CFBHN will pilot evidence-based services for this population in Hillsborough, Lee, and Collier counties and collaborate with DCF and state and local stakeholders to address policy and procedural issues relating to the transition of young people with serious mental health conditions to adulthood.

The Florida Youth Transition to Adulthood project will be implemented through state and local partnership. DCF will contract with Central Florida Behavioral Health Network (CFBHN), a full service Managing Entity (ME) that manages State child and adult behavioral health treatment, prevention, and social service contracts in west central Florida, to manage grant activities. CFBHN will pilot evidence-based services for this population in Hillsborough, Lee, and Collier counties and collaborate with DCF and state and local stakeholders to address policy and procedural issues relating to the transition of young people with serious mental health conditions to adulthood.
