SAMHSA-Recognized Electronic Custody and Control Form Provider List

All Department of Health and Human Services (HHS) certified test facilities (laboratories and Instrumented Initial Test Facilities - IITFs) must submit a request for approval of a digital electronic Federal Custody and Control Form (ECCF) by August 31, 2026. The Substance Abuse and Mental Health Services Administration (SAMHSA) has taken several actions to facilitate compliance, including instituting an ECCF System Recognition program for standalone ECCF systems (i.e., ECCFs that can be used by any certified laboratory or IITF) that meet program requirements.

Before an HHS-certified test facility can use a Federal ECCF for regulated specimens, the test facility must submit a detailed plan and proposed standard operating procedures (SOPs) for the ECCF system for HHS review and approval (through the National Laboratory Certification Program - NLCP). Proposed systems must be reviewed on-site by the NLCP prior to implementation.

SAMHSA recognizes these ECCF systems that meet program requirements. This list does not constitute approval for any test facility to use the ECCF system. Laboratories and IITFs may work with these ECCF providers and use an abbreviated process for SAMHSA approval to use that ECCF.
