Project Description The California Department of Public Health, Maternal Child and Adolescent Health (MCAH) Division will partner with four county public health systems to expand the success of California Project LAUNCH. Strategies include: 1) adding mental health consultation to home visiting, 2) strengthening families and building parent leadership and engagement through Parent Cafes, and 3) cross-sector collaboration and systems integration efforts at the state and local levels. These successful strategies will be replicated in three pivotal counties that demonstrate both high need and readiness to implement both systems and client level changes: Butte, San Francisco and Fresno. The populations to be served include ethnically diverse families living in poverty with high rates of child maltreatment and lack of access of adequate mental health supports. The total number of families that will be served over the life of the grant are 1075; approximately 360 are expected to be served annually. As a result of this project, clients will experience improved mental health; parents will increase their parenting and leadership skills; home visitor programs will be able to better meet the mental health needs of their clients; and local and state systems will address barriers to integration and coordination so that families access to services is improved and more effective services are available. Project Description The Department of Community Health (DCH), in collaboration with the Michigan State Housing Development Authority (MSHDA), the Department of Human Services (DHS), the Michigan Interagency Council on Homelessness (Michigan ICH), and the Detroit community are requesting funding for enhancement to the CABHI grant originally awarded in 2013 to support the Michigan Housing and Recovery Initiative (MHRI). MHRI addresses the housing and support service needs of persons who are experiencing chronic homelessness with substance use or co-occurring substance use and mental health disorders in the City of Detroit. The enhancement funds will be used to support expanded activities under the grant that are designed specifically to enhance and improve the current program activities. These activities are: 1.) Providing additional housing access and navigation services by adding a team of Outreach Peer Support Specialists, Clinical Case Managers, nurses, and Crisis Counselors to the MHRI service teams, 2.) Project Description Wisconsin Department of Health Services (WDHS) has joined with the Wisconsin Department of Veterans Affairs (WDVA) to create the Veterans Outreach and Recovery Program (VORP). This comprehensive coordinated outreach, treatment and recovery support program for veterans who are homeless and have a behavioral health disorder will supplement the existing treatment and homeless programs in northern Wisconsin, and establishing an Outreach and Recovery Specialist (ORS) in each of the six identified northern regions where veterans have difficulties accessing adequate housing and treatment. The ORS will establish key local contacts including county homeless, behavioral health and veterans groups to encourage referrals of veterans to be screened for behavioral health needs and enrolled in VORP. VORP takes a resourceful approach to address the needs of homeless veterans with behavioral health needs in rural and under-resourced parts of the state. This enhancement will expand services to the west and provide additional services to the northern counties already covered under the VORP program. Expanded services include hiring a SSI/SSDI SOAR Specialist, better utilizing the Disabled Veterans Outreach Programs (DVOP) and Local Veterans' Employment Representatives (LVER) program, and adding additional Outreach and Recovery Specialists (ORSs). Short Title: CABHI-States-Enhancement
Project Description Wisconsin Department of Health Services (WDHS) has joined with the Wisconsin Department of Veterans Affairs (WDVA) to create the Veterans Outreach and Recovery Program (VORP). This comprehensive coordinated outreach, treatment and recovery support program for veterans who are homeless and have a behavioral health disorder will supplement the existing treatment and homeless programs in northern Wisconsin, and establishing an Outreach and Recovery Specialist (ORS) in each of the six identified northern regions where veterans have difficulties accessing adequate housing and treatment. The ORS will establish key local contacts including county homeless, behavioral health and veterans groups to encourage referrals of veterans to be screened for behavioral health needs and enrolled in VORP. VORP takes a resourceful approach to address the needs of homeless veterans with behavioral health needs in rural and under-resourced parts of the state. This enhancement will expand services to the west and provide additional services to the northern counties already covered under the VORP program. Expanded services include hiring a SSI/SSDI SOAR Specialist, better utilizing the Disabled Veterans Outreach Programs (DVOP) and Local Veterans' Employment Representatives (LVER) program, and adding additional Outreach and Recovery Specialists (ORSs). Project Description In 2014, the Connecticut Department of Mental Health & Addiction Services (CT DMHAS) initiated the Critical Time Intervention Plus (CTIP) program for people experiencing chronic homelessness to provide housing, income, and recovery supports to 432 individuals 3 areas of the state. This expansion would allow us to cover the remainder of the state; to serve 380 additional individuals; and to improve services through training in trauma and wellness. CTIP offers a new approach. With additional funding from SAMHSA, CT DMHAS will create 4 new CTIP teams for the state, located within experienced supportive housing organizations. These teams will provide four services, in any needed combination, together with existing or new permanent supportive housing and behavioral health care resources, to ensure that participants make a successful transition to independent housing and maintain that housing over time. The four elements of CTIP are: (1) CTI, which offers a structured move to housing through 9 months of transition, try-out, and transfer of care; (2) SSI/SSDI Outreach, Access & Recovery (SOAR), which facilitates applications for disability entitlements; (3) Supported Employment, to connect participants who are able to work with jobs; and (4) Peer Recovery Specialists, who will use their personal experience and specialized training to provide on-going support and encouragement. Project Description MISSION: Housed Enhancement will enhance and expand the MISSION model services and infrastructure changes provided by MISSION: Housed. The project will continue to integrate Critical Time Intervention case management, Integrated Mental Health and Substance Abuse Treatment, Peer Support and Trauma Informed Care as evidence-based practices. The expansion of services will include expanding the service delivery for current participants to two years. New participants will be referred from standing lists of Boston's most vulnerable homeless and from the New England Center for Homeless Veterans. New participants will have been recently housed by the City and by the New England Center for Homeless Veterans. Services will be enhanced through Supported Employment Specialist sited with each service provider, trained to work specifically with the homeless population with co-occurring disorders, starting with MISSION: Housed participants. A full-time SOAR Specialist will improve the utilization of the SOAR model to facilitate access to SSI/SSDI by training case managers across the state in use of the SOAR model, tracking applications and providing technical assistance to increase successful applications. MISSION: Housed Enhancement will hire an experienced professional to work directly with a work group within the Office of Medicaid to develop a new or improved Supportive Housing Services benefit. These enhancements to the current H3Ok program will support Oklahoma to create a stronger state level infrastructure and renewed vision shared with the Governor's Interagency Council on Homelessness (GICH) to end homelessness in Oklahoma. The addition of SOAR and supported employment training will allow the case managers on the CABHI team to work concurrently with consumers to attain housing, supportive services, employment and/or benefits in an integrated team approach. The goal is to serve 150 persons annually and during the course of this two year grant cycle to serve a total of 300 persons. The Dartmouth IPS Supported Employment Center will be the agency providing training and consultation to the staff using the Individual Placement and Supports (IPS) model of supported employment.
