, but not limited to, Diabetes, COPD, Emphysema, Obesity, Hypertension, or Tobacco Use Disorder. The goal of the CNS PBHCI project is to improve the physical health status of CNS adult consumers who have or are at risk for co-morbid primary care conditions and chronic diseases. The objectives of the program are to (1) improve the health of CNS consumers with SMI and co-occurring SUD; (2) enhance the consumer experience of care, including quality, access, and reliability; and (3) reduce and/or control the per capita cost of care. Project Description Proyecto Integracion de Salud/Health Integration Project provides coordinated and integrated services through the co-location of primary and behavioral health at Gandara's outpatient center in Springfield, MA. The population of focus is adults living with serious mental illness or co-occurring substance use disorders that are enrolled in behavioral health care as provided by Gandara Center (hereinafter, Gandara), with an emphasis on low-income Hispanic adults with chronic health conditions in Springfield, Massachusetts. The project proposes to develop increased infrastructure and capacity to fulfill primary and preventive health care needs of Gandara's clients with SMI or co-occurring disorders and to provide co-located, integrated primary and behavioral health services, to individuals with serious mental illness. The project strives to provide a participant-driven treatment and peer-driven peer wellness program that engages and supports the four dimensions of recovery. The project will target adults primarily from racial, ethnic, and sexual minority communities who 1) have serious mental illness and co-occurring BH diagnoses; 2) are at high risk of or diagnosed with chronic diseases including diabetes, hypertension, dyslipidemia, HIV/AIDS, Hepatitis A, B & C; 3) have diagnosed and undiagnosed trauma histories; and 4) have no primary care physician or do not seek routine primary care. Project Description LifeWays Community Mental Health and the Center for Family Health, a Federally Qualified Health Center (FQHC) and other primary care providers, will partner to increase the life expectancy and improve the overall health of individuals living with multiple chronic physical and mental health conditions. This collaboration, known as the LifeWays Health integration Project, will effectively and efficiently integrate the health care of these individuals through the delivery of comprehensive care management, care coordination, health promotion, comprehensive transitional care from inpatient to other settings, individual and family support, and referral to community and social support services. This integrated approach will also include coordinated, accessible, and seamless access to primary care services within the behavioral health care setting and tailored to best suit the needs of the individual. Project Description LifeWays Community Mental Health and the Center for Family Health, a Federally Qualified Health Center (FQHC) and other primary care providers, will partner to increase the life expectancy and improve the overall health of individuals living with multiple chronic physical and mental health conditions. This collaboration, known as the LifeWays Health integration Project, will effectively and efficiently integrate the health care of these individuals through the delivery of comprehensive care management, care coordination, health promotion, comprehensive transitional care from inpatient to other settings, individual and family support, and referral to community and social support services. Project Description Link2Health Solutions (L2HS), in partnership with the National Association of State Mental Health Program Directors (NASMHPD), the National Council for Behavioral Health (NCBH), Living Works (LW), the National Association of Crisis Organization Directors (NASCOD) and CONTACT USA (CUSA), will manage the National Suicide Prevention Lifeline (Lifeline) and the Disaster Distress Helpline (DDH), to further enhance and strengthen the Lifeline and DDH's capacity to effectively reach and serve the total potential number of persons at risk for suicide and/or disaster-related distress who might seek telephonic, chat or text-based (SMS) help within the U.S. L2HS will work with West, Inc.-the nation's top telephony systems vendor for connecting 911 calls--to maintain a system of toll-free suicide prevention lines that efficiently links callers to the nearest networked center for local assistance. L2HS will continue to facilitate 24/7 provision of chat (Lifeline) and SMS (DDH) technology platforms through SiteMax and iCarol (chat) and Mosio (SMS) systems. L2HS will continue to regularly provide data to SAMHSA and the network demonstrating connectivity and other user trend reports to monitor service activity and quality.
