Application Forms & Resources

If you are applying for a SAMHSA grant, you must download the application forms found on Grants.gov. You may require additional forms found on the SAMHSA website. Read the Notice of Funding Opportunity (NOFO) carefully to determine which forms you must include in your application. Applications that do not include all required forms will be screened out and will not be eligible for review.

Budget Templates and Sample SF-424 and SF-424A Forms

All applications must include a detailed budget and narrative that explains the federal and the non-federal expenditures broken out by the object class cost categories listed on SF-424A − Section B (Budget Category) for non-construction awards.

SAMHSA Budget Template

To expedite review of your application, it is highly recommended you use the following PDF budget template to complete the Detailed Budget and Narrative for submission with your application:

Use the following instructions for successful download.

  • Right click the above “SAMHSA Budget Template (PDF | 1.2 MB)” link to access the shortcut menu. Note: DO NOT click the link, as this will open the budget template directly in your browser and generate a “Please wait …” message.
  • Click "save link as" to save the document to a folder located on your computer.
  • Go to the folder on your computer where the file was stored
  • Right click the file, select “open with” and choose your adobe software from the list to open the file locally on your computer. Note: If your browser is set as the default application for opening Adobe file types, double clicking the file may result in the “Please wait …” message.

Assurances and Certifications

When the Authorized Organization Representative (AOR) signs the Application for Federal Assistance SF-424 form, they certify that the statements contained in the SAMHSA List of Certifications (PDF | 11 KB) are true. Their signature also certifies that they agree to comply with the Assurances for Non-Construction Programs (SF-424B) (PDF | 65 KB).

Charitable Choice Form SMA 170

You may be required to submit an Assurance of Compliance with SAMHSA Charitable Choice Statutes and Regulations form SMA 170 (PDF | 23 KB). Check the NOFO to determine if this form is required for the grant program for which you are applying.
