Short Title: NITT-Healthy Transitions (NITT-HT) The Oklahoma Department of Mental Health and Substance Abuse Services (ODMHSAS) will implement the Oklahoma Now is the Time (ONITT) Initiative. The ONITT will plan, develop and implement a continuum of education, outreach, identification and linkages to effective treatment and support for youth and young adults ages 16-25 that either have, or are at risk of developing a serious mental health or co-occurring condition. The ODMHSAS will partner with Okmulgee, Washington, and Oklahoma Counties as local laboratories to develop the continuum referenced above, including a practice model customized for youth and young adults in transition (YATs). YATs will be served through outreach and screening activities and have access to a Transition to Independence Process (TIP) TIP-Informed Wraparound approach or a TIP option. Project goals relate to increased awareness and understanding of symptoms for early detection of serious mental health conditions, the delivery of effective outreach strategies, and engagement and linkage for YATs to effective treatment. ONITT strategies include a multi-media educational campaign, school campus and street outreach, and engagement activities offered throughout the communities by service providers and advocacy organizations.

Short Title: NITT-Healthy Transitions (NITT-HT) The Moving Forward (NITT-HT) Initiative will address the Governor's priority of improving outcomes for youth of transition age by helping youth and young adults with or at risk of serious mental illness and co-occurring disorder achieve independence by addressing education, housing, employment, relationships, and other needs using evidence-based practices. The Initiative will serve youth and young adults ages 16 - 25, who are experiencing (or at risk of) serious mental health conditions, substance use, at risk of suicide, and who are disconnected from their families or at risk of homelessness in Androscoggin and Cumberland Counties. Moving Forward (NITT-HT) will implement: 1) Transition to Independence (TIP), an evidence -informed practice which emphasizes youth-directed planning and development of practical life skills leading to independence; 2) Portland Identification and Early Referral (PIER) program and evidence-based intervention (Family-aided Assertive Community Treatment [FACT]); 3) Trained Peer Youth Specialists to support youth to reach their goals; and 4) a Learning Collaborative among the two sites. Additionally, the Initiative will identify and address systemic issues bridging the children and adult mental health systems such as funding, policy differences, structures and eligibility criteria, service philosophies and treatment approaches.

The Creek Nation Ensuring Hope project shall define the need for services, the gaps between needed and available services, barriers to care, and other problems related to the need to implement suicide and substance abuse prevention and early intervention activities for American Indian youth and emerging adults at risk of or currently experiencing issues that may lead to suicide. The project shall involve communities, assess needs, identify organizations, improve coordination, implement evidence-based strategies, and manage youth at risk for suicide within the tribal service area. Further, the Creek Nation shall enlist area communities, tribal members, youth, area child-serving agencies, educational institutions, Creek Nation health facilities, and public schools in the planning, assessment, implementation, and evaluation phases of this project. The result will raise awareness, improve the continuity of care, engage communities, and save lives. Short Title: State/Tribal Youth Suicide Prevention Cooperative Agreements Arkansas is the only state in the nation that has never been awarded this funding. The purpose of this proposed project is to save the lives of at-risk youth in Arkansas. Within the total population of youth ages 10-24, several groups face heightened risk, including those age 18-24 who have the highest rates of suicide, as well as those identified as high risk by the National Action Alliance for Suicide Prevention including, but not limited to: Latina youth; lesbian, gay, bisexual, transgender, or questioning (LGBTQ); American Indian and Alaska Natives; military family members; and veterans. Other high-risk groups include individuals with disabilities, survivors of suicide attempts and of suicide loss, and young working-aged men and women. From these groups we form our population of focus. Following the goals of the National Strategy for Youth Suicide Prevention, the overarching goals of this project are as follows: 1) build youth suicide prevention capacity across service sectors; 2) provide early identification training for caregivers and professional gatekeepers; 3) improve suicide risk assessments, management, and treatment through use of evidence-based practices; 4) provide resiliency training for adolescents; 5) increase public awareness, promotion, and use of the National Suicide Prevention Lifeline; 6) improve continuity of care and follow-up of youth identified at risk for suicide; and, 7) promote state systems-level change. Cultural competence on ethnicity, sexual orientation, and military culture issues are built into proposed interventions.
