The four objectives of the program are: 1) Reach level 6 in the SAMHSA/HRSA Center for integrated Health Solutions' Standard Framework for Levels of Integrated Healthcare; 2) improve the health of individuals and reduce health disparities of population with SMI; 3) Enhance the consumer experience of care and 4) Reduce/control the per capita cost of care for the individual, as measured by annual cost of care, before and during their participation in the program. The program will be created within the Niagara County Adult Mental Health Clinics (NCDMH) through the expansion of our primary care services, the development of health and wellness classes, and the expansion of our partnership with Northpointe Council, an agency that has been providing a full range of substance abuse treatment services in Niagara County for over 45 years. The program is designed to provide services to SMI individuals in the County who are most at risk for heart disease, strokes, diabetes, and early death. Services will initially focus on individuals who are currently enrolled in treatment with the agency partners who do not currently have an active relationship with a primary care provider. Criteria will expand as program capacity expands. In Year 1, 104 individuals will be served; Year 2 260, Year 3 416, and in Year 4 520. Based on our current population, an approximately equal number of males and females will receive program services. Care Coordination and Integrated Behavioral Health and Physical Health services are the cornerstone of this project. Therapists will utilize evidence based strategies, including CBT and motivational interviewing and the Wellness Center's offerings, including smoking cessation, diabetes education, chronic disease self-management, have all been designed to be effective for people with SMI. Peer support to facilitate recovery is an integral component of the program.

Short Title: PBHCI The ACCESS Health and Research Center currently includes a co-located community mental health clinic and primary care medical clinic in Dearborn, an integrated satellite clinic (mental health and primary care) in Sterling Heights, which targets resettled refugees, and a specialty treatment program for victims of crime and torture survivors located in Dearborn above the clinic. All clinics and programs serve a significant number of clients with serious mental illness. Southeast Michigan is the largest resettlement site in the US for Iraqi refugees, having resettled more than 12,000 Iraqi refugees from the 2003 War in Iraq during the past five years. Southeast Michigan is also considered the largest placement site for the anticipated resettlement of Syrian refugees in late FY2015 and FY2016. Mental illness prevalence among Iraqi refugees is very high. The purpose of ACCESS's proposed PBHCI project is to develop coordinated and integrated service for co-located delivery of primary, specialty, and mental health care in a behavioral health setting. The project goal is to improve the physical health status of the population of focus with serious mental illness, including those with co-occurring substance use disorders, who have or are at risk for comorbid primary care conditions and chronic disease. The project objectives are to (1) improve the health of the population of focus, (2) enhance the quality, access, and reliability of care, and (3) reduce and control the per capita cost of care. The proposed project's population of focus will include clients who are already active in the clinics mentioned above. This population of focus currently totals 895 individuals with SMI. Short Title: PBHCI SMA Behavioral Healthcare, Inc. (SMA) will implement a Primary and Behavioral Health Care Integration (PBHCI) Health Home to serve adults with a serious mental illness (SMI) and those with co-occurring substance use disorders who are currently receiving outpatient psychiatric care at SMA's Daytona Beach Outpatient Clinic in Volusia County, Florida. The Home Health Team will include a Project Director, Nurse Care Manager, Primary Care Physician, Primary Care ARNP, Integrated Care Managers, a Co-Occurring Substance Abuse Counselor, and a Peer Wellness Coach, with the Psychiatrist available at all times for consultation. SMA will prioritize enrolling those who currently do not have access to primary care and those who are uninsured. SMA will assist participants in securing Medicaid/Medicare, SSI/SSDI, private insurance and other benefits for which they may be eligible. The project will be implemented using the Tennessee Cherokee Health System (CHS) model as the foundation for SMA's integrated Health Home and will be guided by SAMHSA/HRSA Center for Integrated Health Solutions' Standard Framework for Levels of Integrated Healthcare. Multiple evidence based prevention, health promotion and treatment interventions will be employed by the Health Home. SMA will co-locate the Health Home onsite at the Daytona Beach Outpatient Clinic (Level 3) within four months of award with the goal of full collaboration in a transformed/merged integrated practice (Level 6) by the fourth year of the grant. SMA will serve a minimum of 10% of its census of 800 eligible adults with SMI during the first year (100), 25% during year 2 (200), 40% during year 3 (320) and 50% during the final year (400). SMA's strategic plan includes a goal to expand the PBHCI Health Home model to all SMA Outpatient Psychiatric clinics in its four-county area.

Short Title: PBHCI This Project, Primary and Behavioral Health Care Integration at U.N.C. WakeBrook, will expand outpatient primary care services at WakeBrook (WB), a behavioral health facility located in Wake County, North Carolina, creating a fully integrated, patient-centered medical home. Consumers of the WB primary care office have a serious mental illness (SMI), many have or are at-risk for developing a chronic physical illness, and did not have a prior primary care provider. In addition, a smaller group of these consumers regularly utilize emergency departments (EDs) and inpatient care; these high utilizers have increased rates of substance abuse, and significant trauma exposure, and are also more likely to have co-morbid personality disorders. Wake County, North Carolina consists of 61% Caucasian, 19% Black or African American, 10% Hispanic or Latino, 51% females, with an average age of 34.5 years. The population at WB includes more African Americans, males, homeless, and under- and uninsured individuals, fewer Hispanic or Latino patients; and slightly older patients than Wake County as a whole. Our initial assessment of consumers at WB demonstrated major gaps in preventive care (e.g., colorectal screening, mammograms, and pap smears). WB consumers also exhibit a variety of health conditions and concerns, including hypertension, asthma/COPD, diabetes, current tobacco use, and BMI's in excess of 30 or 40.

Short Title: PBHCI Volunteers of America, Utah (VOA Utah) is a human services nonprofit organization located in Salt Lake City whose mission is to reach and uplift those in greatest need. VOA Utah provides outpatient mental health and substance abuse treatment, homeless services and transitional housing, counseling for survivors and perpetrators of domestic violence, primary care, child care, and other recovery supports. With this project Volunteers of America aims to improve the physical health status and overall wellness of adults with serious mental illness (SMI) living in Salt Lake County, Utah. This will be accomplished through a partnership with the Utah Department of Health's Health Clinics of Utah. Health Clinics of Utah currently has a primary care clinic onsite of VOA Utah's Cornerstone Counseling Center (the community mental health center). Basic primary care services are delivered to Cornerstone's clients one day per week. With PBHCI funding, the partners will expand the size, scope and impacts of this medical clinic, progressively increasing the level of integration of behavioral health and primary care services from a current Level 3 to a Level 6 within four years. Via a health home setting, program partners will provide holistic, evidence-based, culturally appropriate, integrated primary health care, mental health and/or substance abuse treatment, care coordination, integrated treatment planning, peer navigation support, prevention and health promotion activities and education, wraparound services, and comprehensive transitional care from inpatient to other settings, including appropriate follow-up, to ensure uninsured and underinsured adults with SMI gain access to care and enhanced opportunity for maximum health. The project will draw participants from a pool of approximately 1,200 clients with serious mental illness (SMI) currently served by RR, and will provide primary care and support services to 780 unique individuals over the four-year term of the grant (120 in year 1, 300 in year 2, 480 in year 3, 600 in year 4). It is anticipated that of the total 351 will be white, 421 will be black, and 8 will be from another race or multiple races. This population has an elevated risk of early death from co-morbid health problems including heart disease, overweight/obesity, diabetes, asthma, and tobacco use, with an average life span of 25 years less than those without SMI. RR will partner with the MetroHealth System, Northeast Ohio Neighborhood Health Services, Visiting Nurse Association of Ohio, Neighborhood Family Practice, and the Free Medical Clinic of Greater Cleveland. They will provide on-site, integrated primary care through a combination of prescriber-level providers (MD, NP) and nursing care, as well as dental care, and off site referrals when appropriate. This program has three main goals: 1) Improve the physical health status of adults with SMI and those with co-occurring substance use disorders who have or are at risk for co-morbid primary care conditions and chronic diseases through the delivery of on-site primary care; 2) Enhance the consumer experience of care; and 3) Reduce/Control the per capita cost of care.

This project will utilize evidence based practices and programs such as primary care service delivery, care coordination, Whole Action Health Management, Learning about Healthy Living and the Chronic Care Model, in addition to others. Project goals will be: To improve the physical health status of adults with serious mental illness and those with co-occurring substance use disorders who have or are at risk for primary health co morbidities and chronic diseases; Support the triple aim of improving the health of individuals with serious mental illness; Provide quality driven, cost effective, culturally appropriate and person and family centered integrated healthcare services; Coordinate and provide access to high quality healthcare services informed by evidence based clinical practice guidelines; Coordinate and provide access to preventative and health promotion services, including prevention of mental illness and substance use disorders; Coordinate and provide access to comprehensive care management, care coordination and transitional care across the healthcare spectrum; Provide increased access to evidence based chronic disease management; Develop a person centered care plan for each individual that integrates all aspects of their primary physical and behavioral healthcare needs; and Establish a continuous quality improvement plan to collect and report all required data and permit an evaluation of services in order to better facilitate the coordination of care.

The goal of this innovative project is to reduce early mortality from preventable chronic and infectious conditions among people with Serious Mental Illness (SMI), through the provision of primary and preventive healthcare within their trusted behavioral health home. Consistent with the goals of this grant, this initiative focuses on full integration of primary care and behavioral health, expanding on lessons learned during the integration of these services in Apalachee Center's Leon County clinic. The anticipated benefits of this project include increased access to care, improved health outcomes, lower healthcare costs, and an expansion of the use of peers within the integrated health workforce. Specifically Apalachee will fully integrate a primary care team into the behavioral health clinic to address the preventable and treatable chronic and infectious diseases that dramatically reduce life expectancy for the outpatient population of persons with SMI, and provide a full service health home for Apalachee Center's clients in Gadsden County. Apalachee estimates that among the target population, approximately 150 clients would receive integrated services during the first year, 300 during the second year, 400 during the third year, and 500 within the final year of the grant.
