Project Description The Wellness and Recovery Medicine (WaRM) Center at the Massachusetts Department of Mental Health (DMH)'s Massachusetts Mental Health Center (MMHC) is a public-sector Health Home for adults with serious mental illness, providing on-site primary care and wellness services and facilitated access to key subspecialty medical care, all fully integrated and coordinated with comprehensive, recovery-oriented and person-centered mental health services. MMHC serves 1,300 DMH service-eligible adults, all of whom have a serious mental illness and most (50-60%) of whom have a schizophrenia-spectrum disorder. Eighty percent are below 200% of the Federal Poverty Level. The population represents the diversity of the metro-Boston catchment area: 62% male, 49% White, 28% African American/Black, 7% Hispanic/Latino/a, and 9% whose preferred language is Spanish. Individuals served by MMHC have rates of cardiovascular disease risk factors 1.6 to 2.8 times higher than the general population of Massachusetts. Despite high rates of physical health co-morbidity and resulting early mortality, many lack meaningful access to primary care and wellness services and have elevated rates of utilization of acute care medical services. Utilizing evidence-based programs targeted toward tobacco cessation, peer delivered health promotion and disease prevention, and chronic disease management. We plan to serve 185 persons in year 1 of the grant period, 580 in year 2, 1,035 in year 3, and 1,330 in year 4. The majority (at least 58%) of the persons with SMI to be served across the City of Charlottesville and the counties of Albemarle, Fluvanna, Greene, Louisa, and Nelson is low income, is predominantly White or African American (65% and 26%, respectively), and is largely between the ages of 18-39 (41%). (CHESI) to implement Health Home Illinois (HHIL), a co-located integrated health home for 225 adults (18+) with serious mental illness (SMI) and chronic, co-morbid health conditions over 4 years (Year 1: 45; Year 2: 113; Year 3: 180; Year 4: 225) in rural Williamson County, Illinois. HHIL will implement the Reverse Co-location integration model and 3 promising wellness interventions. Project Description Lakes Region Mental Health Center, Inc. (d/b/a Genesis Behavioral Health, GBH) will collaborate with its project partners, Health First Family Care Center (HFFCC) and Mid-State Health Center (MSHC) to develop OneHealth, an integrated model of care that will become a person -centered behavioral health home for persons with serious mental illness (SMI) and/or co- occurring substance use disorders. The population of focus is individuals with SMI and/or co- occurring substance use disorders who have or are at risk for developing co-morbid primary care conditions and/or chronic diseases. GBH currently has 860 adult patients with SMI. One-third of the current caseload reports an Axis III disorder. Thirty-five percent of current individuals with SMI entered services without insurance. Less than 20% are noted in the EHR as having a primary care provider, and most who reported having a PCP are patients of the project partners, poising this collaboration to achieve improved outcomes, integration, and communication. Of the individuals to be served, at least 50% will have a co-occurring substance use disorder, more than half will be female, and at least 75% will be under the age of fifty. Project Description Catholic Family Center (CFC) project will create a seamless and stable network of behavioral health treatment, primary health care and health (wellness) promotion services that will improve health outcomes for individuals with severe mental illness who may also have co-occurring substance use disorders. The coordinated physical and behavioral health care which CFC is proposing to implement, in tandem with intensive Health Home care coordination and case management, will result in the following for program participants with SMI: an increase in stable primary health care, a decrease in hospital admissions and readmissions for the individuals we serve, a decrease in unnecessary ED visits, and an increase in overall health. These achievements will also result in a decrease in the per capita cost of care. Our project goals are centered on three pillars of wellness: Provider integration, Wellness promotion, and Recovery Support = PWR. Our agency is well-positioned and qualified as our City's largest human services provider; providing direct services to 34,000 in 2014. CFC's mental health services included an outpatient clinic and satellite clinic for those with co-occurring mental health and substance abuse disorders. Project Description Clay Behavioral Health Center (CBHC) is excited to expand our services in Clay County by collaborating with the Florida Department of Health and other primary care providers in the community to offer integrated physical and behavioral health care to individuals with serious mental health and substance abuse issues. Because everyone matters, we are pleased to partner with SAMHSA in offering services to an often overlooked and underserved population. Through a Primary and Behavioral Health Care Integration grant, the HEALTH MATTERS program will serve at least 460 adults (approximately 115 unduplicated clients each year) with serious mental illnesses (SMI) over a four year period with an expectation to expand services to all CBHC clients, after the four year period. Reducing health disparities among subpopulations will be a focus of the program. Adults with serious mental illness have been shown to have heightened morbidity and mortality, in part due to evaluated incidence and prevalence of obesity, diabetes, hypertension and dyslipidemia. Project Description Clay Behavioral Health Center (CBHC) is excited to expand our services in Clay County by collaborating with the Florida Department of Health and other primary care providers in the community to offer integrated physical and behavioral health care to individuals with serious mental health and substance abuse issues. Because everyone matters, we are pleased to partner with SAMHSA in offering services to an often overlooked and underserved population. Through a Primary and Behavioral Health Care Integration grant, the HEALTH MATTERS program will serve at least 460 adults (approximately 115 unduplicated clients each year) with serious mental illnesses (SMI) over a four year period with an expectation to expand services to all CBHC clients, after the four year period. Reducing health disparities among subpopulations will be a focus of the program. Adults with serious mental illness have been shown to have heightened morbidity and mortality, in part due to evaluated incidence and prevalence of obesity, diabetes, hypertension and dyslipidemia. The goal of the program is to improve the physical health status of adults with SMI and those with co-occurring substance use disorders who have or are at risk for co-morbid primary care conditions and chronic diseases.
