Project Description Built on the foundation of ECCS, Spark NH was created to promote a comprehensive system of early childhood programs and services to realize its vision that all New Hampshire children and their families are healthy, learning, and thriving now and in the future. Using a life course approach, New Hampshire will address health risks early in the life span and at critically sensitive developmental periods when prevention, early intervention and health promotion can yield strong benefits. As the pilot community for Project LAUNCH NH, Manchester will meet goals through the expansion and enhancement of evidence-based services. The Manchester team proposes to increase screening and assessment; integrate behavioral health interventions into primary care practice; increase identification and treatment of behavioral health issues in child care and Head Start; improve social and emotional wellbeing of young children through enhanced home visiting services; and improve families' ability to support the multidimensional needs of their children through training and education.... Project Description Oklahoma's Project LAUNCH seeks to promote wellness of children 0-8 and enhance social emotional health by strengthening the established child-serving systems in Claremore, Oklahoma. This opportunity aligns goals, leverages resources, reduces duplication of effort and will allow sharing of lessons learned to create a model for replication in other areas of the state. Project LAUNCH aims to empower Oklahoma's families by building the competency of professionals knowledge and skills to strengthen integration across systems and reduce fragmentation of service coordination. Rogers County has experienced substantial population growth in recent years (23% between 2000 and 2010), while the overall population for the state increased only 8.7%. In addition, Rogers County has experienced a 70% increase in the rate of violent crimes committed by juveniles from 2007 to 2010. These increases coupled with higher rates of household violence and divorce indicate that young children in this county have a higher likelihood of exposure to risk factors which left unattended will lead to later social, emotional, cognitive physical and behavioral problems. The pilot community of Claremore, located within Rogers County, is a small rural community that has significant health disparities and access to care issues. The local community coalition identified vulnerable subpopulations experiencing exposure to risk factors. Project LAUNCH will target the approximately 3,975 children currently under the age of 8 residing in Claremore within two designated school districts, Head Start and child care programs. Of these children, 28% are Native American, 6% are Hispanic, 2.5% are African American and 62% are Caucasian. Oklahoma's Project LAUNCH will serve 1,699 children annually for a total of 8,495 children over the lifetime of the grant.... View More

Project Description As a systems improvement grant with a public health approach, Project LAUNCH presents a key opportunity at a critical time in Louisiana to help ensure that these systems are coordinated and aligned, and they are effective at the community level in supporting resilience and healthy development for young children and their families. Louisiana-LAUNCH will build upon existing innovative partnerships between the states health, welfare, and education agencies along with universities and community partners. The Department of Health and Hospitals (DHH) Offices of Public Health (OPH) and Behavioral Health (OBH) propose to advance a coordinated young child wellness system in the state overall, with a focus on the south central region of Louisiana Lafayette and the surrounding parishes. According to the Louisiana State University/Tulane Early Childhood Policy Data Centers report Early Childhood Risk in Louisiana, there are parishes that demonstrate high or moderate high risk for poor developmental or wellbeing outcomes based on indicators such infant mortality, percent of births to single mothers, teen birth rate, births to non-high school graduates, children living in poverty and children scoring in need of intensive intervention at kindergarten entry.... Project Description The Gun Lake, Nottawaseppi Huron and Pokagon Bands-all federally recognized Potawatomi tribes located in their ancestral homeland in southwest Michigan and/or north central Indiana--have formed a Consortium to promote the wellbeing of Potawatomi children from birth to 8 years living in its service area by addressing the physical, social, emotional, cognitive, behavioral and cultural aspects of their development using evidence-based practices. The Consortium's Project will build on existing tribal programs, especially those provided through the four Tribal Health Clinics and three Tribal Head Start Programs that already reach the targeted children and families. A variety of evidence-based practices will be implemented, including Incredible Years and Positive Indian Parenting.
